We are seeking Primary Care Physicians to join our growing, outpatient practices in Glendale, Colorado. Our Team Physicians work closely with the Medical Directors to play a critical role in both care delivery and ongoing practice innovation. What you'll likely work on

• Provide best in class comprehensive primary care in an outpatient setting.
• Partner with the care team and leverage One Medical Seniors coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change.
• Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.

• Weekly protected time during the work day, to gather with fellow One Medical Senior providers for team building and shared discussion of challenging cases.

What you'll need:

• At least 2 years of primary care experience required.
• Currently licensed or ability to obtain licensure in the state of Colorado.
• The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups.
• Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes.
• Board Certification in Internal or Family Medicine required.

The base salary range for this role is $249,000 to $265,000 based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. POSITION SUMMARY: Assists with the delivery of direct, non-licensed patient care in accordance with established methods and procedures, under the supervision and guidance of appropriate personnel.

• Demonstrates positive interpersonal skills when dealing with patients, family members and staff.
• Functions as a patient advocate by resolving patient/family concerns through appropriate actions.
• Assists with admission, transfer, and discharge procedures.
• Answer patient call lights independently, promptly, and to the patient's satisfaction; referring patient concerns to the RN or Nurse Manager.
• Provide close observation and care safely for patients who require 1:1 observation.

• Obtains and accurately documents all required patient information; reporting changes and abnormal findings to the RN in a timely fashion.
• Perform unit-specific lab tests as dictated by the Point of Care Testing lab standards (Glucose, stool, and emesis) record results, and report findings to the RN. Perform quality control measures on unit assigned tests and equipment.
• Properly label specimen(s) and form(s) for lab test completion.
• Participates in achieving institution wide and unit based quality outcomes.
• Performs duty/job with appropriate level of independence.
• Performs duty/job with accuracy and efficiency.
• Accepts accountability, as an individual and as a team member for all services provided.

• Participates in the on-call responsibilities for this position as required.
• Other duties as assigned.

• HS diploma or equivalent (GED).
• Completion of the educational requirements (additional formal education/training; maximum supervision for initial competency; high degree associated risk factors; yearly competency evaluation and training).
• AHA CPR/BLS certification required within 30 days of hire.

Some details about this position:

• No Dialysis experience is required.
• Training may take place in a facility or a training clinic other than your assigned home clinic.
• Potential to float to various clinics during and after your training.
• You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays.

What you can expect:

• Direct Patient Care. Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.
• Our PCTs care for multiple patients.
• Technician Duties. To monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. Please note you will have exposure to blood and needles.
• Building long-term relationships with your patients who are suffering from end-stage renal disease and are receiving treatment on average 3 times/week. You will also be responsible for educating patients on Kidney Dialysis related topics.
• Team. Cross functional team of clinicians including technicians, nurses, dietitians, social workers and leadership. PCTs work under the supervision of a RN.

Requirements:

• Desire to enter the health care field to care for other people in need
• High school diploma or equivalent
• Must be comfortable working around blood and needles
• Must be comfortable mixing acid or bicarb
• Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet so wear comfortable shoes! Our work ends when it's safe for our patients.
• Willingness to train and work across multiple clinics within the territory as needed.
• Health care experience a plus including: BONENT Certified, Certified Hemodialysis Technician (CHT), Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or Phlebotomy certification.
