POSITION SUMMARY
The Health First Homecare (HFHC) Registered Nuse (RN) Intake Coordinator facilitates admission and access to Health First Homecare services through clinical referral review, care management coordination, and collaboration with referral sources. The RN Intake Coordinator supports the work of the Home Care Clinical Liaisons, the Hospital Division Care Managers, and the Integrated Delivery Network (IDN) of licensed practitioners to ensure safe transitions of care to home health.
PRIMARY ACCOUNTABILITES
* Performs individualized clinical assessments of patient's medical records to ensure safe transition of care from hospital to the outpatient environment.
* Administers patient assessments in a designated time frame to include health status, care gaps, and barriers to health success in Home Care.
* Communicates with all referral sources to gather demographics, diagnoses and clinical information, surgeries, physician's orders, and reimbursement or insurance information to facilitate appropriate continuity of care for patients and clients.
* Utilizes clinical nursing skills and judgment in a collaborative process to identify and verify appropriateness of patients for home health level of care and facilitate exceptional healthcare outcomes.
* Serves as an educational resource for referral resources, patients, and family members throughout the home health referral process.
* Builds relationship and communicates with patients and families regarding all aspects of home care, including coordination with community partners for complex discharge needs; maintains exceptional on-going communication and documentation regarding referral status and final disposition.
* Collaborates with clinical team, nursing supervisors and scheduling in the coordination of appropriate home health referrals, and establishment of an individualized plan of care.
* Meets monthly monetary targets in collaboration with liaison team to enhance business development opportunities.
* Captures potential patients and referral sources by outlining the services and benefits that differentiate HF Homecare from the competition.
* Identifies needs and opportunities for other services within the IDN and facilitates referrals for alternate services.
* Reports daily on all assigned metrics to Home Care leadership team.
Work Experience
MINIMUM QUALIFICATIONS
* Education: Associate degree in Nursing or Nursing diploma.
* Work Experience: Two (2) years of acute care experience as an RN.
* Licensure: Current RN Licensure in the State of Florida or endorsement.
* Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Care prior to start date and maintain.
* Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.
* Skills/Knowledge/Abilities:
* Excellent written and oral communication skills.
* Strong computer and typing skills with proficiency in Microsoft Office Suite.
* Self-directed with ability to function autonomously.
PREFERRED QUALIFICATIONS
* Education: BSN
* Work Experience: One (1) year home health nursing or case management experience.
* Skills/Knowledge/Abilities:
* Knowledge of ICD-10 codes, PDGM and Medicare qualification for home health admission.
* Knowledge of Medicare/The Joint Commission regulations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.
• Sets up, tests, and operates hemodialysis machines for patient treatments.
• Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
• Evaluates vascular access pre-treatment and performs vascular access cannulation.
• Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
• Monitors patients’ response to dialysis therapy.
• Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
• Reports any significant information and/or change in patient condition directly to supervisor.
• Enters all treatment data into the designated clinical application in an accurate and timely manner.
• Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
• Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
• Assists other healthcare members in providing patient education.
• Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
EDUCATION AND LICENSES :
• High school diploma or G.E.D. required.
EXPERIENCE AND REQUIRED SKILLS :

• Previous patient care experience in a hospital setting or a related facility (preferred but not required).

This position is responsible for: To perform as a customer service representative for assigned divisions accurately obtaining and entering all essential information needed to complete registration and scheduling processes following standard business practices. Provide service to internal and external customers as a member of the clinic team.
- Customer service: answer phone lines in a prompt and courteous manner, triage call and handle appropriately.
- Registration: collect, maintain and ensure accuracy of information through the use of standard business practices.
- Coordinate all necessary paperwork for registration, scheduling, and appointment.
- Financial: collect, verify and maintain patient insurance information, including authorizations and referrals.
- Collect co- pays, deductible, co-insurance and balances on accounts. End of day balancing.
- Scheduling: understanding visit types, templates and fundamentals of how to efficiently schedule an office visit. Manage no-show and cancelation process. Confirm appointments with patient/guardian. Prepare charts with the appropriate paperwork for the physician visit.
- Communication: triage and manage electronic communication daily. Scan all new paperwork into EMR or place in patient chart. Scan incoming mailed and faxed patient results into EMR and task the physician responsible for reviewing OR place information with patient chart and for provider to review. Check in and checkout patients in office.
- Coordinates the release of information and medical record process. Process all incoming and outgoing medical record requests.
- Ensures coding information is complete on billing documents.
- All other duties as assigned by supervisor.
Job Requirements
- High School Diploma required.
- Specialized (1 year of training beyond high school).
- Minimum of one (1) to three (3) years' experience preferred.
- Customer Service and Healthcare experience preferred.
- Medical Office/Call Center experience preferred.
- Travel to other primary care locations as needed for coverage is required.

* Sterilizes all equipment/instruments using proper techniques, as well as technical usage of all other standard equipment used at the Wound Clinic/and or Physician office.

* Provides leadership by working cooperatively with ancillary nursing and their patient team personnel in maintaining standards for professional nursing practice in the clinical wound care setting.
* Assists in use of equipment and materials in a cost-effective manner.
* Completes all documentation supporting billable activities in a timely manner.
* Maintains documentation and assessment of patients' vital signs, drug allergies and description of patients' complaints on all charts.
* Under the supervision of an RN, case manages patients with their wound care.
* Applies dressings as ordered per the physician.

* MINIMUM QUALIFICATIONS
* Education: Licensed Practical/Vocational Nurse licensure from a credentialed program supported by the Florida Board of Nursing and obtained prior to hire date or job transfer date.
