**Responsibilities:**
GENERAL SUMMARY
The Sr Public Health Project Specialist organizes, coordinates and supports all work to be done in completion of the "Reducing Maternal Mortality: Informing Targeted Universal Interventions" grant through the Monroe County Mothers and Babies Health Survey 3.0 funded by ARPA through the Monroe County Department of Public Health. The specialist will assure the day-to-day operations of the project and will be responsible for meeting project goals and supervising the students to oversee the survey distribution and work with the MCDPH to develop and populate a birthing mothers database. This work is done in collaboration with the Primary Investigator, Theresa Green PhD, MBA, Associate Professor in the Public Health Sciences Department.
**ESSENTIAL FUNCTIONS**
+ Planning and coordination of the efforts for interventions identified in the grant proposal.
+ This may involve researching best-practices and evidence-based interventions, attending community meetings and forums, meeting with hospital and community leaders to assure implementation, and evaluation of the outcomes for quality improvement.
+ Coordinate the progress on implementation, track and evaluate the projects, note barriers and successes to the team.
+ Complete all required forms and reports required for the grant.
+ Oversight of the data collection and coordination process in collaboration with the Monroe County Department of Public Health.
+ All birthing mothers in Monroe County will be entered into a database and given a unique number so that identifying information can be held in confidence.
+ The database will be managed by an epidemiology student and a data scientist under the direction of the Specialist to track survey completion and to connect with birth certificate data.
+ Oversight of master and graduate students for the original mailing, and survey distribution and collection.
+ The Specialist will oversee the creation and mailing of the original packet of resources and survey link to new families.
+ Under the Specialist's guidance, students will follow up to assure the maximum number of surveys are completed.
+ The Specialist will organize and deliver community sharing sessions where results of the survey responses are shared with community members for reaction and validation.

**MINIMUM EDUCATION & EXPERIENCE**
+ Bachelor's degree with major coursework in appropriate health, social science, public health, or closely related field. Required

+ Planning and coordination of the efforts for interventions identified in the Community Health Improvement Plan (CHIP).
+ This may involve researching best-practices and evidence-based interventions, attending community meetings and forums, meeting with hospital champions to assure implementation, applying for grants and programs relevant to the implementation plan, and evaluation of the outcomes for quality improvement.
+ Coordinate the progress on implementation, track and evaluate the projects, note barriers and successes to the team.
+ Oversight of the creation of the Monroe County Community Health Needs Assessment (CHNA) and the Community Health Improvement Plan (CHIP) every three years in response to federal and state regulations for non-profit hospitals.
+ The CHNA/CHIP requires population data analysis, priority assessment, community input and establishment of objectives and metrics.
+ Report on CHNA/CHIP to hospital leadership teams.
+ Maintains and implements compliance with annual reporting requirements to the state department of health and federal government through tax submissions.

Establish an annual budget, invoice hospitals for resources, and manage the finances of the CHIW. Oversee a "mini-grant" process to provide funding for exceptional community-based proposals.
+ Report on the progress of the mini-grantees and the CHIW to stakeholders.
+ Provide leadership and content expertise to the Center for Community Health & Prevention (CCHP) for community health needs and emerging topics in community health.
+ Connect CCHP directors to community-based agencies working in topic areas of interest. Inform CCHP faculty and staff of community initiatives such as the 1115 Waiver, Doula reimbursement changes, etc.
+ Stay informed and involved with important community-based agencies and initiatives.
+ Work with the Director to assist in the UR Strong community benefits reporting annually.

**Responsibilities:**
Provides professional comprehensive care coordination services to patients of the Golisano Children's Hospital. + Works in collaboration with Complex Care clinical team, monitors utilization of services and encourages enrollees to follow treatment recommendations, ensures that care is accessible, attended and effective. Conducts routine follow-up, with patients, to ensure patient needs are met.
+ Interacts with patients via telephonic outreach and utilizes telemedicine capabilities to participate in patient enrollment visits.
+ Coordinates outreach and engagement activities focused on finding, enrolling and retaining patients in Complex Care Services.
+ Other duties as assigned
**MINIMUM EDUCATION & EXPERIENCE**
+ Bachelor's degree in healthcare related field required or equivalent combination of education and experience
+ 1-year experience in community health, human services, and/or patient liaison/advocate roles preferred
**KNOWLEDGE, SKILLS AND ABILITIES**
+ Excellent customer service communication skills, ability to prepare presentation materials and present to small groups, computer and organizational skills

Provides professional comprehensive care coordination services to patients of the Golisano Children's Hospital. Collaborates with health, behavioral health and social service providers and is responsible for assessing patient's needs, enrolling new patients in the Complex Care Program, and coordinating supportive services as needed. Special focus will be serving the most complex, high risk patients enrolled in the Complex Care Program. Services include, but are not limited to: care coordination, heath promotion, comprehensive transitional support, enrollee and family support, referral to community and social supports, use of technology to link services.
**ESSENTIAL FUNCTIONS**
+ Develops, reviews and discusses plans with patient and care team, focusing on linking individuals to Complex Care services, as well as clinical and social services within URMC and outside community providers.
+ Works in collaboration with Complex Care clinical team, monitors utilization of services and encourages enrollees to follow treatment recommendations, ensures that care is accessible, attended and effective. Conducts routine follow-up, with patients, to ensure patient needs are met.
+ Interacts with patients via telephonic outreach and utilizes telemedicine capabilities to participate in patient enrollment visits.
+ Coordinates outreach and engagement activities focused on finding, enrolling and retaining patients in Complex Care Services.
+ Other duties as assigned
**MINIMUM EDUCATION & EXPERIENCE**
+ Bachelor's degree in healthcare related field required or equivalent combination of education and experience
+ 1-year experience in community health, human services, and/or patient liaison/advocate roles preferred
**KNOWLEDGE, SKILLS AND ABILITIES**
+ Excellent customer service communication skills, ability to prepare presentation materials and present to small groups, computer and organizational skills
