Position Summary:
The RT Vent Field Clinician is a Respiratory Therapist providing respiratory patient care to Vent patients for optimal outcomes. Provides respiratory care to patients in alternate sites in accordance with AdaptHealth's policies and procedures. Respiratory care will be preventative, rehabilitative, and palliative in nature. The RT will utilize all the resources available within the agency and community to accomplish care objectives. This position will provide education and care to the patient and communicate with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.
Essential Functions and Job Responsibilities:
- Utilizes various sources of information to attain greater competence about his or her position, including attending educational events (including attending optional in-services) and asking questions.
- Utilizes acquired knowledge to increase his or her competencies.
- Consistently demonstrates ability to adequately complete all documentation and charting procedures in compliance with company policy and procedures.
- Maintains complete and accurate patient files by updating all documents per company policy and procedures.
- Reviews Plan of Treatments and Care Plans to assure they are accurate and up to date.
- Documents procedures including how the patient tolerated a procedure, side effects and other pertinent information.
- Assists with authorization for Ventilator referrals for patients.
- Shows adequate knowledge of respiratory equipment and displays ability to utilize knowledge in the clinical setting.
- Displays knowledge of assessment skills and demonstrates application of clinical skills during set-ups, follow-ups, and in-services.
- Participates in discharge planning of highly technical cases.
- Performs clinical assessments as needed and reports results and recommendations to the referral and physician.
- Participates in highly technical discharges and prepares in advance to assure the patient and caregivers have a smooth transition to the home setting.
- Performs in-services to hospital staff, referrals, other professionals regarding equipment & issues of clinical nature.
- Follows up with physician and referrals regarding patient status and documents accurately and in a timely manner.
- Retain knowledge of and consistently adhere to procedures for the use of Personal Protective Equipment (PPE), infection control and hazardous materials handling.
- Works to promote AdaptHealth by new program development, operational backup, personal visits, coordination of educational activities, etc.
- Assume on-call responsibilities during non-business hours in accordance with company policy.
- Uses clinical expertise in evaluating vent patients records once a ventilator set up has been completed by the branch Respiratory Therapist.
- Ensures accuracy of prescriptions and plan of care was followed and documented. Also reviews delivery tickets, home inspection, ventilator check, and patient equipment competencies are complete and documented.
- Maintains proficient knowledge of ventilator patients including compliance software, new technology, units, and supplies supported by Adapthealth.
- Ability to demonstrate and instruct on use of vent units and supplies. Ability to make decisions for patients based on compliance data and assessment.
- Communicates with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.
- Maintains working knowledge of Medicare/Medicaid and other third-party payer guidelines related to ventilation.
- Electronically documents patient care activity, intervention provided and all communication regarding the patient. Documentation is accurate, complete and follows company standards.
- Appropriate steps taken to ensure recommendations and orders sent are acknowledged and followed up in a timely manner.
- Responsible for accuracy, clarity, and timeliness of verbal and written communications as it relates to role.
- Responsible for documentation that supports data collection to track and trend outcomes.
- Assists in establishing clinical documentation when needed for third party reimbursement or justification.
- Uses knowledge in working with referral sources to educate about best practice standards.
- Works collaboratively and pro-actively with peers and other team members to resolve issues and assure optimum outcomes for patients, referral sources and staff.

Education and Experience Requirements:
- Associates degree from an AMA approved respiratory program,

- One (1) year of clinical experience as a Health Care RT, HME RT or clinical nursing with Vent experience.

Physical Demands and Work Environment:
- Must be able to lift 50 pounds, stand, bend, stoop, and be able to sit at a computer for extended periods of time.

POSITION SUMMARY

Summary
This position manages a caseload of participant cases by administering the policies and procedures of the program as directed by the contracting agency and the Program Manager. Learns, uses, and models principles of the "What Works" system through EBPs in all interactions with participants and other staff members. This position reports on program operations to ensure that all requirements of the criminal justice system are met.
Primary Duties and Responsibilities
- Manages a participant caseload using principles of the "What Works" system. Conducts risk/need assessments and creates individual treatment plans for each assigned participant. Meets regularly with participants and documents participants' progress or regress in Company database.
- Facilitates evidence-based cognitive-behavioral therapy (CBT) programs and other life skills programs to participants groups each week. Documents participants' attendance, participation and progress in Company database.
- Conducts orientation meetings with new participants to explain program rules and regulations. Verbally explains the participant contract in detail to ensure that the participant understands his/her responsibilities.
- Sets participant schedules and specifies all approved activities based on sentencing information. Approves temporary schedule changes for approved activities.
- Monitors participant compliance with alcohol or drug counseling when required.
- Reports all program violations to the appropriate agency or official via the established reporting parameters.
- May review daily summaries each morning and forward reports to the appropriate agency as required.
- Reports on the status of program operations regularly to ensure all necessary documentation is prepared for court review.
- Represents the company in contacts with elected officials and other public figures in the criminal justice system.
- Assists Employment Placement Specialist in verifying to verify participant's jobs to ensure participants' employment is suitable under established program plan.
- Counsels unemployed participants by providing guidance in obtaining employment in cooperation with the Employment Placement Specialist.
- Verifies participants' pass plan through pass sponsorship according to contract so that the participant is accountable for his/her whereabouts in the community.
- Prepares terminal reports within established time parameters upon participant release to summarize program performance and to satisfy contract requirements.
- Conducts group and individual meetings utilizing motivational interviewing techniques.
- Works with participants to develop and implement a rehabilitative plan.
- May testify in court regarding participants' compliance with court orders.
- May screen participants at jail for entry into the program.
- Provide face-to-face services to the day reporting center participants.
- May be required to learn and develop proficiency in Case Coordinator duties, including opening and closing the office, and performing participant intakes, check-ins, breath analysis and drug screens, to provide backup when necessary.

PURPOSE
Bayer is expanding in its efforts to advance its mission "Health for All, Hunger for None." The Women's Health Specialist (WHS) will be responsible for managing the Intrauterine Devices (IUDs) business portfolio as well as driving the launch objectives for a new therapy in the retail space. Key responsibilities include driving demand, providing clinical education/training, and achieving territory sales objectives by developing, coordinating, and executing a strategic business plan for key customers in both the contracted account and community settings. The WHS will report to the WHC Area General Manager (AGM) and be a key member of the Area Customer Squad, working closely and collaborating with Marketing, Medical, Contracting, Market Access, Patient Services, and other internal teams to achieve US Pharma outcomes. This position covers Oakland, San Jose and the surrounding areas. Candidates must be based within 50 miles of the territory.
YOUR TASKS AND RESPONSIBILITIES

Drive clinical demand, education and sales, building deep understanding of disease state, unmet needs and treatment options to effectively educate healthcare professionals and business stakeholders on the unique benefits of assigned Women's Health products while balancing both efficacy and safety through approved clinical information for healthcare professionals and health systems;

Demonstrate expertise and knowledge of disease state, the marketplace, competitors, industry, and cross-functional activities/plans as well as possess analytical rigor to anticipate and identify business opportunities and challenges;

Prioritize sales and educational activities in accounts that produce highest ROI in both unit and revenue growth;

Partner with the Area Business Development Lead and monitor contract performance and ensure completion of quarterly customer business review meetings on top priority accounts. Support implementation of pull-through initiatives that accelerate demand and contract performance on contracted accounts and also priority accounts such as; IDNs, Academic Medical Centers, etc;

Penetrate established accounts to gain additional market share for product portfolio;

Effectively discover and address account needs with compelling messages and solutions;

Execute at a high level of communication and teamwork within your area squad;

Take ownership and demonstrate being fiscally responsible of your territory budget including Professional Education Budget, Displays/Conference, and all resources;

Dynamic development and execution of a comprehensive strategic and tactical territory sales business plan utilizing data reporting platforms including quarterly plan to achieve overall sales and revenue goals for all promoted products;

Deliver results by collaborating and partnering within your Squad; with Squad leader, peers, Medical Team, and all applicable internal stakeholders to execute/develop/improve and implement sales strategies;

Document all field activities in Veeva Irep;

Demonstrate procedural proficiency and knowledge of clinical data for all products;

Demonstrate procedural proficiency of all pricing and customer access channels that support portfolio;

Partner with targeted physicians/centers to develop referral networks for portfolio;

Employees can expect to be paid a salary of between $112,000 - $168,000. Additional compensation may include a bonus or commission (if relevant). Additional benefits include health care, vision, dental, retirement, PTO, sick leave, etc. This salary range is merely an estimate and may vary based on an applicant's location, market data/ranges, an applicant's skills and prior relevant experience, certain degrees and certifications, and other relevant factors.

Provides mental health assessment, diagnosis, treatment and crisis intervention services for adult and/or child members who present themselves from psychiatric evaluation with a broad range of mental health needs. Collaborates with treating physician, psychiatric and allied health professional team to plan and direct each individual member treatment program.
Essential Responsibilities:

Develops, implements, coordinates, and evaluates clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental illness. Participates in staff conferences to select, plan, and evaluate treatment programs. Provides outpatient psychotherapy to individuals, couples, families and groups.

Instructs and counsels patients and their families regarding compliance with prescribed therapeutic regimens and adherence to prescribed medication regimens, within the scope of practice. May administer specialized therapeutic procedures, as appropriate. Provides appropriate support to members family. May develop and conducts psychoeducational classes and groups.

Prepares intake summaries, treatment plans, and case summaries and maintains ongoing confidential records. Charts members treatment and progress in accord with state and NCQA regulations and in keeping with accepted community standards. May be required to participate in the department on-call rotation.

Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice. Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment. May develop, implements, and evaluates behavioral medicine and health psychology programs in a variety of settings, including primary care. Provides consultation to other care providers and health educators on matters relating to mental health, health psychology and behavioral medicine.
