Overview: Care Coordinator, Youth and Family Support

Pathways to Success

This full-time position will join the Heritage Youth and Family Support Services team to provide direct clinical services to Pathways to Success eligible youth. Pathways to Success is a program for Illinois Medicaid enrolled youth under the age of 21 who have complex behavioral health needs requiring intensive services and provides access to an evidence-informed model of intensive care coordination and additional home and community-based services. Pathways to Success utilizes the evidenced-informed Care Coordination and Support model. This is a structured approach to care coordination that adheres to required procedures for child and family engagement, individual care planning, identifying and utilizing strengths and natural supports while monitoring progress and fidelity to the required process. Services will be provided in array of settings including individual's home, community-settings, community agencies, schools, and Heritage.

Core Responsibilities Include:
  • Develop and update as needed Crisis Prevention Plan and Crisis Safety Plan for each youth
  • Complete and update as needed a Strengths, Needs, and Cultural Discovery for each youth
  • Assemble a unique Child and Family team for each youth including formal and natural supports involved in the youth and family's lives.
  • Facilitate regular Child and Family team meetings for the purpose of identifying the needs and strengths of the youth and family, service planning, coordination of care, and regular review of the child and family's progress toward meeting their goals.
  • Utilize IM+CANS to develop an individualized strengths-based service plan
  • Contact youth as required to maintain program fidelity
Knowledge, Skills and Abilities:
  • Candidates should be energetic and internally driven to assist youth to succeed in obtaining their desired outcomes
  • Organized and able to develop a program plan with goals, objectives, and timeframes that are consistently met
  • Knowledge of community resources and/or how to find
  • Previous experience working with youth is highly recommended
  • Effective oral and written communication skills
  • Ability to work in a team-based environment
  • Strong customer service skills
  • Ability to navigate electronic health record
  • Proficient in Microsoft Word and Excel
Education and Experience:
  • Minimum high school diploma or equivalent is required.
  • Candidates are required to have minimal designation as a Mental Health Professional (MHP).

A Mental Health Professional (MHP) is defined as:
  1. 5 years supervised experience in a behavioral health setting;
  2. Certified Recovery Support Specialist (CRSS)
  3. Bachelor's Degree in psychology, social work, or human services.
  • Valid driver's license, reliable means of transportation, and proof of automobile insurance are required

Salary Range: 23.00 to $26.00 per hour dependent on education and experience

Overview

Our Patient Access Specialist pre-registers and register patients. Schedule patients for procedures and tests at MHS facilities. Collects accurate patient demographic and billing information in a timely manner. Interviews incoming patients or Associates, enter information into potential all appropriate software packages. Serves as a liaison between ancillary departments and other Patient Access Services areas.

Qualifications

Education:
  • High School Graduate or equivalent required .
Experience:
  • One year customer service experience preferred. Previous clerical, medical terminology, medical office, registration or billing experience preferred.
Word processing/computer application experience and knowledge desired.

Other Knowledge/Skills/Abilities:
  • Minimum typing skill of 40 WPM preferred.
  • Demonstrates excellent interpersonal and communication skills.
  • Demonstrates ability to work independently.
Responsibilities
  • Greet the majority of visitors and patients, answer patient questions (via telephone/ in person) and give directional information.
  • Effectively perform general clerical/administrative functions.
  • Responsible for completing all steps of pre-registration/registration including patient interview, obtaining of signatures, providing Advance Directive information and distributes hospital specific literature.
  • Pre-register and register all types of patients in multiple software systems.
  • Demonstrates an ability to be flexible, organized and function well in stressful situations.
  • Maintains a professional demeanor in respect to patients and fellow employees.
  • Ability to conduct financial collections and referrals for Financial Counseling. Ability to interview/prescreen self pay patients for possible financial assistance.
  • Understands and complies with state and federal regulations as well as hospital, department and The Joint Commission policies and procedures related to patient access.
  • Communicates with ancillary department, physicians, medical offices and within Patient Financial Services department.
  • Conducts insurance verification tasks, pre-certification, or referral information from MD offices and/or insurance companies and authorization for elective and emergent patients.
  • Ability to complete legal admission paperwork for psychiatric admits in accordance to DHS guidelines.
  • Ensures accurate documentation of patient information.
  • Responsible for checking and re-stocking supplies as needed.
  • Participates in performance improvement activities for the department and organization.
  • Adheres to all HIPAA guidelines and patient confidentiality policies.
  • Completes annual educational and training requirements.
  • Promotes the mission, vision, and goals of the organization and department.
  • Performs other related work as required or requested.
