Qualifications: Must be at least 18 year of age (21 years old with 1 year experience and good communication skills is preferred); A minimum of a High School Diploma or GED. Summary of Job Duties: The Personal Care Aide is a part time position paid at an hourly rate. The Personal Care Aide will facilitate independence and promote integration into the community for an individual with mental illness.

Essential Duties and Responsibilities:
  • Provide therapeutic training to promote independence according to Individual Plan of Care goals and objectives.
  • Facilitate the individual to be as independent as possible.
  • Assist, observe, and support the individual's personal care needs.

Job DescriptionJOB SUMMARY Responsible for a wide range of duties in support of departmental efficiencies which may include but not limited to performing registration, patient pre-admission and admission, reception and discharge functions, arranging support Hospital services requested by patients through referrals, performs thorough analysis of admission discharge transfers (ADT), Revenue Cycle Reports, leads shift Patient Access Operations, and collaborates with Department leaders in process and operational excellence.ESSENTIAL DUTIES AND RESPONSIBILITIESInclude the following. Others may be assigned. Greeting patients following Conifer Standards of Care, provides world-class customer service, completes full patient registration at date of service adheres to financial & cash control policies & procedures, thoroughly explains and secures Hospital & patient legal forms (i.e., Advance Directives, Conditions of services, Consent for treatment, Important Message from Medicare, EMTALA, etc.). Scan Protected Health Information, create and file patient information packets/folders for upcoming Hospital services. May also assist with scheduling diagnostic or surgical procedures, conducting physician office/patient interviews, and explains hospital procedure guidelines and policies. Provides full patient financial counseling, education & referrals, employs and completes all patient liability collection escalations through proper, compliant patient liability collection techniques before, during & after date of service, performs Hospital cash reconciliation & secured payment entry in adherence to financial & cash control policies & procedures. Secures medical necessity checks/verification in accordance to Centers for Medicare & Medicare services, verifies insurance, benefits, coverage & eligibility, completes assigned registration financial clearance work lists activities, obtains insurance authorizations for scheduled & unscheduled Hospital services, and secures inpatient visit notification to payors. Performs thorough analysis of admission discharge transfers (ADT), Revenue Cycle Reports, leads shift Patient Access Operations, and collaborates with Department leaders in process and operational excellence.KNOWLEDGE, SKILLS, ABILITIESTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. JOB SUMMARY AND QUALIFICATIONS:

Responsible for the overall Clinical services to families in the Family Recovery Program in Laurel County, to include children. It requires a reasonable combination of the following knowledge and skills; Work with individuals with substance use disorder, ability to use and implement cognitive behavioral therapies and use of community resources and ability to serve as an appropriate role model. Compassion towards people living with mental illness, substance abuse disorders, and HIV/AIDS and ability to convey respect to the target populations are critical. Must learn and apply agency, personnel and program policies and procedures. Must have knowledge and understanding of the current treatment modalities. Preference given to those that have the ability to train and supervise other clinical staff and interns.

WHAT YOU SHOULD HAVE FOR THIS ROLE :

The position requires a minimum of a Master's degree in social work or a related field AND the accompanying certification (CSW, MFT, LCPA) with a preference given to a clinical license (LCSW, LMFT, or LPCC) Preference given to candidates who have completed a practicum working in behavioral health and substance use issues.

RESPONSIBILITIES :

1. Manage caseload and client needs

2. Work effectively with referral sources.

3. Document services through current, complete records.

4. Counseling on personal, family financial, employment, behavioral, crisis and substance abuse matters.

5. Advise other staff on treatment matters and coordinate team treatment as appropriate.

6. Discuss and review client progress

7. Participate in regular client review.

8. Maintain a current CPR and First Aid certificate.

9. Participate in weekly clinical supervision.

10. Conduct group and individual sessions daily.

11. Maintain billing requirements.

12. Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.
