**Job Description Summary:**
MINIMUM QUALIFICATIONS
High School Diploma or Equivalent AND one of the following:
· Completion of a High School Medical Assistant Career Technical Program, or
· Graduation from an accredited School of Medical Assisting, or
· Completion of Military Medic training, or
· Certification as a Paramedic (EMT-P), or
· At least one year of experience as a physician-trained Medical Assistant in a similar clinical setting.
This position is responsible for administrative and clinical duties delegated by the physician. The Medical Assistant is responsible for assisting physician with patient care in compliance with the Ohio Administrative Code. Other responsibilities include, but are not limited to, routine clinical and administrative duties as needed to meet the needs of the practice to which it is assigned. The duties of the Medical Assistant vary from office to office based on size and specialty of the practice
**Responsibilities And Duties:**
Facilitates efficient and effective patient flow including preparing the patient for the visit and providing specific services as determined by providers in accordance with provider licensure, OH policy and scope of practice.
Successful completion of Care Connect EMR documentation skills.
Performs necessary administrative duties that assure all information is current and updated. Includes maintenance and monitoring of EMR in basket.
Maintains and cleans environment, work area, equipment, and communicates any equipment malfunctions to appropriate dept/unit personnel.
Communicates with patients and customers in a respectful and caring manner.
Communicates, collaborates, and anticipates the needs of the health care team to ensure continuity and quality of care and coordination of services.
Collects and monitors patient data; reports patient data and patient need as directed.
Maintains documentation guidelines around patient chargeable items/services.
Ensures processes and services are continuously monitored for quality, cost effectiveness, and efficiency.
Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.
Engages in continuous study of the entire professional field, including best practices, to maintain the professional competence, knowledge, and skills necessary for the satisfactory performance of all assigned responsibilities.
Completes required continuous training and education, including department specific requirements.
Promotes and advocates for patient quality, safety, and experience initiatives. Acts as a role model.
Serves as SME/resource to the dept/unit/practice in areas such as quality, performance improvement, and EMR (i.e., EPIC Super User).
Has oversight of efficient and effective patient flow including preparing the patient for the visit/admission and providing specific services and education as determined by the providers in accordance with licensure and policy.
Leads and supports development of standard work and develops necessary customizations.
Functions as a clinical preceptor and is responsible for training new staff.
Is compliant with scope of practice.

National Youth Advocate Program is proud to announce that we were selected as the Care Management Entity (CME) in Catchment Area C, made up of 11 Ohio counties: Allen, Auglaize, Champaign, Clark, Darke, Hardin, Greene, Logan, Madison, Miami, and Shelby. The CME is responsible for delivering wraparound care coordination for children and youth enrolled in OhioRISE who have moderate behavioral health needs, and for helping to grow the system of care in the communities served to ensure the behavioral health needs of children and their families are met.
As a result, NYAP is seeking a fulltime CME Moderate Care Coordinator which will cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around model of care coordination covering all of Logan County and overlap into Champaign and Hardin Counties as needed.
Under direct supervision of the Care Coordination Supervisor, this employee will aim to achieve the ultimate goal to keep youth in their homes, communities, and schools by assessing and delivering the appropriate services needed and reducing unnecessary out-of-home placement and potential custody relinquishment.
**Responsibilities:**
+ Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination
+ Develop and maintain the Wraparound Team, including coordinating and leading team meetings
+ Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan with providers and community resources; update plan as necessary
+ Ensure family support and stabilization during crises
+ Provide and document the initial and ongoing Life Domain Assessment
+ Maintain all service documentation requirements, evaluation outcome requirements and data as required
+ Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan
+ Utilize and monitor Flexible Funding and service coordination
+ Obtain weekly reports from subcontracted providers
+ Participate in after hours on-call response
+ Attend Program staff meetings, supervision and any other meetings as required
+ Participate in the Agency and Program CQI Peer review process
+ Perform duties to reflect Agency policies and procedures and comply with regulatory standards
+ Meet Agency training requirements
+ Report all MUl's to Site Manager and Supervisor immediately
+ Other duties as assigned
**Qualifications**
+ An MCC Care Coordinator will be a licensed or an unlicensed practitioner in accordance with rule of the Administrative Code
+ MCC care coordinators will complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM
+ MCC Care Coordinators will successfully complete skill and competency-based training to provide MCC
+ MCC Care Coordinators will have experience providing community-based services to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field for:
(i) three years with a high school diploma or equivalent; or
(ii) two years with an associate's degree or bachelor's degree; or
(iii) one year with a Master's degree or higher
**CME Moderate Care Coordinators will:**
+ Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
+ Be culturally competent or responsive with training and experience necessary to manage complex cases
+ Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child- serving systems (e.g., child welfare, juvenile justice, education)
+ Live in one of the counties included in Catchment Area C

OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio's first highly integrated care program for youth with complex behavioral health and multi-system needs.
National Youth Advocate Program is proud to announce that we were selected as the Care Management Entity (CME) in Catchment Area C, made up of 11 Ohio counties: Allen, Auglaize, Champaign, Clark, Darke, Hardin, Greene, Logan, Madison, Miami, and Shelby. The CME is responsible for delivering wraparound care coordination for children and youth enrolled in OhioRISE who have moderate behavioral health needs, and for helping to grow the system of care in the communities served to ensure the behavioral health needs of children and their families are met.
As a result, NYAP is seeking a fulltime CME Moderate Care Coordinator which will cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around model of care coordination covering all of Darke County and overlap into Shelby and Miami Counties as needed.
Under direct supervision of the Care Coordination Supervisor, this employee will aim to achieve the ultimate goal to keep youth in their homes, communities, and schools by assessing and delivering the appropriate services needed and reducing unnecessary out-of-home placement and potential custody relinquishment.
**Responsibilities:**
+ Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination
+ Develop and maintain the Wraparound Team, including coordinating and leading team meetings
+ Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan with providers and community resources; update plan as necessary
+ Ensure family support and stabilization during crises
+ Provide and document the initial and ongoing Life Domain Assessment
+ Maintain all service documentation requirements, evaluation outcome requirements and data as required
+ Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan
+ Utilize and monitor Flexible Funding and service coordination
+ Obtain weekly reports from subcontracted providers
+ Participate in after hours on-call response
+ Attend Program staff meetings, supervision and any other meetings as required
+ Participate in the Agency and Program CQI Peer review process
+ Perform duties to reflect Agency policies and procedures and comply with regulatory standards
+ Meet Agency training requirements
+ Report all MUl's to Site Manager and Supervisor immediately
+ Other duties as assigned
**Qualifications**
+ An MCC Care Coordinator will be a licensed or an unlicensed practitioner in accordance with rule of the Administrative Code
+ MCC care coordinators will complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM
+ MCC Care Coordinators will successfully complete skill and competency-based training to provide MCC
+ MCC Care Coordinators will have experience providing community-based services to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field for:
(i) three years with a high school diploma or equivalent; or
(ii) two years with an associate's degree or bachelor's degree; or
(iii) one year with a Master's degree or higher
**CME Moderate Care Coordinators will:**
+ Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
+ Be culturally competent or responsive with training and experience necessary to manage complex cases
+ Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child- serving systems (e.g., child welfare, juvenile justice, education)
+ Live in one of the counties included in Catchment Area C

**Join Our Team as a Lead Injury Prevention Coordinator - Anna, OH**
Are you passionate about keeping workers safe and healthy? We're seeking a dynamic **Lead Injury Prevention Coordinator** to champion injury prevention initiatives within the workplace at our client site located in **Anna** **, OH.** This role is ideal for someone with a strong background in industrial ergonomics, injury prevention, and staff leadership, especially within manual laborer or industrial populations.
Responsible for the coordination and delivery of work conditioning and early intervention services, with a primary focus on preventing work-related injuries. This role involves collaborating closely with clients to set program goals and priorities, ensuring the effective execution of injury prevention strategies. Additionally, the role includes leading the development and implementation of interventions/programs focused on the manufacturing environment to meet client objectives.
**Full-time:** 40 Hours/week
**Schedule:** M-F, combination of opening and midday shifts (i.e. 6a-2:30p and 10a-6:30p) **.** Occasional coverage required for later shifts.
**Location:** 100% in-person, providing oversite at two client locations. Main site in Anna, OH, secondary location in Russells Point, OH.
**Key Accountabilities:**
+ Oversee injury prevention team: carries out supervisory responsibilities in accordance with HealthFitness policies, procedures and applicable laws including recruiting, orienting, training and evaluating; assists in the planning and assignment of work duties in collaboration with the client to ensure staff remain challenged and productivity is maximized; provides mentorship and assistance to all direct reports; conducts formal semi-annual performance appraisals and initiates formal corrective action. Guide and support a team in delivering top-notch injury prevention programs, including work conditioning programs, ergonomic evaluations, and early intervention initiatives.
+ Lead the development and implementation of work conditioning and early interventions/programs focused on the manufacturing environment through collaboration with clients and their stakeholders to develop and implement interventions, annual goals, and metrics. Assess client needs and identify HealthFitness programming solutions.
+ Conduct employee injury prevention programs, education, and training. Develop exercise programs specific to injury prevention and conditioning needs. Including evaluation, follow-up and potentially including placement of employees.
+ Collect and evaluate appropriate program data to support the ROI process and report data for assigned areas.
+ Promote and market wellness and general education programs to the target population.
+ Projects or other duties as needed/assigned.
**Minimum Qualifications:**
+ Bachelor's degree or higher in a health-related field from an accredited college or university program. (i.e. Kinesiology, Exercise Science, Exercise Physiology, Athletic Training, etc.)
+ OSHA compliant Adult First Aid/CPR/AED from American Red Cross, American Heart Association, National Safety Council, or American Safety & Health Institute.
+ 2-5 years' experience in athletic training and/or exercise science program delivery.
+ Minimum 1 year previous Supervisory/Staff management experience.
+ Experience in industrial occupational health environment.
+ Exceptional interpersonal communication and customer service skills.
+ Proven ability to lead a team towards collective goals and strategy.
+ Proven ability to collaborate, integrate, and partner with multidisciplinary teams/departments.
+ Computer proficiency in Microsoft Office programs including at minimum Word, Excel and Power Point.
+ Ability to effectively organize and prioritize work demands.
+ Ability to effectively work independently or as part of a team.
+ Ability to travel to other client locations in personal or company pool vehicle, valid driver's license required.
