To help ensure that each participant remain stable at the site and make progress toward his permanency goals prescribed by his permanency order. This position will be responsible for performing day-to-day support services assistance to the participants of the program.
To manage all aspects of a participant's case (Court visits and court reports, ACR reports and visits, participant advocating, medical/dental visits, hospital visits, family attachment, etc.) as it relates to their residence and growth while at Garden of Prayer Youth Center.
ORGANIZING AND COORDINATING
Screening/Intake
- Conduct intake process and intake meetings with therapist, DCFS worker, and participant.
- Assure the retrieval of necessary documentation for participant admission into the program.
- Conduct participant orientations to familiarize them with program expectations, conduct clothing inventory, and build rapport.
- Conduct the process of receiving new participants.
- Ensure 906 is completed promptly.
- Some duties may be reassigned and other duties may be asked of you as required.
- Maintain proactive, effective communication with participants, direct care staff, therapist, and Program Coordinator regarding all participant issues and concerns.
- Maintain proactive, effective communication with participants, participant family, DCFS workers, DCFS Monitor, DCFS Licensing Monitor, Program Coordinator, GAL, and all other stakeholders involved with the participant's case.
- Ability to observe participant behavior and participation, assess appropriateness of such, and prepare reports and other correspondence in order to advocate and communicate all pertinent information regarding participants.

- Set a good example for participants including cleanliness, punctuality, sportsmanship etc.

- Complete and/or make sure that UIR's are written, distributed and maintained
- Assure that the client has access to and meets with GAL and all legal representation
- Assure that all court, probation or parole orders are followed and adhered to
- Arrange sibling or parental visits if appropriate and upon request and document all visits
- Assure youth is at all court appearances when attendance is mandatory
- Weekly visitation to youth when hospitalized or incarcerated and provide youth with allowance
- Contacting SASS for an evaluation/screening in the event that a youth is psychiatrically hospitalized
- Obtain all necessary consents
- Unusual Incident Reporting
- All other duties as assigned according to DCFS policy
