Overview:
Interprets orders for requested exams, produces CT and diagnostic medical images, positions and transfers patients. Selects proper technical factors on an individual patient basis, selects and operates equipment as directed. Provides protection in accordance with prescribed safety standards. Assists physicians in administering enhancing agents and medications. Responsible for delegated areas of authority in the performance of CT and special imaging techniques. Must be able to proficiently operate and use equipment related to all exams performed, the computerized radiology information system and office equipment. Assists within all areas of radiology as needed. Functions as a cooperative team member on weekend and evening shifts and reports personnel and procedural incidents to the Section Manager. Works in specialized areas as needed. Recommends the procurement and necessity of maintenance of equipment. Maintains current CPR certification and assists in performing first aid as necessary. Remains current with credentialing agencys required continuing education.
Qualifications:
Education: Satisfactory completion of formal Radiologic Technology training.
Experience: Has acquired experience and possesses capability as determined to be adequate for the position.
Certifications: A.R.R.T. registered or registry eligible upon hire, and A.R.R.T. achieved within six months of employment.
Other: Performs duties in the manner that reflects the basic Mercy Values of love and concern for others. The Care Coach 2 (Transition/Custodial Prevention Specialist) assesses and evaluates member needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members. The Care Coach 2 work assignments are often straightforward and of moderate complexity.
The Care Coach 2 (Transition/Custodial Prevention Specialist) will meet members in the field at home, Nursing Facility (NF) or location of the member's choice, spending quality time assessing their goals, needs and barriers and then connecting members with quality services to promote their ultimate well-being and drive person-centered health outcomes.
**Responsibilities include:**
+ Provides specialized support for members receiving Long Term Services and Support (LTSS), with a focus on addressing health-related social needs (HRSNs), providing psychosocial support, and ensuring LTSS meets the member's service needs.
+ May support members residing in NFs by building relationships with facility staff, advocating for member care (including access to needed behavioral health services), and assessing the member's desire and ability to return to the community.
+ Contacts members telephonically and face-to-face to establish goals and priorities, evaluate resources, develop plans of care, and identify LTSS providers and community partnerships to provide a combination of services and supports that best addresses the needs and goals of members and caregivers through person centered thinking approaches.
+ Focuses on supporting members and/or caregivers utilizing an interdisciplinary approach in accessing long term services and support, social, housing, educational and other services, regardless of funding sources to meet their needs.
+ Develops and modifies Individual Service Plans and involve applicable members of the care team in care planning (Informal caregiver coach, PCP, etc.).
+ Supports members through navigation of their LTSS and related environmental and social needs.
+ Utilize available information pertaining to members to prevent the need for administration of duplicative assessments.
+ Make recommendations for appropriate Home and Community-Based Services (HCBS) to enable member's independence in the community.
+ Facilitates interactions with other payer sources, providers, and interdisciplinary care teams.
+ Educates members in maintaining Medicaid eligibility.
+ Assist with entry of annual Level of Care assessment into state portal.

**Required Qualifications**
+ **Applicants must reside in the state of Illinois or within a 10-mile radius of Illinois in a bordering state.**
+ Bachelor's degree in social sciences, social work, human services, or a related field.
+ Two (2) years of prior experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), and/or managed care organizations.
+ One (1) or more years of professional experience working with members in custodial and transitional care.
+ Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
+ Exceptional oral and written communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
+ Proven ability of critical thinking, organization, and problem- solving skills.
+ Ability to manage multiple or competing priorities in a fast-paced environment.
+ Ability to use a variety of electronic information applications/software programs including electronic medical records.
+ This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
+ Ability to travel in the region to meet face to face with members and/or their families, community partners and care teams.
+ This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individuals must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$5,000 bodily injury per event / 10,000 for property damage or whichever is higher.

**Additional Information**
+ **Workstyle:** This is a Field position - Employees perform their core duties at noncompany locations, such as providing services at business partner facilities or prospects' and members' homes.
+ **Travel:** 75 - 90% field-based interactions with members and/or their families, community partners and care teams. May need to attend occasional onsite meetings in Humana's Schaumburg, IL office.
+ **Work Schedule:** Monday - Friday; 8:00 AM - 5:00 PM Central Standard Time (CST), with flexibility available. Additional hours may be required based on business needs.

Cape Girardeau Area Nurse Needed!
Make a meaningful impact every day as a CenterWell Home Health nurse. You'll provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you'll develop and manage care plans that support recovery and help patients get back to the life they love.
LPN based in Cape Girardeau Areas
As a **Home Health LPN** , serving patients in the Cape Girardeau area you will:
+ Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
+ Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.
+ Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
+ Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
+ Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
+ Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
+ Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is dearly in the best interests of the patient.
+ Appropriately communicate to ensure adherence to professional standards in the provision of and availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.
+ Prepare, submit and maintain documentation as required by the Company and/or facility. Visit/shift notes documented on day services are rendered.

**Required Experience/Skills:**
+ Live in the Cape Girardeau MO area
+ Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
+ Current nursing license in the practicing state.
+ Valid drivers license, auto insurance and reliable transportation.
+ Current CPR certification.
+ Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
Pay Range
- $28.00 - $0.00 - pay per visit/unit
- 44,600 - 61,400 per year base pay

As a **Patient Coordinator** , you will report to the Manager and perform daily front office tasks and duties to help create lasting impressions and build trust and loyalty with patients. When you join an Aspen Dental practice, you'll participate in a four-week training program to succeed in your role.
+ Provide superior patient service with compassion and care in accordance with patient needs, company policies and procedures, government regulations, and dental board standards
