Works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.

Primary Responsibilities:

* Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
* Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
* Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
* Completes follow-up and post-discharge assessments according to documented standard operating procedure
* Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
* Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
* Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
* Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
* Provides counsel, support and education to staff as appropriate
* Actively participates in ongoing meetings pertaining to patient care and clinical excellence
* Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
* Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews

STG International (STGi) is currently seeking a Clinic Manager to provide services at our Community Based Outpatient Clinic. The primary purpose of this position is to serve as the Patient Relations Specialist, who represents the interests of Veteran patients in all areas of the clinic and coordinate care with the Medical Center and stakeholders. The position requires an understanding of the programs, processes and procedures of the clinic and relationships of various services. The position is directly linked to Customer Service, and Service Excellence program initiatives. The general duty of the Clinic Manager is to provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) and to manage the day to day activities of the clinic.

Essential Functions:

  • Perform managerial duties related to organization, leadership, and delegation of tasks, critical thinking, and prioritizing competing actions/activities.
  • As the designated CBOC Safety Officer, demonstrates, understands and performs tasks related to safety and security principles and practices.
  • As the designated CBOC infection Control Officer, demonstrates, understands, and performs tasks related to infection control principles and practices.
  • As the designated CBOC Patient Advocacy Officer, demonstrates, understands and performs tasks related to customer service and patient satisfaction.

  • Work under direct supervision of a Physician for clinical practice in accordance with state and federal guidelines and rules.
  • Provide primary and preventive medical care to meet the needs (physical, mental, and emotional) of patients under a limited scope of Current Procedural Terminology (CPT) codes and within his/her skills and training.
  • Provide medical patient care to the scope and standards of practice established by a nationally recognized credentialing organization representing Registered Nurses.
  • Function within the scope of practice of the state and state Medical Practice Act.
  • Emphasize wellness, prevention, and early detection.
  • Supervise support staff in delivery of care to patients.
  • Conduct preliminary review of the Patient Aligned Care Team (PACT) metrics.

  • Assist the CBOC staff to ensure all patient information on access/wait time documentation is complete and accurate.

Required Skills
  • Must have exceptional diplomatic communication skills and experience in handling difficult customer situations.
  • Must be detailed oriented and have the ability to multi-task.
  • Proven track record in advanced high level customer service and follow through a plus.
  • Must be a positive, professional team player.
  • Current RN state license preferred. Must comply will all state requirements and regulations.
  • Possess Basic Life Support (BLS from AHA or MTN).
  • Knowledge of Microsoft Office Software and computer skills including experience with electronic medical records systems.
Required Experience
  • Minimum of one (1) years' experience as a clinic manager in a related area of patient care - I.e. Family Practice/Adult Practice/Women's Health.
  • Graduate of an accredited school of nursing and a graduate of an accredited program for RN from a National League for Nursing (NLN)-accredited school - Bachelor of Science in Nursing (BSN) OR Bachelor of Management in Healthcare administration preferred.

Working Conditions/Physical Requirements:

  • Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.
  • Work is normally performed in a typical interior/office work environment.
  • Required to respond to any medical emergency within the clinic, where a fast walk or jog may be necessary to provide assistance.

The Care Coordinator collaborates and documents all efforts to help the member address any barriers to access to appropriate community-based referrals.Care Coordination includes the appropriate use of and facilitation of referral to various community-based support modalities, including consideration of referral to 12-step and other self-help programs, peer recovery services, social service agencies, and other community-based resources appropriate to the member's recovery.Organizes and participates in interdisciplinary care planning that consists of at least monthly meetings of the multidisciplinary treatment team (including all relevant medical and behavioral health care professionals involved in providing and coordinating the member's care) and documents this collaboration appropriately.The Care Coordinator, in participating in the interdisciplinary treatment team meeting, will review the patient's complete medical record (including urine drug screens and laboratory tests), discuss the current status of the patient's progress toward meeting their goals as specified in their plan of care, assist in addressing any barriers toward the individual's progress in meeting their identified treatment goals as well, as the actions which the treatment team will undertake to address those barriers.The Care Coordinator will participate in identifying any new problems and goals and modification of the IPOC action plan accordingly. Works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
**Primary Responsibilities:**
+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
+ Completes follow-up and post-discharge assessments according to documented standard operating procedure
+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
+ Provides counsel, support and education to staff as appropriate
+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence
+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
+ Perform related duties as necessary and other duties as assigned

**Required Qualifications:**
+ Completion of Nurse Practitioner accredited program
+ Current NP license
+ Current state RN license

As a Speech-Language Pathologist, you'll contribute to our success in the following ways:
+ Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
+ Evaluates the Speech-Language pathologist patient to determine the rehabilitation needs and potential for achievement and develops a plan of care in accordance with the findings.
+ Admits the patient and assesses speech-language therapy and all other needs according to OASIS and PPS regulatory guidelines. Makes the initial therapy evaluation visit and re-evaluates the patient's speech language therapy needs during each visit. Makes follow-up assessments according to OASIS and PPS regulatory guidelines.
+ Initiates plan for patient safety, using the patient, family, and community resources.
+ Communicates significant findings, problems, and changes in condition or environment to the appropriate supervisor, the physician, and other team members involved with patient care.
+ Participates in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning. Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines.
+ Writes physician orders to cover additional visits and changes to the Plan of Care, per agency policy.
+ Incorporates patient care goals established in the Plan of Care, into therapy care, as evidenced by documentation in therapy notes. Initiates and revises the Plan of Care in response to identified patient care issues.
+ Performs direct Speech-Language pathology services in accordance with accepted standards of practice and certified by the patient's physician.
+ Selects diagnostic and therapeutic materials appropriate to patient needs. Instructs and provides patients and/or their caregiver in the home therapy program providing modification and clarification when necessary.
+ Assures clinical notes indicate continuing communication and coordination of services with physician, other staff, and disciplines.
+ Ensures that all therapy notes, supply log/visit logs are accurate and turned into the office according to office policy.
+ Physically demanding, high stress environment
+ Full range of body motion including handling, lifting and transfer of patients
+ Potentially work irregular hours including call hours if applicable
What is Required?
+ Master's degree from an accredited educational program in Language Pathology
+ Current, unrestricted SLP license in the state of work
+ Minimum one (1) year of experience as a SLP in a clinical care setting
+ Able and willing to travel within branch/office coverage area.
+ Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
+ Must be able and willing to travel 50%
+ Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
