Health and Wellness Program Manager - Healthcare Point Solutions
Position Overview
The Health and Wellness Program Manager will enhance the experience of Shaw associates through Total Rewards. They will work closely with all areas of Total Rewards including, but not limited to Compensation, Payroll, Financial Wellness, and the Health and Wellness Benefits teams. Primary responsibilities will include managing high profile projects with the Shaw Family Health Center (SFHC) and ancillary benefits in regard to associate experience at Shaw and aligning with The Shaw Way. This includes benchmarking, preparing proposals, managing ongoing project tasks , implementing approved solutions, developing measurements for success, and completing follow-up, as appropriate . They will work with the Sr. Manager of Health and Wellness to lead the day-to-day management of the Shaw Family Health Centers and programming. The ideal candidate must balance being task-oriented while also thinking strategically. Being organized and paying attention to detail are imperative to the success of this role. They should be a quick learner in order to properly assess situations, programs, etc., and make the appropriate decisions for next steps/actions. They will need strong financial acumen that considers tangible and intangible cost exploration that improves the quality of Shaw's Healthcare options Relationships will play a key role in working with third party vendors, HR Operations, and our Shaw Associates.
POSITION R ESPONSIBILITIES :
+ Partner with the Health and Wellness team, a n d Shaw Family Health Center to execute Shaw's corporate wellness strategy
+ Partner with the Total Rewards Project Coordinato r on communication needs
+ Collaborate with Total Rewards partners on a ssociate e ngagement needs and initiatives (focus groups, surveys, research, etc.);
+ Work with internal partners to ensure the Health and Wellness section of MyShaw is properly maintained and up to date ;
+ Manage projects related to the growth, and expansion for the Shaw Family Health Center (SFHC) and other ancillary projects ;
+ Work with the Sr. Manager of Health and Wellness to determine growth strategy for new services ;
+ Create capital projects to secure funding;
+ Oversee both the Shaw and vendor-led implementation teams;
+ Partner with vendor(s) to ensure appropriate metrics are utilized to determine path forward, if successful ;
+ Partner with Risk Management and Health Services on Health and Wellness initiatives, as needed ;
+ Manage multiple projects concurrently and be capable of initiating, implementing, and monitoring change; follow-up with project team members to obtain and consolidate project status updates; provide regular project updates as appropriate ;
+ Manage vendor/carrier relationships to ensure satisfactory performance is achieved, and performance measures are met and/or exceeded . These relationships include:
+ Ensure management of associate eligibility and engagement in these benefits and programs;
+ Conduct regular check-in meetings with vendor team to be certain open items are progressing and completed timely ;
+ Coordinate quarterly, mid-year, and annual meetings to review plan and vendor performance;
+ Complete any vendor "report cards" providing feedback to the vendor team;
+ Leading implementation efforts for any new vendors or vendor transitions in this space ;
+ I nterpret data in order to understand plan/vendor performance, make recommendations for plan/benefit changes, etc.
R EQUIRE D QUALIFICATIONS :
+ 5-7 years' work experience in project/program and/or vendor management
+ Limited travel necessary ( likely two to four times annually)
+ Self-starter with the ability to operate independently with little supervision
+ Exhibits proper time management of assignments and responsibilities, including being organized and task-oriented
+ Demonstrated ability to make decisions showing good judgment, proclivity to ask for and listen to team feedback, and propensity to action
+ Strong leadership and interpersonal skills with an ability to work in a team-oriented environment including coaching and developing others
+ Effective oral and written communication skills including preparing for and leading team meetings, professional and informative responses to customers and vendors
+ Proficient in Microsoft Office
+ High level of confidentiality
Prefered:
+ Four-year degree
LOCATION REQUIREMENTS:
This position is a hybrid role that will require the associate to be in the office (Dalton, GA) 2-3 days a week.

**The role you'll contribute:**
The Home Health Physical Therapist (PT) Care manager is a professional therapist who coordinates and directs the home care patient's services based on individual patient needs. The PT Care manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The PT is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes pace regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The PT Care manager cares for a caseload of home health patients requiring therapy as the primary service by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. S/he provides follow up by evaluating effectiveness of the home care plan, and monitoring patient/family's response to the plan to achieve patient/family goals and top decile outcomes. The PT Care manager also identifies performance improvement and home health standard of care initiatives and assists to design or implement programs to address needed changes.

+ Coordinates and directs the care of a caseload of home patients requiring physical therapy as the primary skill. Provides comprehensive assessment, planning, implementation and evaluation for that caseload as the primary Care manager.
+ Sets priorities of home care caseload adapting to the changing needs of the home care patients and families. Optimizes schedule daily to support productivity, efficiency and maintain best practice visit utilization.
+ Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record.
+ Utilizing assessment data, formulates a patient specific plan of care along with the patient, family and physician which is feasible within the physical, financial and emotional resources of the family. Establishes individualized, realistic, measurable patient centered goals in consultation with the patient, family and other health care providers including goals to improve or stabilize patients medical condition, functional abilities and promote independence. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient when developing the plan of care.
+ Periodically reassess the patient every 30 days: provide the ordered therapy service, functionally reassess the patient and compare the resultant measurement to prior assessment measurements. Document the measurements in the clinical record along with the therapists determination of the effectiveness or therapy or lack thereof.
+ Observes and records activities and findings in the clinical record and reports to the physician the patients reaction to treatment and any changes in patients condition, or when there are deviations from the plan of care.
**The expertise and experiences you'll need to succeed:**
**Minimum qualifications:**
+ 1 year relevant clinical physical therapy experience
+ Physical Therapist (PT)
+ Valid in state Drivers License with current car insurance
+ Active American Heart Association BLS
**Preferred qualifications:**
+ Doctorate Physical Therapy
+ 1 year recent, relevant experience in a Medicare-certified home health agency as a case-manager
+ Home Health Care-Manager Certification
+ Certificate for OASIS Specialist Clinical (COSC)
