Become the Case Manager you always wanted to be
  • Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
  • Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
  • Participate in planning for and the execution of patient discharge experience.
  • Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
  • Facilitate team conferences weekly and coordinate all treatment plan modifications.
  • Complete case management addendums and all required documentation.
  • Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
  • Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
  • Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
  • Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
  • Perform assessment of goals and complete case management addendum within 48 hours of admission.
  • Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
  • Schedule and facilitate family conferences as needed.
  • Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
  • Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
  • Make appropriate/timely referrals, including documentation to post discharge providers/physician
  • Ensure accuracy of discharge and payor-related information in the patient record
  • Participate in utilization review process: data collection, trend review, and resolution actions.
  • Participate in case management on-call schedule as needed.

Qualifications

  • License or Certification:
    • Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
    • If licensure is required for one’s discipline within the state, individual must hold an active license.
    • Must meet eligibility requirements for CCM® or ACM™ certification upon entry into this position OR within two years of entry into the position.
    • CCM® or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.

·    Minimum Qualifications :

    •  For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
    •  For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor’s degree and graduate degree is preferred.
    • 2 years of rehabilitation experience preferred.

Master's degree in counseling psychology or a related field and 2 years of professional experience

Preferred Qualifications

UHC Minimum:
  • Master's degree in social work, Marriage and Family Therapy, Psychology, Counseling, or related field.
  • Must have at least an associate license in the State of Georgia (LMSW, LAPC, LAMFT)

Position Summary

The Mental Health Professional II is an integral member of the CAPS Professional Staff. Clinicians will provide primary, secondary, and tertiary services to eligible members of the UGA community, within the CAPS scope of service. These services include group, individual, crisis interventions, outreach, and consultation. Clinicians provide mental health evaluations, formulation of conceptualization, diagnoses, treatment plans, crisis intervention, and internal/external referrals as needed for best care. Services are delivered in accordance with UHC/CAPS policies and procedures. Clinicians follow professional and organizational standards for care and documentation. All are valued team members who participate in performance improvement initiatives and are actively involved in team activities, i.e. required meetings, committees. Team members demonstrate respect for all while maintaining inclusive attitudes and appreciation of individual aspects of diversity.

  • Strong interpersonal skills and the ability to work in a fast-paced work environment.

Services Area/Programs: Intensive Case Management

Description of Duties: The individual in this role will serve as Case Manager of the Intensive Case Management Team . The focus of this program is to provide community rehabilitative services that provide direct treatment and restorative interventions as well as case management. The ICM Team is dedicated to supporting clients in reducing psychiatric or substance use symptoms, promoting long-term symptom stability, and restoring essential community living and interpersonal skills. The team also provides immediate crisis intervention to de-escalate urgent situations and ensures timely connection to appropriate community services and resources. Responsibilities of the Case Manager include: Effectively manage a caseload of 15-30 individuals, completing the minimum requirements of no less than (4) face-to-face contacts per month in the individuals' home or choice of a safe community setting. Keep detailed progress notes of visits to home, community locations, and phone contacts. Complete all documentation in the electronic client database within 24-48 hours of service delivery. Meet productivity of no less than 55% or higher each month. Add goals/objectives to Plans of Care that are person-centered according to DBHDD guidelines. Provide follow-up and referral services for individuals as needed. Provide community resources and contacts to the individuals to facilitate delivery of supported housing and employment services as needed. Serve as liaison between individuals, their physicians, and other organizational components to ensure communication and linkage for continuity of care and services. Provide on-call assistance involving rotating on-call duties with team members. Transporting individuals served is a requirement.

Minimum Qualifications: Bachelor's Degree in psychology or related field. Must have a valid, unrestricted Georgia Driver's License. Must be keyboard proficient.

Preferred Qualifications: One to two years of experience working with individuals with behavioral and substance use disorders. Master's degree in a related field.

5. Delivers and serves meals to patients/residents in dining area; food and beverages
6. Prepares between-meal nourishments (make sandwiches, etc.) and serves in accordance with procedural guidelines

**JOB PURPOSE:**
Assists with food preparation in accordance with our established procedural guidelines, and as may be directed by the Dietary Manager or supervisor. Follows facility procedure for storing, serving and transporting food and supplies. Is also responsible for the cleaning, sanitizing and proper maintenance of equipment.
**KEY RESPONSIBILITIES:**
1. Follows diet orders and NPO diet orders
2. Assists cook in preparing meals, desserts and/or snacks for patients/residents; i.e. salads and desserts and other dishes
3. Uses proper food handling processes, procedures and policies
4. Delivers trays to patients/residents on halls
5. Delivers and serves meals to patients/residents in dining area; food and beverages
6. Prepares between-meal nourishments (make sandwiches, etc.) and serves in accordance with procedural guidelines
7. Offers menu substitutions and records them in accordance with procedural guidelines
8. Follows procedures for serving partner meals
9. Communicates well with patients/residents/clients and family members providing warm and friendly greeting and an approachable attitude to families, visitors, patients/residents/clients and responds to expressed concerns while displaying a helpful, caring demeanor. Answers questions when appropriate in a professional manner.
**KNOWLEDGE, SKILLS, ABILITIES:**
- Cleans tables in the dining area after each meal.
- Cleans work area at the end of shift and after meals, including sweeping and mopping
- Removes garbage from kitchen areas and hoses out garbage containers.
- Complies with infection control policies in the work area.
- Scrapes dishes, washes dishes, pots and pans.
- Records freezer and walk-in refrigerator temperatures in accordance with established procedures.
- Checks stock as needed and stores in an appropriate manner according to facility guidelines
- Stores cleaning material appropriately.
- Sets up tray line and performs tray line service.
- Properly stores leftovers/ opened food.
- Makes coffee/ tea as requested.
- Cleans kitchen equipment such as, carts, tables, counters, ice machine, buckets, blender, mixer, meat slicer, freezer, refrigerator, stove, steamer, garbage disposal, dish machine coffee/ tea maker, steam table etc.
- Operates the coffee & tea maker, dish machine, garbage disposal, mixer, blender, meat slicer and steamer.
- Follows standardized recopies.
- Uses serving equipment (ladle, spoon, scoops, etc.)
- Checks and records water temperature (dishwasher, pat and pan sink, etc.)
- Checks dishwasher with sanitizer strips.
- Checks chemical levels and replaces if needed
- Participates in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.
- Attends and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
