Under general supervision, is responsible for the delivery of specific aspects of patient care under the supervision of a Registered Nurse. Care needs and visit frequency are indicated in the Hospice Aide Plan of Care.

Education:
High School diploma or GED.
Licensure:
Current registration with the Maryland State Board of Examiners of Nurses as a Certified Nursing Assistant.
Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
Experience:
Previous experience in an acute care or home care setting.
Skills:
* Knowledge and understanding of the Hospice Philosophy
* Skill in oral and written communication
* Skill in managing issues related to death/dying
* Ability to work independently
* Ability to effectively communicate with facility staff when necessary if assigned to facility-based care.

Principal Duties and Responsibilities:
* Performs personal care needs by attending to patient/family requests promptly. Offers and assists with bedpans and urinals. Provides personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen. Assists in dressing and undressing patients as assigned. Assists in feeding the patient.
* Meets safety needs of patient and uses equipment safely and properly (foot stools, side rails, O2 etc.).
* Plans and prepares nutritious meals, including shopping, as assigned.
* Takes and records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered (within the training limitations of the aide).
* Provides proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
* Reports patient's condition and significant changes to the assigned supervisory nurse. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
* Assists in ambulation and exercise as instructed by the primary care nurse or therapist.
* Provides assistance as assigned with light laundry needs. Keeps patient's living area clean and orderly, as assigned
* Performs range of motion and other simple procedures as an extensional therapy service as ordered (within the training limitations of the aide).
* Provides respite for patient's/family/caregiver when on-site, as appropriate.

Pay Range

$16.74 - $23.21

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.

At UPMC Western Maryland, our inpatient team manages all aspects of nutrition care across diverse patient populations, such as GI, renal, pulmonary, oncology, cardiac, surgical, and critical illness. Job Description

The Behavioral Health Navigation team is hiring a part-time Behavioral Health Navigator I to support UPMC Western Maryland.
The Behavioral Health Navigator I is responsible for the assessment and navigation of patients and families in collaboration with attending psychiatrists and other staff within UPMC Western Maryland. The incumbent will develop initial treatment plan to facilitate disposition planning and assists with bed placement or disposition planning. They will have the ability to identify acuity level of patient and will facilitate the flow of information to promote effective communication regarding the patient care and patient operations and disposition.
The shift for this position will include rotating shifts and weekends. Primarily 8-hour shifts. The incumbent will work both in the Emergency Department as other medical units in the hospital.

Responsibilities:
+ Meet standards for documentation and completes all required paperwork, including psychiatric evaluation, commitment paperwork. Demonstrate ability to utilize DSMV and to develop provisional diagnosis.
+ Demonstrate knowledge of mental health laws and reviews inpatient commitment for accuracy, competencies and proper time frames.
+ Knowledge of insurance programs, processes and able to obtain authorizations as needed.
+ Participate in interdisciplinary activities such as staff meetings and other committees as assigned.
+ Maintain competency requirements.
+ Learn and utilize up-to-date technology in delivery of patient care for assessment and disposition/referral planning.
+ Efficiently take a sizable volume of calls and disposition callers to meet their needs rapidly
+ Follow outpatient scheduling or referral documentation guidelines.
+ Responds rapidly to appointment or inpatient admission requests through email, phone, or voicemail.
+ Review and summarizes the patient information for admissions to a behavioral health unit and triaging patient for appropriate placement.
+ Ability to verbally present the case to physicians and others.
+ Demonstrate triage and assessment skills for medical necessity or appropriate outpatient clinic.
+ Provides accurate, timely information to coworkers, physicians, outside agencies, with a professional and courteous manner.
+ Demonstrates expertise in accessing social systems by providing resource-specific information to patients and families, initiating contact with appropriate resources and facilitating the patient/family's ability to accept referrals.
+ Bachelor's Degree in Psychology, Counseling or Social Work with one-year of mental health direct care experience OR master's degree in Psychology, Counseling or Social Work. Other fields of study may be considered with coursework of at least 12 credits in human services and/or relevant mental health experience.
+ Ability to respond calmly and effectively, using good clinical judgment to both psychiatric and medical emergencies.
+ Ability to identify problems, implement resolution, reassesses situation/problem.
+ Excellent interpersonal relations with the ability to interact in a direct, professional, tactful, gracious manner with all professional/non-professional contacts; includes all areas of verbal/non-verbal communication within the contest of direct/non-direct contact; history of excellent interpersonal relations.
+ Ability to work collaboratively within a multidisciplinary team.
+ Ability to work under stress, with an unpredictable patient flow, in a sometimes-crowded work environment.
+ A personal disposition that is warm, friendly, caring, kind and hospitable towards patients, colleagues. Exhibit openness and willingness to learn. Flexible schedule; rotating shifts. History/record of all above services consistently delivered with a service excellent attitude.
+ Ability and willingness to work in a managed care environment. Excellent ability and history of direct, gracious, courteous, kind face-to-face, telephone and nonverbal skills.
+ May in the course of their job duties be in rooms where medications are stored.

**UPMC Western Maryland is seeking a Speech Language Pathologist to support our team on an as-needed basis.**
This is a dayshift position that will include rotating weekends and holidays. The SLP will provide support to clients in acute care, inpatient rehab, and outpatient adult services settings.
**Responsibilities:**
+ Achieves and maintains a level of productivity that meets or exceeds the standards established for the facility by the organization.
+ Prepares necessary time records.
+ Maintains current status of all pertinent criteria necessary to satisfy competency requirements for regulatory and accrediting agencies such as OSHA, TJC, and CMS and practitioner requirements for participation as a health care provider including but not limited to: required CEU's, current professional license; current curriculum vitae; current CPR certification; current TB test; current competencies in safety, age and equipment; and in some cases current automobile license and current automobile insurance. Performs in accordance with system-wide competencies/behaviors.
+ Independently pursues professional growth and development related to this position and in lines with the mission and activities. Presents in-service and educational programs. Participates in community education and/or continuing education programs sponsored by the organization as requested.
+ Performs all job responsibilities and other duties as assigned by the facility or the organization in a manner that coincides with the mission.
+ Maintains accurate progress notes and professional records on all patients seen for speech pathology services.
+ Adheres to and supports the organizational policies, procedures and the mission, including the Corporate Compliance Program and its Code of Conduct and HIPAA regulations. This includes acceptance of the responsibility to report to the appropriate person(s) any suspected unethical, incompetent, or illegal activities or conduct.
+ Provides appropriate speech pathology treatments for all clients for whom such services are beneficial.
+ Prepares diagnostic and evaluative reports with recommendations for each person seen for Speech-Language Pathology and Audiology services.
+ Maintains a high level of confidentiality.
+ Provides appropriate diagnostic evaluative services for all clients referred for Speech-Language Pathology services.
+ Compiles and reports accurate data for the financial, statistical, and administrative needs of the Speech-Language Pathology Programs.
+ Coordinates the evaluation, scheduling, and treatment programs of clients referred for Speech-Language Therapy.
+ Attends and actively participates in staff meetings, staff development, departmental committees, and in-service education programs.
+ Obtains complete and pertinent historical information on all clients referred for Speech-Language Pathology services.
+ Maintains a positive attitude and provides excellent customer service characterized by caring, concern, and support for the public, supervisors, employees, and coworkers.
+ MS, MEd or MA in Speech-Language Pathology. **Licensure, Certifications, and Clearances:**
+ CCC (Certificate of Clinical Competency) or meet the educational requirements for certification and be in the process of completing his or her Clinical Fellowship year.
+ CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
+ Speech Pathologists license required for state in which employee provides services.
+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

The incumbent will work two 8-hour rotating shifts ever 2 weeks.
