Position Summary:

This person will be a member of the Recovery team of WestCare Tennessee and will primarily be responsible for working collaboratively with the Treatment Counselor, Supportive Employment Coach and Peer Support Specialist in the Supportive Employment grant. The Case Manager will provide services to individuals and meet the needs of the individual to determine appropriate referrals.

Essential Job Functions:

  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Tennessee.

  • Deliver evidence-based curriculum to individuals in a group setting.

  • Educate and refer individuals to community agencies and businesses that appropriately address the individual needs for food, shelter, clothing, employment, transportation, and other community resources.

  • Assist individuals with establishing or re-establishing support systems within their community.

  • Advocate on behalf of the individual in order for them to receive access to needed services in addressing appropriate needs.

  • Participate in staffing decisions regarding individuals’ current enrollment status in order to help them to meet the goals of their Individual Service Plan (ISP) and treatment plan.

  • Complete documentation of services on provided forms promptly.

  • Provide input to the Treatment Counselor regarding the individual treatment needs which may include conducting discharges, incident reports, and other related documentation.

  • Perform individual follow up interviews to document individual progress, to make additional referrals if needed and to maintain active communication throughout the duration of their enrollment in the program.

  • Will engage and complete the required WestCare Tennessee eLearning, and any other program related trainings.

Education:

**Job Summary**
The Patient Care Technician (PCT) provides high-quality, patient-centered care by performing delegated tasks in alignment with the PCT's training and the department's needs. Under the direct supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN) (LVN at Texas facilities), the PCT supports patient care by assisting with activities of daily living, maintaining a safe and organized care environment, and ensuring effective communication within the healthcare team.
**Essential Functions**
+ Assists nursing staff in delivering care, performing delegated basic patient care services, and ensuring a clean, safe, and well-organized environment.
+ Collects and records patient data, including vital signs, height, weight, oxygen saturation, intake/output, and calorie counts, reporting findings to the RN/LPN/LVN.
+ Supports patients with meals, feeding, bathing, oral care, grooming, linen changes, skin care, elimination assistance, and urinary catheter care.
+ Assists with patient positioning, repositioning, dangling, ambulating, and using mobility aids such as walkers, crutches, canes, and wheelchairs.
+ Collects urine and stool samples and performs blood glucose monitoring via finger sticks, documenting and reporting results to the RN/LPN/LVN.
+ Communicates patient information effectively to the care team, adapts to change, and maintains professionalism in all interactions.
+ Maintains a clean, neat, and safe environment for patients and staff, adhering to infection control and safety protocols, including appropriate use of personal protective equipment (PPE).
+ Participates in performance improvement initiatives, risk management reporting, and compliance with National Patient Safety Goals and Core Measures.
+ May be required to maintain continuous visual observation of the patient and remains with them at all times unless relieved by appropriate personnel.
+ Performs other duties as assigned.
+ Complies with all policies and standards.
**Qualifications**
+ 0-2 years of experience in an acute care setting or currently enrolled in a Nursing program preferred

The successful candidate should be able to perform ALL of the following functions at a pace and level of performance consistent with the actual job performance requirements.
1. Maintain a good working relationship with facility staff, nursing staff, and contract providers and outside provider agencies.
2. Utilize established corporate, facility and correctional policies and procedures in making decisions, but use sound independent judgement in meeting the responsibilities and performing the duties of the position.
3. Assist in the formulation of facility policy for the medical unit, interpret, enforce and adhere to policies, procedure or contract requirements.
4. Evaluate and recommend methods of improving operational efficiency and cost effectiveness of health-related services.
5. Communicate effectively and coherently to administration, staff, inmates/residents, visitors and the general public, particularly in situations requiring tact, diplomacy, understanding, fairness, firmness and good judgment. This includes interviewing applicants, giving information, instructions and directions, mediating disputes, advising of rights and processes and providing reliable testimony, in court and other formal settings.
6. Complete an annual written evaluation of all staff who are under direct supervision.
7. Provide for adequate for adequate staffing at the facility, fill in as needed during periods of short staffing, may perform nursing duties including, but not limited to, executing physician's orders, assisting physician in examinations and treatment, dispensing and administering medications, treating emergencies and screening patients for referrals.
**Qualifications:**
+ Graduate from an accredited college or university with a degree in nursing or in a healthcare/business related field is required.
+ Advanced degree in nursing or related healthcare field is preferred.
+ Three (3) years relevant management experience in a healthcare environment required.
+ A valid driver's license is required.
+ Must demonstrate knowledge of correctional custody methods and techniques, pertinent facility rules, regulations and standards, principles and practices of supervision and training, and principles and practices of management.
+ Minimum age requirement: Must be at least 18 years of age.
