**Primary Responsibilities:**
+ Conduct in-home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening
+ Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick and foot exam (as appropriate)
+ Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
+ Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
+ Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care
+ Educate members on topics such as disease process, medication and compliance
+ Comply with all HIPAA regulations and maintain security of protected health information (PHI)
Since this position involves in-home visits, the ability to work autonomously and extensive travel will be required. You should also be comfortable working with geriatric patients who have a variety of health complications.

**Required Qualifications**
+ Current, unrestricted NP licensure in the State of Practice, or the ability to obtain by assigned start date
+ Certification through the American Academy of Nurse Practitioners (AANP), or the American Nurses Credentialing Center (ANCC)
+ 1+ years of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse
+ Current, unrestricted RN licensure in the State of Practice, or the ability to obtain by assigned start date
+ Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
+ Ability to obtain DEA licensure/prescriptive authority post-hire in states where applicable
+ Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco users
+ Ability to lift a 30 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations
**Preferred Qualifications**
+ Experience in Gerontology, Cardiology, Internal Medicine or Endocrinology
+ Home care or home visit experience
+ Computer literate and able to navigate the internet
+ Proven excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
Compensation for this specialty generally ranges from $104,500 - 156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. Position in this function works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
**Primary Responsibilities:**
+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
+ Completes follow-up and post-discharge assessments according to documented standard operating procedure
+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
+ Provides counsel, support and education to staff as appropriate
+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence
+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
+ Perform related duties as necessary and other duties as assigned

**Required Qualifications:**
+ Completion of Nurse Practitioner accredited program
+ Current state RN license
+ Current NP license

**Preferred Qualifications:**
+ 2+ years clinical experience, primary care, home-based medical care, geriatrics or palliative care or emergency/urgent care

+ Knowledge of CPT, ICD-10 and HCPC codes
+ Knowledge of clinical standards of care

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits re subject to eligibility requirements). The salary for this role will range from $9,800 to 160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
***PLEASE NOTE*** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.

**PRINCIPAL RESPONSIBILITIES AND DUTIES:**
+ Assesses patient's knowledge and provides education regarding the relationship of diet and kidney disease and the impact on the treatment process.
+ Provides education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
+ Completes nutritional assessment and works with the interdisciplinary team to develop an individualized comprehensive plan of care for patients.
+ Maintains complete and accurate medical records including progress notes.
+ Provides ongoing assessment and counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
+ Identifies malnourished or at-risk patients and works collaboratively with the interdisciplinary team to identify appropriate interventions, resources, or solutions.
+ Monitors, adjusts, and documents response to nutrition therapy and addresses any issues impacting the patient's ability to achieve the goals.
+ Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V.
+ Reports on nutrition QAI results and participates in the interdisciplinary QAI program.
+ Reviews nutrition related lab results:
+ Counsels patient and/or caregiver and formulates appropriate action.
+ Recommends treatment changes to the interdisciplinary team as appropriate.
+ Identifies candidates to refer to physician for nutritional supplements.
+ Communicates with physician and/or facility staff as appropriate.
+ Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable.
**EDUCATION:**
+ Registered Dietitian as per Commission on Dietetic Registration.
+ Board Certified Specialist in Renal Nutrition encouraged.
+ Current state licensure if applicable.
**EXPERIENCE AND SKILLS:**
+ 1+ year experience in clinical nutrition as a Registered Dietitian.
+ Previous renal experience preferred.
