Works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
**Position Highlights & Primary Responsibilities:**
+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
+ Completes follow-up and post-discharge assessments according to documented standard operating procedure
+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
+ Provides counsel, support and education to staff as appropriate
+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence
+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
+ On-call for caseload during business hours. Participation in clinical after hours on-call program on rotational basis
+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
+ Perform related duties as necessary and other duties as assigned

**Required Qualifications:**
+ Certification through the American Academy of Nurse Practitioners (AANP), or the American Nurses Credentialing Center (ANCC)
+ Current, unrestricted NP licensure in the State of Practice, or the ability to obtain by assigned start date
+ Current, unrestricted RN licensure in the State of Practice, or the ability to obtain by assigned start date
+ Proven knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems
+ Driver's License and access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy
**Preferred Qualifications:**
+ 2+ years of clinical experience, primary care, home-based medical care, geriatrics or palliative care or emergency/urgent care
+ Knowledge of CPT, ICD-10 and HCPC codes
+ Knowledge of clinical standards of care
+ Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts and benefit systems
***PLEASE NOTE*** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. **The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.**
Compensation for this specialty generally ranges from $104,500 - 156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. **What you will be doing:**
+ Monitors facility; interacts with participants and monitors equipment and participant safety. Educates participants on proper techniques to maximize exercise safety and effectiveness.

Coordinates and leads a variety of HealthFitness and program-specific health improvement programs & promotions. Creates and maintains bulletin boards, newsletters, and other marketing & communication materials.

+ Leads work conditioning classes, delivers proactive, lineside job coaching, leading, or facilitating pre-shift or during shift stretching programs, and leading educational sessions for employees.
+ Collects and documents appropriate program data to support the ROI process.
+ Collaborates with the Client's existing resources and programming, including Heath Services, Safety, Ergonomics, and well-being teams.

**Minimum Requirements:**
+ Bachelor's Degree in Exercise Science, Health Promotion, Kinesiology, or related discipline.
+ Current First Aid and CPR/AED certifications required from American Heart Association, American Red Cross, or American Safety & Health Institute. _Note: certification must have an in-person component and not 100% online/OSHA compliant._
+ Strong interpersonal communication and customer service skills including the ability to motivate others.
+ Computer proficiency in Microsoft Office programs including at minimum Word, Excel, and Outlook.
+ Ability to work effectively both independently and as part of a team with enthusiasm, initiative, and creativity.
+ Applicants must meet hiring requirements including but not limited to pre-employment drug test.

Home Health Aide provides patients with assistance in activities in daily living. The Home Health Aide performs varied activities and moderately complex administrative/operational/customer support assignments. Assists patients within the home with daily living activities. Typically works on semi-routine assignments.
A **Home Health Aide** ( HHA ):
+ Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care (POC).
+ Correctly assists the patient with self-administered medications by opening bottle caps for the patient, reading medication labels to the patient, checking the dose being self-administered against the prescribed dose on the container label and observing the patient takes the medication
+ Consistently takes accurate temperature, pulse and blood pressure measurements and recognizes and reports abnormal results to supervisor
+ Helps patient maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care and other activities
+ Assists in feeding patients. Is able to communicate basic principles of nutrition, observe and record food and fluid intake when necessary. Safely positions patient for meals and feeds or assists in self feeding
+ Assists with patient toileting including use of bed pan/urinal, change and position catheter bags and bag change procedures on well-regulated ostomies
+ Provides necessary skills to safely assist the patient with patient mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and CenterWell Home Health policy
+ Provides necessary skills to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. Documents interactions with patients, caregivers, doctors and other staff members appropriately, legibly, thoroughly and in the amount of time allowed
+ Practice acceptable infection control principles. Provide a clean, safe and comfortable environment
+ Willingly assists with other household duties including light laundry, bed changing and bed making, light meal preparation, light housekeeping and shopping (if no other assistance is available and an MD order is present).
