* Demonstrates, teaches, and supervises the patient, family members, and/or caregivers in patient care and treatment procedures.
* Evaluates the patient's response to care, updates physician and other pertinent caregivers, and reassesses on an ongoing basis the condition and needs of the patient.
* Completes required documentation according to agency policy.
* Supervises Agency Ancillary staff such as HHA and LPN, including preparing a Home Health Aide Care Plan and monitors both Home Health Aide Care Plan and Nursing Care Plan implementation.
* Maintains equipment, supplies, and environment in a safe manner.
* Maintains knowledge of current trends in nursing and home care.
* As an RN Case Manager, assures communication of patient's plan of care is carried out and goals are appropriate for patient's condition and visit plan is effective.
* Maintains professional standards, compliance with State, Federal (CMS conditions of participation), and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations - standards of care guidelines.

General Requirements • Current RN license in the State of Michigan • Valid Driver's License • Proof of Auto Insurance • Active American Heart Association BLS Certification upon hire Work Experience • Minimum of 1 year RN experience in acute care, long-term care, or home care OR 2 years' experience as an LPN or CNA in a Long Term Care Facility Education • Must be a graduate from an accredited school of Nursing Specialized Knowledge and Skills Required: • Excellent assessment skills • Demonstrated ability exercise independent judgment • Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team • Demonstrated understanding of public health principles and practices • Demonstrated ability to document and assemble data in proper format • Ability to enter/exit patient homes that may/may not be handicap accessible • Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job Preferred: • Bachelor's Degree in Nursing • Recent experience in home care or public health

  • Administer, build and support Epic, Rhapsody, SQL, JavaScript, APIs, and C# solutions to deliver end-to-end system integrations for the health system
  • Creatively design automation and integration solutions spanning disparate technologies of varying levels of sophistication
  • Continually review and recommend new tools and methods that enhance the stability of our integrations
  • Provide technical direction and project coordination for integration and data-conversion projects
  • Follow our structured development life cycle from requirements gathering, design, build, testing and deployment

Systems Analyst II - Oracle Health Clinical Platforms

Oracle Health is seeking a Systems Analyst II to provide frontline support for our clinical platforms, with a focus on Cloud Sepsis and Legacy Chart Search . This role supports Linux-based, pull-driven data processing systems and requires a strong blend of technical troubleshooting, systems coordination, and stakeholder communication. The Systems Analyst II will play a critical role in ensuring system reliability, timely issue resolution, and operational continuity.

Key Responsibilities

  • Deliver Tier 1 and Tier 2 application and infrastructure support for Linux-based platforms, including Cloud Sepsis and Legacy Chart Search.

  • Monitor, triage, and resolve support tickets related to data pull pipelines and platform dependencies .

  • Coordinate and participate in testing and validation activities during upgrades, code deployments, and configuration changes.

  • Document bugs, operational issues, and resolutions using Jira or equivalent tools , and track progress with IT, development, and hosting teams.

  • Maintain and audit role-based access controls in alignment with organizational policies.

  • Provide end-user training and support , including troubleshooting guidance and system functionality education.

  • Represent platform users in technical forums, operational reviews, and cross-functional working sessions .

  • Assist with log analysis, data flow validation, and infrastructure health checks as directed.
