Position Description:
The Triage Registered Nurse (RN) will provide assistance and consultation with patients over the phone to assess the severity of the patient's related health concerns using proper protocol. Works with the physicians, managers and staff to promote quality and cost-effective daily operations. Establishes and maintains good interpersonal relationships with patients, their families, the public and co-workers. This position works collaboratively with all other staff to assist the patient and facilitate successful patient interaction with the practice team. This position reflects and carries forward the mission and goals of the practice both internally and throughout the community.
Requirements:
* Education/Certification/Licensure
* Current Licensure as a Registered Nurse in the State of New York.
* Current Basic Life Support certification.

* Experience/Skills/Proficiencies:

* If hired as a Graduate Nurse, a NYS limited licensure work permit must be obtained within ninety (90) days of graduation and the GN must successfully pass the NCLEX exam within 120 days of the beginning of employment to obtain the required professional nursing licensure with New York State.

Pay Range: $6.25 - 53.70
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

In this position, this Clinical Social Worker provides evidence based mental health services with a focus in trauma informed care, harm reduction, and recovery-oriented care when addressing patient concerns related to new HIV diagnosis, struggling with an existing HIV diagnosis, depression, anxiety, self-injurious behavior, suicide concerns that can include thoughts/behaviors/attempts, psychological trauma, and areas of concern related to possible LGBTQ identity. This Social Worker contributes to the achievement of optimum health, satisfaction, and cost-effective care by addressing the behavioral, social and environmental issues which may impede care. This position provides comprehensive psycho-social management and support to at risk patients and families through assessment, completion of letters of support for GAMST as needed, psychotherapy (individual and groups), counseling, intervention, planning, and referrals to meet the needs of patients. The Social Worker should possess knowledge of a wide range interventions and treatment modalities, such as CBT, DBT, EMDR, and/or MI. This is an essential role to support the assessment of behavioral health needs of a high-risk population in the community and surrounding area served in Inclusive Health Services Clinic at Upstate. This position is part of the interdisciplinary team that recommends and delivers appropriate therapeutic interventions in the context of both specialty and primary care. Other assigned duties and tasks are possible based upon operational needs.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
Preferred Qualifications:
L.C.S.W preferred.
Work Days:
Monday-Friday, days with rotating evening/extended hours.
