Become the Case Manager you always wanted to be
  • Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
  • Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
  • Participate in planning for and the execution of patient discharge experience.
  • Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
  • Facilitate team conferences weekly and coordinate all treatment plan modifications.
  • Complete case management addendums and all required documentation.
  • Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
  • Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
  • Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
  • Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
  • Perform assessment of goals and complete case management addendum within 48 hours of admission.
  • Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
  • Schedule and facilitate family conferences as needed.
  • Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
  • Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
  • Make appropriate/timely referrals, including documentation to post discharge providers/physician
  • Ensure accuracy of discharge and payor-related information in the patient record
  • Participate in utilization review process: data collection, trend review, and resolution actions.
  • Participate in case management on-call schedule as needed.

Qualifications

  • License or Certification:
    • Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
    • If licensure is required for one’s discipline within the state, individual must hold an active license.
    • Must meet eligibility requirements for CCM® or ACM™ certification upon entry into this position OR within two years of entry into the position.
    • CCM® or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.

·    Minimum Qualifications :

    •  For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
    •  For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor’s degree and graduate degree is preferred.
    • 2 years of rehabilitation experience preferred.

Master's degree in counseling psychology or a related field and 2 years of professional experience

Preferred Qualifications

UHC Minimum:
  • Master's degree in social work, Marriage and Family Therapy, Psychology, Counseling, or related field.
  • Must have at least an associate license in the State of Georgia (LMSW, LAPC, LAMFT)
  • Must apply for full licensure within 90 days of meeting State clinical and supervisory hour requirements.
  • 2 or more years' experience in the evaluation and treatment of individuals whose age and psychological development are similar to traditional and non-traditional college students.
UHC Preferred:
  • Independent licensure in GA by start date (LPC, LCSW, LMFT, PsyD, PhD)
  • CPR/AED certification required within first year of employment.
  • Training and 2 years or more of experience working with individuals from various backgrounds
  • Skill and/or interest in preventive psychological education and outreach programs.
  • 1 year or more of experience working in a college counseling center.
Position Summary

The Mental Health Professional II is an integral member of the CAPS Professional Staff. Clinicians will provide primary, secondary, and tertiary services to eligible members of the UGA community, within the CAPS scope of service. These services include group, individual, crisis interventions, outreach, and consultation. Clinicians provide mental health evaluations, formulation of conceptualization, diagnoses, treatment plans, crisis intervention, and internal/external referrals as needed for best care. Services are delivered in accordance with UHC/CAPS policies and procedures. Clinicians follow professional and organizational standards for care and documentation. All are valued team members who participate in performance improvement initiatives and are actively involved in team activities, i.e. required meetings, committees. Team members demonstrate respect for all while maintaining inclusive attitudes and appreciation of individual aspects of diversity.

Knowledge, Skills, Abilities and/or Competencies
  • Strong interpersonal skills and the ability to work in a fast-paced work environment.
  • Two years of clinical background in a college or university counseling or mental health setting.
  • Training and skills in providing telehealth services.
  • Advanced skill with disorders typically seen at a university counseling service.
  • Background and familiarity with accreditation (JC, APA, IACS) requirements and standards.
  • Competence and background in a multidisciplinary setting.
  • Knowledge or interest in mental health outreach programming.
  • Ability and interest in working in a setting involved in the training and supervision of counseling and psychotherapy to students in training or post-graduate professionals.
  • Background in providing supervision of counseling and psychotherapy.
  • Competence and background in using an electronic health record.

**PRINCIPAL DUTIES AND RESPONSIBILITIES:**
- Maintains, troubleshoots, and repairs dialysis equipment, ancillary equipment, and water treatment systems.
- Maintains the physical plant, as directed.
- Ensures outside vendor services are completed per contract.
- Purchases and retain an inventory of service parts.

- Provides scheduled after hour technical on-call coverage for your clinics as needed.
**EDUCATION AND CREDENTIALS:**
+ High school diploma or G.E.D. required.
+ Associate Degree in electronics / biomedical technology, or equivalent (preferred but not required)
+ Valid Driver's License issued in the employee's state of residence required.
+ Continued employment is dependent on successful completion of Fresenius Biomedical Technician training within the first 24 months of service from date of hire.
**EXPERIENCE AND REQUIRED SKILLS:**
+ Qualities and traits: collaborative, independent, analytical, mechanical, self-motivated, self-starter, continuously strives for staying up to date and increasing their knowledge/skills
+ Less than 1-year related experience.
+ Prior experience working in a mechanical, automotive, or construction industry, dialysis, or medical technical setting (preferred but not required).
+ Manual dexterity required - ability to use common hand tools and small power equipment.
+ Good verbal and written communication skills.
+ Ability to lift up to 50 lbs. and safely move large/bulky equipment
