As a Speech-Language Pathologist, you'll contribute to our success in the following ways:
+ Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
+ Evaluates the Speech-Language pathologist patient to determine the rehabilitation needs and potential for achievement and develops a plan of care in accordance with the findings.
+ Admits the patient and assesses speech-language therapy and all other needs according to OASIS and PPS regulatory guidelines. Makes the initial therapy evaluation visit and re-evaluates the patient's speech language therapy needs during each visit. Makes follow-up assessments according to OASIS and PPS regulatory guidelines.
+ Initiates plan for patient safety, using the patient, family, and community resources.
+ Communicates significant findings, problems, and changes in condition or environment to the appropriate supervisor, the physician, and other team members involved with patient care.
+ Participates in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning. Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines.
+ Writes physician orders to cover additional visits and changes to the Plan of Care, per agency policy.
+ Incorporates patient care goals established in the Plan of Care, into therapy care, as evidenced by documentation in therapy notes. Initiates and revises the Plan of Care in response to identified patient care issues.
+ Performs direct Speech-Language pathology services in accordance with accepted standards of practice and certified by the patient's physician.
+ Selects diagnostic and therapeutic materials appropriate to patient needs. Instructs and provides patients and/or their caregiver in the home therapy program providing modification and clarification when necessary.
+ Assures clinical notes indicate continuing communication and coordination of services with physician, other staff, and disciplines.
+ Ensures that all therapy notes, supply log/visit logs are accurate and turned into the office according to office policy.

What is Required?
+ Master's degree from an accredited educational program in Language Pathology
+ Current, unrestricted SLP license in the state of work

+ Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.

**Job Summary**
The Cath Lab Technician is responsible for assisting physicians during cardiac catheterization procedures and other interventional procedures to diagnose and treat cardiovascular conditions. This role involves preparing and maintaining equipment, monitoring patients during procedures, and ensuring the highest standards of patient care and safety. **Essential Functions**

+ Assists physicians during procedures by operating specialized equipment, such as fluoroscopy machines, intravascular ultrasound (IVUS), or hemodynamic monitoring systems.

**Somatus offers a Hybrid Telehealth environment with a combination of remote days and visits to members' homes.**
This position will be focused on high needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations that face multiple challenges, from accessing resources to adhering to a physician's treatment plan. The CHW-Patient Advocate will work as an extension of the clinical care team, specifically under the guidance of RN Care Manager and act as an advocate guiding them through their care needs. The individual taking this role will manage his/her caseload through in-person, telephonic and electronic means of communications and coordination. The Patient Advocate will be the first and primary representative of Somatus to our members. He/she will be the key holder of the patient relationship and trust and will be responsible for building this relationship. He or she will be tasked as the person that schedules the initial wellness exam and care management assessment meeting with Somatus clinicians. After an individual care plan is developed, will be the person that facilitates connecting and scheduling the many resources within and beyond Somatus to the patient (including the various members of our care team as well as PCPs, Nephrologists, etc.). The Patient Advocate will also be involved in the community to "plug in" the patient with others and help facilitate their overall wellbeing. This position is a market-based position.
Responsibilities

+ Operates CT equipment and processing systems to produce high-quality diagnostic images, ensuring compliance with departmental protocols and standards.
+ Prepares and positions patients appropriately for CT procedures, using suitable positioning and immobilization techniques based on patient needs and comfort.
+ Ensures accurate patient identification, verifies physician orders, and screens patients for contraindications to the examination.
+ Performs point-of-care creatinine testing and calculates GFR when required for contrast studies. Documents results and adheres to contrast administration protocols.
+ Administers contrast media as ordered, following safety protocols. Monitors patients for adverse reactions and responds appropriately.
