Job Description
As an RN at ASVH-Flagstaff, you will deliver compassionate, resident-centered nursing care to veterans in a skilled long-term care environment, working the night shift (6:00 PM - 6:30 AM). You'll play a key role in monitoring resident well-being, administering medications, and collaborating with the care team to ensure high-quality, safe, and respectful care.
Key Responsibilities
- Clinical Care & Medication Administration
- Administer medications and treatments per physician orders, following state licensure scope
- Monitor residents' conditions, vital signs, and responses to interventions throughout the night .
- Assessment & Care Planning
- Perform initial admission nursing assessments.
- Assist in developing and reviewing MDS assessments, quarterly care plans, and updates
- Documentation & Record Keeping
- Maintain accurate, timely clinical records: diagnoses, interventions, progress notes, and summary reports
- Prepare shift handover reports for day-shift staff.
- Supervision & Leadership
- Lead subordinate staff-including LPNs, CNAs-in providing ADL support (bathing, toileting, feeding, mobility)Perform charge nurse duties during the night, ensuring smooth operation and coverage
- Training & Quality Improvement
- Conduct in-service training and staff development; participate in AMS-driven performance improvement initiatives
- Communication & Collaboration
- Coordinate with interdisciplinary team and communicate effectively with residents and families
- Report changes in residents' conditions to physicians and initiate necessary interventions.
- Active RN license in Arizona
- High school diploma (minimum); BSN preferred
- Current CPR/BLS certification
- Valid Level 1 Fingerprint Clearance (AZ DPS)
- Negative TB test (within past year)
- Seven-year background check including OIG/SAM clearance
- Hepatitis B vaccination or documented declination
- TB Skin Test
- Skills Assessment Checklist
- Competency testing and negative drug screen
- COVID-19 vaccination card
- APS compliance
Knowledge, Skills & Abilities
- Expertise in gerontological/geriatric nursing and long-term care principles
- Proficient in federal, state, and agency regulations.
- Strong critical thinking, attention to detail, and clinical decision-making skills .
- Excellent verbal and written communication, documentation, and interpersonal skills.
- Ability to supervise staff, lead quality initiatives, and implement AMS tools .
