The Acute Care Registered Nurse (RN) provides inpatient nursing care to patients in compliance with the SC Nurse Practice Act and organization protocols, policies, procedures, and guidelines. The RN demonstrates clinical competency utilizing evidence-based practices and research in all areas of the nursing process: assessment, planning, implementation, evaluation, and outcomes. About the job Director of Case Managment
The Director of Case Management is responsible for the oversight of Case Management unit, including but not limited to clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. Accountable for the execution, adaptation and outcome of care management as prescribed in the initial Utilization Review Plan with regard to resources, appropriateness of care and adherence to a designated Geometric Length of Stay (GLOS) for each patient.
Essential Functions:
- The Director is responsible for the overall direction and management of these areas, including planning, organizing and directing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reporting needs within the RRHS, as well as government and regulatory reporting.
- The Director guides Care Coordination activities according to the needs, requirements, and policies of RRHS, the affiliated medical groups and health plans, and any Federal and State agencies, and according to standard practices of the professions under the director's accountability.
- The Director will consult and collaborate with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corportate or facility initiatives and current literature.
- This position will work closely with all departments at the medical center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge.
- The Director participates regularly in RRHS, Service Area, or Corporate meetings pertinent to the accountable areas, and also participates in corporate strategic planning and performance improvement teams and programs as necessary.
- This position requires the full understanding and active participation in fulfilling the Mission of the Organization. It is expected the director will demonstrate behavior consistent with the Core Values of the organization. It is expected the director will support the Organization's strategic plan and the goals and direction of the Performance Improvement Plan (PIP).
- Assists with developing specific departmental goals, standards, and objectives which directly support the strategic plan and vision of the organization.
- Assesses the quality of patient care delivered and coordinates patient care services with patients, staff, physicians, and other departments.
- Creates and fosters an environment that encourages professional growth.
- Integrates evidence-based practices into operations and clinical protocols.
- Works with physicians, nurses, ancillary staff and social services at the point of care to facilitate multi-disciplinary decision-making that is consistent with the goals and objectives of the plan of care and the wishes of both the patient and family.
- Must understand payer issues with regard to patient management and resulting implications of clinical decisions and anticipate to opportunities to reduce expense and capture revenue appropriately from admission through discharge.
- Educates Physicians and staff in Case Management standards and assists them in meeting the regulations and standards as requested by Joint Commission, OSHA, CMS, HCFA, AHCA and other regulatory agencies. Educates and mentors all staff in UR standards and LOS topics.
- Insures effective Utilization Review Process.
- In addition, this position considers the population served by RRHS and area clinical integration programs and leads efforts to optimize care coordination across the care continuum. This coordination ensures a plan of care for patients in all states of health needs.
Associate's Degree in Nursing required
Bachelor's Degree in Nursing or Master's Degree in related field preferred.
Current RN license to practice in NC (NC licensure or multi-state (compact) license).

+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
+ Completes follow-up and post-discharge assessments according to documented standard operating procedure
+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
+ Provides counsel, support and education to staff as appropriate
+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence
+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
+ On-call for caseload during business hours. Participation in clinical after hours on-call program on rotational basis
+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
+ Perform related duties as necessary and other duties as assigned

**Required Qualifications:**
+ Certification through the American Academy of Nurse Practitioners (AANP), or the American Nurses Credentialing Center (ANCC)
+ Current, unrestricted NP licensure in the State of Practice, or the ability to obtain by assigned start date
+ Current, unrestricted RN licensure in the State of Practice, or the ability to obtain by assigned start date
+ Proven knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems
+ Driver's License and access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy
**Preferred Qualifications:**
+ 2+ years of clinical experience, primary care, home-based medical care, geriatrics or palliative care or emergency/urgent care
+ Knowledge of CPT, ICD-10 and HCPC codes
+ Knowledge of clinical standards of care
+ Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts and benefit systems
***PLEASE NOTE*** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. **The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.**
Compensation for this specialty generally ranges from $104,500 - 156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
