The Home Care Nurse provides direct nursing care, treatment, and health instructions to patients in their homes.
Essential Duties:

* Demonstrates, teaches, and supervises the patient, family members, and/or caregivers in patient care and treatment procedures.
* Evaluates the patient's response to care, updates physician and other pertinent caregivers, and reassesses on an ongoing basis the condition and needs of the patient.
* Completes required documentation according to agency policy.
* Supervises Agency Ancillary staff such as HHA and LPN, including preparing a Home Health Aide Care Plan and monitors both Home Health Aide Care Plan and Nursing Care Plan implementation.
* Maintains equipment, supplies, and environment in a safe manner.
* Maintains knowledge of current trends in nursing and home care.
* As an RN Case Manager, assures communication of patient's plan of care is carried out and goals are appropriate for patient's condition and visit plan is effective.
* Maintains professional standards, compliance with State, Federal (CMS conditions of participation), and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations - standards of care guidelines.

**Position Purpose:**

**Department:** Atrium East - Med/Surg - Our Atrium East unit at our Chelsea hospital cares for Medical-Surgical patients. Patient diagnoses can vary from medical such as: COPD, Sepsis, withdrawal, GI bleeds, CHF or Surgical: bariatric, Mastectomy, colorectal surgeries, hernias. This unit has 24 beds and specializes in complex high acuity medical and surgical patients with length of stay from 3-5 days.
**Location:** Chelsea, MI hospital
**Pay:** $16.00-$19.49 per hour based upon applicable on the job patient care experience + shift premiums when working afternoons, overnights and weekend shifts

**Position Purpose:**
Exceptional opportunity for a Patient Care Assistant (PCA) to support our vision and serve with us as a compassionate and transforming healing presence in our community. We seek exceptional individuals from a variety of backgrounds, because diversity makes us stronger.
The PCA, under the direction of a registered nurse, is responsible for direct patient care considering age specific, developmental, cultural and spiritual needs. Training is provided to prepare our colleagues for success. The PCA assists patient in completing ADLs including: Bathing, showering, toileting, positioning, turning, transferring, ambulation using assistive devices and with feeding and menu selection. They are responsible to record patient information and required data in appropriate areas in order to meet documentation requirements.
**Shifts Available:** Evening Shift (3pm-11:30pm)
**Status Available:** Part time, benefit eligible (40 hours per pay period - every two weeks)
_Every other weekend and holiday rotation required_
**Required Qualifications**
· High school diploma, GED, or a minimum of 18 years of age.
· Recent experience desired.
