Position OverviewThe CT technologist works under the supervision of the physician, provides radiologic care, and services to patients by arranging and operating equipment in order to produce images of the human body for the purpose of diagnosis and treatment of anatomic and physiologic disorders. The CT Technologist ensures that the Radiology department meets current accreditation program and American Registry of Radiologic Technologists standards and achieves the Singing River Health System objectives. He/She operates equipment requiring detailed concentration for extended periods of time, stocks an adequate inventory of supplies at all times, and maintains quality control of procedures and quality assurance of equipment.

EducationGraduate from an accredited, state approved, School of Radiologic Technology required.
LicenseMust be currently registered by the American Registry of Radiologic Technologists. CT Registration by the American Registry of Radiologic Technologists preferred. Licensed by the Mississippi State Department of Health required.
CertificationMust complete BCLS certification by the end of position orientation; must maintain current BCLS certification.
ExperienceA minimum of six (6) months experience in Computerized Tomography Scanning required.
Physical DemandsWork is moderately active: involves sitting with frequent requirements to move about the office, move about the facility, and to travel to another facility within the SRHS service area. Work involves exerting a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. Work involves using many physical motions in performing daily work activities; subject to exposure of body fluids, communicable diseases, CRT (computer) equipment, sputum and tissues, which may carry the hazard of infectious disease. Work involves using repetitive motions: substantial movements of the wrists, hands, and or fingers while operating standard office equipment such as computer keyboard. Work involves being able to perceive the nature of sound at normal speaking levels with or without correction; the ability to make fine discriminations in sound. Work requires close visual and acuity and the ability to adjust the eye to bring an object into sharp focus, i.e. shift gaze from viewing a computer monitor to forms/printed material that are closer to compare data at close vision. Must be able to be active for extended periods of time without experiencing undue fatigue. Must be able to work schedules assigned with the understanding that changes may be instituted according to the needs of the hospital for off days, shifts or weekends.
Mental DemandsMust demonstrate keen mental faculties/assessment and decision making abilities. Must demonstrate superior communication/speaking/enunciation skills to receive and give information in person and by telephone. Must demonstrate strong written and verbal communication skills. Must possess emotional stability conducive to dealing with high stress levels. Must demonstrate ability to work under pressure and meet deadlines. Attention to detail and the ability to multi?task in complex situations is required. Must have the ability to maintain collaborative and respectable working relationships throughout SRHS and other organizations. Must possess proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.) Must have intermediate knowledge of MS Outlook,and Word. Must be able to provide appropriate clinical judgment and apply professional skills to a patient population of all ages. Must assume responsibility for accuracy as oversight could cause errors in asepsis procedures that could result in injury to the patient.

Position Overview:
The BHS Social Worker works in collaboration with physician and nurse practitioners to deliver mental health care services to patients. They perform psychosocial assessments and individual therapy; and, provides direct services to patients and families. The BHS Social Worker delivers mental health therapy services to a population of patients within their scope of certification and education. The BHS Social Worker works primarily in the outpatient setting with Singing River Health System. They work in any SRHS location affiliation or association, and assumes other duties as assigned within the expectations of the department. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.

Education:
Master's Degree in Social Work from an accredited school required.
License:

**Job Summary**
Molina Healthcare Services (HCS) works with members, providers, and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long-term care, for members with high need potential. HCS staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
**KNOWLEDGE/SKILLS/ABILITIES**
+ Follows member throughout a 30-day program that starts at hospital admission and continues its oversight through transitions from the acute setting to all other settings, including nursing facility placement and private home, with the goal of reduced readmissions.
+ Ensures safe and appropriate transitions by collaborating with the hospital discharge planner, as well as collaborating as needed or at the request of the member with hospitalists, outpatient providers, facility staff, and family/support network.
+ Ensures member transitions to a setting with adequate caregiving and functional support, as well as medical and medication oversight as required. Works with participating ancillary providers (LTSS/HCSS, DME), public agencies or other identified service providers to make sure necessary services and equipment are in place for a safe transition.
+ Conducts face-to-face visits of all members while in the hospital; home visits of high-risk members post discharge.
+ 40-50% local travel required.
+ Coordinates care and reassesses member's needs using the 2-day, 7-day and 14-day post-discharge timeline recommended by the Coleman Care Transitions Model.
+ Educates and supports member focusing on seven primary areas (ToC Pillars): medication management, use of personal health record, follow up care, signs and symptoms of worsening condition, nutrition, functional needs and or Home and Community-based Services, and advance directives.
+ Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.
+ Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
+ Facilitates interdisciplinary care team meetings and informal ICT collaboration.
+ ToC Coaches in Behavioral Health and Social Science fields may provide consultation, resources and recommendations to peers as needed.
**JOB QUALIFICATIONS**
**Required Education**
Any of the following:
+ Completion of an accredited Licensed Vocational Nurse (LVN)
+ Licensed Practical Nurse (LPN) Program
+ Bachelor's or master's degree in a social science, psychology, gerontology, public health or social work.
**Required Experience**
+ 1-3 years in case management, disease management, managed care or medical or behavioral health settings.
+ Knowledge of or experience using the Care Transitions Intervention or similar model; background in discharge planning and/or home health.
**Required License, Certification, Association**
+ If required by applicable State, an LVN/LPN license in good standing.
+ Otherwise, If licensed, license must be active, unrestricted and in good standing.
+ Must have valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation.
**Preferred Experience**
3-5 years in case management, disease management, managed care or medical or behavioral health settings.
