Under the general supervision of the Manager, Clinical Analytics, the Health Care Analyst is responsible for providing high quality analyses that answer questions about the most pressing healthcare needs and is comfortable utilizing various data modelling approaches to forecast future network's performance. Interprets and provides recommendations on quality and cost utilization trends by identifying key drivers of performance. Is capable of writing appropriate drill down queries (SQL based queries) to identify opportunities for improvement. Assures, by data testing, the quality and accuracy of BILHPN's claims and EMR data. Responsible for developing reports for upper-level management and preparing monthly reporting packages. Compiles medical, pharmacy and EHR related information via the use of computer-based applications which are then used to understand the current trends and drivers in the health care system and to make well-informed decisions about performance improvements of the plan and care management of the patients. Participates in setting definitions and efficient ways to analyze various healthcare concepts and promotes defining data related efficiencies. Handles the data with confidentiality especially around PHI and sensitive information.

This position is responsible for a full range of activities which ensure the analytical effectiveness and excellence of the BILHPN team.

Job Description:

Essential Duties & Responsibilities including but not limited to:

  • Understands the medical and pharmacy claims adjudication circle terminology, healthcare quality and EHR terminology and general healthcare related concepts.

  • On regular basis, from analytical perspective, tests healthcare concepts in BILHPN's data warehouse to assure accuracy and integrity of data.

  • Collaborates with other analysts and contributes/leads the process to define appropriate mappings for healthcare terminologies to be displayed in the data warehouse.

  • Understands and utilizes back-end entry to data warehouse, knows SQL and apprehends complexities of data architecture.

  • Designs and performs complex analyses for variety of stakeholders within BILHPN and network using either the super pivots, running own SQL queries to drill through data or other computer-based applications.

  • Translates analytical findings into easy-to-follow recommendations and summaries.

  • Performs own quality checks on performed analyses and utilizes internal benchmarks to verify the results. Documents performed processes and assures the correct documentation exists in the reporting documentation repository.

  • Collaborates with the Medical Director(s) and business associates (e.g., healthcare plan, PBM, consultants) to define the best approaches to find areas for improvement and trend mitigations.

  • Serves as analytical resource and internal consultant for organization's multidisciplinary quality and utilization improvement initiatives.

  • Upon discussions with other stakeholders, provides actionable data and models the impacts of various opportunities (such as cost savings opportunities) for BILHPN's population.

  • Assists in reporting package preparation for the Local Care Units' Leadership and Board of Directors.

  • Collaborates with others to identify, understand and promote opportunities to analyze data more efficiently and accurately, and improve data integrity.

  • Uses visualization tools to display data for key customers.

  • Works independently and can handle multiple projects simultaneously with little supervision.

Minimum Qualifications:

Education:

  • Requires a minimum of a Bachelors' degree. Prior experience from the payers or other provider system a plus.

  • A master's degree in directly related program may be considered in lieu of some of the required work experience.

Licensure, Certification & Registration :

  • None

Experience :

  • A minimum of 3-5 years of healthcare work experience is necessary either through a prior Health System or a Payer experience. Knowledge of programming language (e.g. SQL) is required, experience of Epic/Healthy Planet, data visualization tool of Power BI or Tableau is a plus

Skills, Knowledge & Abilities :

  • Strong written and verbal communication skills, project management skills, computer skills, and database skills.

  • Strong problem-solving abilities and analytical skills to ensure end-user needs are met.Possesses good organizational skills and the ability to multitask and prioritize daily assignments to ensure smooth workflow.

  • Advanced Excel Skills demonstrated ability in using query tools and relational data bases such as SQL.

Summary : The Healthcare Coordinator will coordinate medical nursing services and oversee healthcare services, and medical documentation, and provide the supervision of assigned residential nursing staff.

Essential Duties and Responsibilities:

  1. Provide supervision of the daily nursing activities within assigned geography.

  5. Assure regular communication between program directors, supervisors, and the nursing team.
  6. Provide supervision to the nursing team members assigned by holding monthly residential meetings and daily support for clinical questions or concerns.
  7. Communicate with the Director of Health Care by attending weekly supervision and by providing a weekly written report.
  8. Provide input and assist in the preparation of reporting staff members' evaluation process.
  9. Assist in the hiring and interview process for new nursing staff.
  10. The triage nursing team field calls for clinical assistance to assigned nursing team members.
  11. Attend pertinent meetings regarding changing health care status upon discharge from the hospital, LTCF, or other medically necessary situations.
  12. Oversee the transition and placement of new referrals to existing or newly developed Bridgewell programs.
  13. Other duties as assigned.

Education and/or Experience:

  1. Associate's or bachelor's Degree and current Registered Nurse license with the Massachusetts Board of Registration on Nursing.
  2. One year experience working with individuals with disabilities.
  3. One year of experience in a community setting.

  1. Knowledge of DDS Regulations, and CARF standards.

Responsible for performing diagnostic imaging procedures to aid in the diagnosis and treatment of patients. You will operate radiographic equipment, position patients, and capture high-quality images for interpretation by radiologists or other healthcare providers.
Essential Functions
-Perform various diagnostic imaging procedures as ordered by physicians or other healthcare providers.
-Prepare and position patients for imaging procedures, ensuring their comfort and safety throughout the process.

-Operate and adjust imaging equipment, including X-ray machines, CT scanners, MRI scanners, and related accessories.

-Recognize and troubleshoot technical issues to produce accurate and diagnostic images.

Required:

-Satisfactory completion of a formal Radiologic Technology Training program accredited by the Joint Review Committee on Education in Radiologic Technology (JRC/ERT) required.
-Licensure with the Department of Public Health to practice Radiologic Technology in the state of Massachusetts required. Registration or registration eligible* with the American Registry of Radiologic Technologists required, for Radiography.

A. PURPOSE AND SCOPE:

C.EDUCATIONAL REQUIREMENTS:

Must possess a state license for the state(s) in which he/she is employed to work.
Must be certified in CPR
Must be registered by the ARRT and/or NMTCB.

D.EXPERIENCE REQUIREMENTS:

A minimum of one-year prior nuclear medicine experience is required. Excellent interpersonal skills are required. Must be proficient in starting IVs.

E.PHYSICAL REQUIREMENTS:

Must be able to sit and utilize a computer for extended periods of time. Must be able to assist patients with mobility difficulties. Must be able to push, pull, and lift a minimum of twenty-five pounds.

F.INTERNAL/EXTERNAL INTERACIONS:

Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience. Typically responsible for: managing projects / processes, working independently with limited supervision, coaching and reviewing the work of lower level professionals, resolving difficult and sometimes complex problems.

Job Description

Minimum Qualifications:
1. Massachusetts RN Licensure.

2. 12 Months - Less than 3 Years of RN Experience.

3. Current Basic Life Support (BLS) Certification.

4. Department Specific: Certification(s) relevant to the department and the type of patient care being provided may be required.

Preferred Qualifications:
1. Bachelor of Science in Nursing (BSN).


Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list.Other duties and responsibilities may be assigned.

1. Participates in and maintains awareness of performance improvement initiates.

2. Conscientiously evaluates and integrates evidence-based practice and research into clinical practice.

3. Conducts and documents accurate clinical assessments.

4. Develops, implements, and evaluates the patient's individualized plan of care.

5. Uses professional nursing judgment to individualize the plan of care based on assessment of the patient's needs.

6. Manages own patient assignment by delegating and effectively based on patient needs and skills of team members.

7. Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources, and documents according to department standards.

8. Recognizes patient/family rights through support of their informed decisions and advanced directives.

9. Continually evaluates and revises the care plan according to changes in the patient's health status.

10.Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.

11. Supports the development of new staff members and helps facilitate their transition into unit.

12. Guides the professional growth of department staff through precepting.

13. Participates in professional development by identifying learning needs and seeking appropriate education.

14. Participates in departmental committees as needed.

Physical Requirements:
1. Occasional lifting of 30-35 lbs and turning patients.

2. Prolonged standing and walking.

3. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.

4. Exposure to infectious disease and possible carcinogenic chemicals.

5. Exposure of blood-borne pathogens.

6. Requires fine manual dexterity to lift and maneuver patients.

7. Requires visual and aural acuity necessary to monitor patient care.

Skills & Abilities:
1. Ability to effectively communicate with patients, families, physicians, and healthcare team.

2. Knowledge of and application of the nursing process.
