Works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.

**Primary Responsibilities:**

+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants

+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable

+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable

+ Completes follow-up and post-discharge assessments according to documented standard operating procedure

+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions

+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided

+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer

+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness

+ Provides counsel, support and education to staff as appropriate

+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence

+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery

+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews

+ Perform related duties as necessary and other duties as assigned

**Position Highlights & Primary Responsibilities:**

+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable

+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter, including visits to address changes in condition

+ Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization

+ Consults with Physician Leaders and coordinate with PCP in these decisions as available

+ Comfortable with basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc. New hire and annual skills training provided

+ Participates in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery

+ Participates in clinical after hours on-call program on rotational basis

Summary The Harry S. Truman VA in Columbia, MO, is recruiting for an Intermediate Care Technician. The Intermediate Care Technician will function in the Emergency Department within Ambulatory Care Nursing Service. Responsibilities The incumbent assists ED licensed personnel with the delivery of care to patients with complex, specialized, and potentially life-threatening situations. This may include setting up equipment and passing instruments in the event of: cardioversions, insertion of thoracostomy tubes, or initiation of mechanical ventilation to restore normal physiological function or prevent deterioration of patient's condition. The incumbent must possess the knowledge to recognize life threatening situations and act to correct these emergent situations according to established national or local protocols. The incumbent initiates cardiopulmonary resuscitation (BLS) and use of automatic external defibrillator(AED) once trained and certified and where appropriate according to American Heart Association (AHA) protocols and policies. Assists with airway management using bag-valve-mask (BVM), oral airway or nasal airway. May use pulse oximetry, capnometry, capnography, and/or other secondary confirmation methods to ensure correct placement of an emergency advanced airway. Operates specialized medical equipment such as defibrillators, electrocardiographs, and oxygen apparatus in accordance with applicable training and standards. Observes patient status and keeps licensed personnel informed of any change in condition. The incumbent provides support, relief, and coverage as needed to central station telemetry. The incumbent documents in the telemetry record and reports all observed electrocardiographic (ECG) changes in the patient's condition to the licensed staff. Incumbent performs measurements of PR,QRS, QT, RR, and QTC intervals, interprets rhythm, and prints rhythm strip every 8 hours or as required for events and informs appropriate staff. Incumbent recognizes lethal rhythms/arrhythmias (i.e. ventricular fibrillation and asystole) and immediately notifies appropriate staff. Incumbent admits, discharges, and assigns telemetry equipment to patients in central monitoring automated system. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent one (1) year of full-time study. If that number cannot be obtained from the school, 18 semester hours will be considered an academic year of graduate study. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended.
