ChristianaCare HomeHealth is seeking a Full Time, 40 hours a week, LPN . The successful candidate will support the area in and around Dover/Smyrna/Milford; work day shift hours, one weekend a month, 2 holidays a year, and two on-call shifts a month.

Duties and Responsibilities:

  • Give skilled nursing care and prescribed treatment to geriatric patients in the community.
  • Must be IV proficient, upon hire we will train new staff and expect proficiency at 90 days.
  • Develop and implement care plans, which are goal oriented and patient centered and consistent with available resources.
  • Develop plan for discharge of patients.

Requirements:

  • Must possess a current DE LPN license, or compact state license
  • 1 year of LPN experience
  • Previous Home Healthcare and Med-Surgical experience preferred
  • IV proficient or willing to train, and perform IV functions.

About This Position

ChristianaCare is currently seeking a full-time Registered Nurse for our Caregiver Health Center in Newark, DE. Some occasional travel will be needed to provide coverage at other ChristianaCare campuses.

In this role, you will be one of a team of four Registered Nurses working in collaboration with our Human Resources Department to provide care for our amazing ChristianaCare staff.

Work Schedule

Monday - Friday, 7:30AM-4:00PM

Principal duties and responsibilities:

  • Demonstrate Knowledge & skills necessary to provide care to adolescent, adult and geriatric patients, including knowledge of growth development, the ability to obtain and interpret information to identify patient needs and provide care needed
  • Performs Wellness Visits, Advance Care Planning and Transitional Care Management call
  • Triages walk-in patients and ensures appropriate disposition
  • Answers and respond to telephone messages under the direction of the provider

CONTEXT OF THE JOB:

Student Health Services (SHS) is the primary student health unit on the University of Delaware Campus. Situated within the Division of Student Life, SHS provides essential services and support to students and campus stakeholders to enhance the development of personal and social responsibility and to foster safe, healthy, inclusive, and supportive communities at UD. Under the general direction of SHS primary care providers, the Nurse I provides and coordinates comprehensive primary care and services to the student population. The Nurse I operates in a clinical setting, assesses patient and associated needs, implements intervention plans, and evaluates response. The Nurse I collaborates with other health care team members and/or community services, as appropriate. This position will provide experience and growth potential to serve additional nursing roles.

MAJOR RESPONSIBILITIES:

* Participates in and completes orientation alongside an experienced nurse, tailored to support the transition into this entry-level role.
* Obtains health histories and performs brief physical assessments on a selected patient population, including acute/chronic medical problems, as per provider-approved protocol.
* Utilizes the Electronic Medical Record for scheduling appointments, charting encounters and any other documentation regarding the care of the patient.
* Reviews medical records including laboratory results and immunizations and consults with the primary care provider, as appropriate.
* Provides patient education and instructions within area of specialty to patients, families, health care staff, faculty and students.
* Practices safety, environmental, and/or infection control methods.
* Maintains supplies in areas of work, i.e., exam rooms and the procedure room.
* Evaluates equipment readiness, such as EKG machine, nebulizer, pulse oximeter, vitals machines, glucometer, crash carts and contents and other items as needed.
* Participates in clinical quality improvement studies as directed by supervisor.

QUALIFICATIONS:

* State of Delaware Licensed Registered Nurse (or RN licensed in a compact state), and a graduate nurse or a nurse with up to three years of nursing experience.
* Ability to react effectively in emergency situations.
* Knowledge of patient evaluation, CPR, patient care charts and patient histories, drugs and their indications, contraindications, dosing, side effects, and proper administration and primary care principles and practices.
* Ability to observe, assess, and record symptoms, reactions, and progress.
* Ability to work effectively with a wide range of constituencies in a diverse community and rapidly changing technical environment.

PURPOSE

YOUR TASKS AND RESPONSIBILITIES

  • Drive clinical demand, education and sales, building deep understanding of disease state, unmet needs and treatment options to effectively educate healthcare professionals and business stakeholders on the unique benefits of assigned Women's Health products while balancing both efficacy and safety through approved clinical information for healthcare professionals and health systems;

  • Demonstrate expertise and knowledge of disease state, the marketplace, competitors, industry, and cross-functional activities/plans as well as possess analytical rigor to anticipate and identify business opportunities and challenges;

  • Prioritize sales and educational activities in accounts that produce highest ROI in both unit and revenue growth;

  • Partner with the Area Business Development Lead and monitor contract performance and ensure completion of quarterly customer business review meetings on top priority accounts. Support implementation of pull-through initiatives that accelerate demand and contract performance on contracted accounts and also priority accounts such as; IDNs, Academic Medical Centers, etc;

  • Penetrate established accounts to gain additional market share for product portfolio;

  • Effectively discover and address account needs with compelling messages and solutions;

  • Execute at a high level of communication and teamwork within your area squad;

  • Take ownership and demonstrate being fiscally responsible of your territory budget including Professional Education Budget, Displays/Conference, and all resources;

  • Dynamic development and execution of a comprehensive strategic and tactical territory sales business plan utilizing data reporting platforms including quarterly plan to achieve overall sales and revenue goals for all promoted products;

  • Deliver results by collaborating and partnering within your Squad; with Squad leader, peers, Medical Team, and all applicable internal stakeholders to execute/develop/improve and implement sales strategies;

  • Document all field activities in Veeva Irep;

  • Demonstrate procedural proficiency and knowledge of clinical data for all products;

  • Demonstrate procedural proficiency of all pricing and customer access channels that support portfolio;

  • Partner with targeted physicians/centers to develop referral networks for portfolio;

  • Thorough understanding of competitive landscape, including Claims Data within your territory;

  • Ensure that all assigned employee training is completed and up- to-date;

  • Operate strictly within the corporate code of conduct and compliance guidelines.

REQUIRED QUALIFICATIONS

  • BA/BS degree required;

  • Strong negotiation skills with in-depth knowledge and experience in contract negotiations, strategy, understanding of contract terms and conditions, and relationship management skills;

  • Excellent verbal and written communication, problem solving and interpersonal skills;

  • Demonstrates solutions-oriented approach and ability to influence without authority;

  • Valid Driver's license with clean driving record;

  • Up to 50% travel.

PREFERRED QUALIFICATIONS

  • Minimum of two years of sales experience; Medical device & Pharmaceutical;

  • Ob/gyn specialty experience;

  • Strong knowledge of healthcare management, compliance, payer landscape, market and competitive landscape, commercial and government channels;

  • Strong analytical skills with the ability to create and interpret financial models.

Employees can expect to be paid a salary of between $112,000 - $168,000. Additional compensation may include a bonus or commission (if relevant). Additional benefits include health care, vision, dental, retirement, PTO, sick leave, etc. This salary range is merely an estimate and may vary based on an applicant's location, market data/ranges, an applicant's skills and prior relevant experience, certain degrees and certifications, and other relevant factors.

Position Overview

The purpose of this position is to provide technical assistance and supportive patient care to meet the needs of individual patients throughout assigned areas.

Essential Job Functions
  • Collect, report, and document clinical data. Take vital signs, height and weight, intake and output, pulse oximetry, and collect specimens
  • Collect laboratory samples and perform lab testing in accordance with established policies and procedures, as well as COLA and CLIA regulations
  • Assist with patient activities and care as assigned by the charge nurse/designee as indicated by the patient’s plan of care
  • Provide for superficial wound care
  • Provide for patient safety related to age and condition
  • Maintain patient privacy and confidentiality of information and records at all times
  • Provide and maintain a clean, safe environment and perform and/or assist others to perform proper aseptic technique, isolation procedures, and infection control measures
  • Assist with the admission, transfer, and discharge of the patient
Other Job Functions
  • Prepare patients and chaperone during examinations/procedures or assists with other patient treatments
  • Maintain patient care areas with appropriate supplies
  • Assist with patient restraints and maintain precautions; collect, report and document data related to patient needs/emergencies and unusual patient and family behavior
  • Travel to all facility locations as required
  • Attend staff meetings or other company sponsored or mandated meetings as required
  • Perform additional duties as assigned
Basic Qualifications
  • High School Diploma or GED, required and completion of one of the following:
    • Completion of Medical Corpsman program and one year of applicable experience (OR) 
    • Completion of EMT-P (Emergency Medical Technician-Paramedic), AEMT (Advanced-EMT), Licensed Paramedic (LP) course/program and one year of applicable experience (OR)
    • Completion of EMT-B (Emergency Medical Technician-Basic) and 1+ years of emergency room experience as an ER Technician or hospital equivalent experience (Oklahoma, North TX, Wisconsin & Pennsylvania markets only) (OR)
    • Completion of the first year of education and current enrollment in an accredited School of Nursing and one year of applicable experience (OR)
    • 3+ years of emergency room experience as an ER Technician (2+ years in Pittsburgh market only) (OR)
    • 3+ years as a CNA with relevant hospital experience (Wisconsin market only)
    • 3+ years as a Certified Anesthesia Tech/OR Tech experience (Wisconsin market only)
  • Current BLS certification through the American Heart Association or the American Red Cross, required
  • Current ACLS/ALS certifications through the American Heart Association or the American Red Cross, required in all markets with the exception of Pennsylvania (strongly preferred) and Wisconsin (only required for EMT-P)

PRIMARY FUNCTION:

To perform hygiene/comfort measures and selected nursing and phlebotomy procedures for patients under the supervision and direction of the professional nurse.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Demonstrates skills and knowledge necessary to provide care appropriate to neonatal, pediatric, adolescent, adult, geriatric patients, including knowledge of growth and development. The ability to obtain information to identify patient needs, and to provide care as needed.
  • Performs assigned nursing procedures for the comfort and well-being of patients.
  • Responds to patient crisis and initiates interventions within the scope of responsibilities and notifies RN.
  • Performs successful venipuncture and finger sticks on adult patients, following all standard laboratory safety policies.
  • Performs defined procedures on point of care instrumentation.
  • Responds with promptness, empathy, and interest to needs of patients.
  • Communicates to patients, employees, and physicians utilizing courteous verbal and written communication skills.
  • Completes mandatory in-service education.
  • Performs essential unit specific tasks under the direction of RN, e.g. EKGs.
  • Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • BLS Certification
KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:Knowledge of basic nursing care provided for specific patient population of unit.
  • Knowledge of department standards, selected procedures, nursing guidelines and procedures. Ability to perform successful venipuncture and micro-procedures on adult patients.
  • Knowledge of Performance Improvement purposes and process. Ability to perform unit specific tasks as directed by the RN.
  • Ability to recognize changes in patient status and take appropriate action.
  • Ability to communicate effectively utilizing verbal and written communication skills. Ability to perform elementary math skills.
  • Ability to perform specific nursing and phlebotomy skills as outlined in nursing and laboratory procedures.
  • Ability to use appropriate safety measures for patients, employees, and self. Ability to perform CPR.
  • Ability to follow universal precautions at all times. Ability to exercise judgment, tact, and diplomacy.
  • Ability to respond to patient crisis situations under the direction of the RN, following Clinical Practice Guidelines.

BCLS certification - Preferred

PHYSICAL DEMANDS:

Ability to stand, walk, sit, bend, lift, push, pull, carry, reach, twist, handle, stoop, climb as per unit based Physical Demand Checklist.

WORKING CONDITIONS:

Frequent exposure to biological elements with occasional exposure to chemical and environmental elements, as per unit based Physical Demands Checklist.

Hourly Pay Range: $19.84 - $29.76 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
