What you do:

Position Overview:

This is a professional position responsible for providing behavioral health services to children, adolescents, and their families.   • Collaborates with psychiatrists and psychiatric nurse practitioners in medication reviews and presents case data with respect to consumer behavior and affect to properly manage consumer psychotropic medication regimens.
  • Provides case management services for consumers and families, developing treatment options, determining appropriate service delivery agencies, monitoring client progress, and evaluating and modifying treatment plan as needed.
  • Maintains case documentation and ensures the security and integrity of private health information in compliance with the provisions of HIPAA, Ottawa County, and CMH policies and procedures.
  • Provides information about behavioral health services, as needed, to community partners.
  • Serves as an advocate for greater access to affordable, quality behavioral health care for the target population.
  • Collaborates efficiently and effectively with other community agencies. This may include written or verbal summaries of CMHOC services or consumer response to services, and testifying in court, if so ordered.

Required Education, Training and Experience

Master's degree in social work, psychology, counseling, or a related mental health field is required. Two years of relevant professional experience in a mental health agency desirable. Twenty-four hours of child-specific behavioral health training per year required, provided by CMH.

Lived experiences with mental illness, developmental disabilities, or substance use disorders are valued.

  • Technical Expertise: 
    • Serve as the primary internal technical resource for querying and obtaining data from OnPoint’s EHR and data warehouse, as well as adjacent systems and platforms, in response to requests from internal and external stakeholders. 
    • Develop and maintain reports, data visualizations, and for use across the agency. 
    • Provide training and technical assistance to coworkers related to accessing and interpreting available data, as well as the use of data analytics tools such as Power BI. 
    • Write work instruction manuals, data capture guidelines, or standard operating procedures. 
  • Reporting and Data Analytics:
  • Compile and analyze data necessary for fulfilling reporting requirements, managing program operations and agency administrative functions, and quality improvement.

    • Develop tools (e.g. reports, dashboards, and audit processes) to monitor and improve the data integrity, accuracy and completeness of records, and adherence to agency documentation standards.  
    • Provide support to agency leadership in the development of key performance indicators.  
    • Advance quality improvement initiatives, performance management, and fidelity measurement through the provision of operational and performance indicator data.

The registered professional nurse (CAP RN 1) position is an entry level designation into the Clinical Advancement Program for Registered Nurses. Promotes infection control, equipment and environmental safety.

Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.

Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

Makes referrals to Social Worker and Registered Dietitian as appropriate for individual patient education and counseling.

Checks availability of adequate emergency equipment and supplies as assigned.

Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.

Supports and promotes the FMCNA's mission, vision, values, and customer service philosophy.

The position provides direct patient care that regularly involves heavy lifting and moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. The employee may occasionally be required to move, with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.

Day to day work includes desk work, computer work and interaction with patients, facility/hospital staff and physicians. The position may require travel to training sites or other facilities.

Assigned oversight of Patient Care Technicians/LPNs as a Team Leader or designated Nurse in charge, after the following:

~Successful completion of all FKC education and training requirements for new employees plus

~Must have a minimum of 9 months experience as an RN plus

~ G raduate of an accredited School of Nursing.

Entry level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.

Minimum of 9 months experience as a Registered Nurse (preferred).

Successful completion of FKC Education and Training requirements for new employees.

Successful completion of all annual required education/training/competencies.

Successful completion of on-going training and education as assigned or required.
