Responsibilities
-
Conducts ongoing assessments to meet the needs of patients served.

Communicates between supervisors, direct care staff members, family members, guardians, and collateral individuals regarding an individual's progress.

Provides a monthly schedule to supervisors of appointments, home visits, and observations to monitor work advancement.

Coordinates home, work and/or community-based visits to meet the requirements of observations, chart reviews, staff interviews, and data analysis.

Ensures the Behavior Intervention plan is implemented effectively and properly.

Participates in Interdisciplinary Team Meetings and other pertinent meetings as required.

Completes monthly and/or quarterly progress notes.

Organizes training to certify staff and other team members are competent in the implementation of the Behavior Intervention Plan and/or documentation sheets.

Provides follow-up interviews, home visits, and observations for continued support of an individual's needs.

Qualifications

Must have experience with behavioral intervention and/or strong educational emphasis on learning and developmental psychology.

Two or more years of experience working with individuals with developmental disabilities or behaviorally challenged population.

Must have reliable transportation.

Ability to perform physical tasks, including standing for extended periods, bending, move from seated to standing regularly.

State Specific Requirements:

Illinois:Board Certified Behavior Analysts (BCBA), Licensed Clinical Psychologist, Board Certified Associate Behavior Analyst (BCaBA), or a minimum of a bachelor’s degree with 1500 hours of supervised experience performed by a Licensed Psychologist or BCBA.

Indiana:A master’s degree in a human related field (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)

Kentucky:A master’s degree in a human related field and 2 years of experience (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)

Georgia:Master’s degree in psychology, special education, counseling, social work, or related fieldORBCBA, Licensed Professional Counselor, Licensed Clinical Social Worker, or Licensed Psychologist/Psychiatrist. All applicants MUST have at least 2 years of experience providing positive behavioral supports (writing BSP, functional assessment; plan development, training, and implementation) to people with intellectual and/or developmental disabilities. Job Description
To help ensure that each participant remain stable at the site and make progress toward his permanency goals prescribed by his permanency order. This position will be responsible for performing day-to-day support services assistance to the participants of the program.
To manage all aspects of a participant's case (Court visits and court reports, ACR reports and visits, participant advocating, medical/dental visits, hospital visits, family attachment, etc.) as it relates to their residence and growth while at Garden of Prayer Youth Center.
ORGANIZING AND COORDINATING
Screening/Intake
- Conduct intake process and intake meetings with therapist, DCFS worker, and participant.
- Assure the retrieval of necessary documentation for participant admission into the program.
- Conduct participant orientations to familiarize them with program expectations, conduct clothing inventory, and build rapport.
- Conduct the process of receiving new participants.
- Ensure 906 is completed promptly.
- Some duties may be reassigned and other duties may be asked of you as required.
- Maintain proactive, effective communication with participants, direct care staff, therapist, and Program Coordinator regarding all participant issues and concerns.
- Maintain proactive, effective communication with participants, participant family, DCFS workers, DCFS Monitor, DCFS Licensing Monitor, Program Coordinator, GAL, and all other stakeholders involved with the participant's case.
- Ability to observe participant behavior and participation, assess appropriateness of such, and prepare reports and other correspondence in order to advocate and communicate all pertinent information regarding participants.
- Build positive rapport with participants, colleagues, all DCFS personnel, and all other stakeholders involved with participant's case.
- Set a good example for participants including cleanliness, punctuality, sportsmanship etc.
- Maintain positive public relations with participant's families.
- Facilitate interaction between participant, and participant as well as participant, and staff.
- Participate in department and company employee meetings.
- Provide frequent feedback and positive reinforcement to participants.

- Maintain client case file in AP-5 order
- Complete and/or make sure that UIR's are written, distributed and maintained
- Assure that the client has access to and meets with GAL and all legal representation
- Assure that all court, probation or parole orders are followed and adhered to
- Arrange sibling or parental visits if appropriate and upon request and document all visits
- Assure youth is at all court appearances when attendance is mandatory
- Weekly visitation to youth when hospitalized or incarcerated and provide youth with allowance
- Contacting SASS for an evaluation/screening in the event that a youth is psychiatrically hospitalized
- Obtain all necessary consents
- Unusual Incident Reporting
- All other duties as assigned according to DCFS policy

- Prepare and submit all documentation and reports accurately in a grammatically correct fashion.

QUALIFICATIONS
Education: Requires a bachelor's degree in social work or a related human services field from an accredited college or university
Experience: 3 years experience working with at-risk youth.
Credential: Requires DCFS licensure as a child welfare specialist
Candidates must have a valid driver's license, good driving record, and a clear background

**Job Summary**
Molina Healthcare Services (HCS) works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long-term care, for members with high need potential. HCS staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
