Position Summary:


The RT Vent Field Clinician is a Respiratory Therapist providing respiratory patient care to Vent patients for optimal outcomes. Provides respiratory care to patients in alternate sites in accordance with AdaptHealth's policies and procedures. Respiratory care will be preventative, rehabilitative, and palliative in nature. The RT will utilize all the resources available within the agency and community to accomplish care objectives. This position will provide education and care to the patient and communicate with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.


Essential Functions and Job Responsibilities:

  • Utilizes various sources of information to attain greater competence about his or her position, including attending educational events (including attending optional in-services) and asking questions.
  • Utilizes acquired knowledge to increase his or her competencies.
  • Consistently demonstrates ability to adequately complete all documentation and charting procedures in compliance with company policy and procedures.
  • Maintains complete and accurate patient files by updating all documents per company policy and procedures.
  • Reviews Plan of Treatments and Care Plans to assure they are accurate and up to date.
  • Documents procedures including how the patient tolerated a procedure, side effects and other pertinent information.
  • Assists with authorization for Ventilator referrals for patients.

  • Performs in-services to hospital staff, referrals, other professionals regarding equipment & issues of clinical nature.
  • Follows up with physician and referrals regarding patient status and documents accurately and in a timely manner.
  • Retain knowledge of and consistently adhere to procedures for the use of Personal Protective Equipment (PPE), infection control and hazardous materials handling.
  • Works to promote AdaptHealth by new program development, operational backup, personal visits, coordination of educational activities, etc.
  • Assume on-call responsibilities during non-business hours in accordance with company policy.
  • Uses clinical expertise in evaluating vent patients records once a ventilator set up has been completed by the branch Respiratory Therapist.
  • Ensures accuracy of prescriptions and plan of care was followed and documented. Also reviews delivery tickets, home inspection, ventilator check, and patient equipment competencies are complete and documented.
  • Maintains proficient knowledge of ventilator patients including compliance software, new technology, units, and supplies supported by Adapthealth.
  • Ability to demonstrate and instruct on use of vent units and supplies. Ability to make decisions for patients based on compliance data and assessment.
  • Communicates with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.
  • Maintains working knowledge of Medicare/Medicaid and other third-party payer guidelines related to ventilation.
  • Electronically documents patient care activity, intervention provided and all communication regarding the patient. Documentation is accurate, complete and follows company standards.
  • Appropriate steps taken to ensure recommendations and orders sent are acknowledged and followed up in a timely manner.
  • Responsible for accuracy, clarity, and timeliness of verbal and written communications as it relates to role.
  • Responsible for documentation that supports data collection to track and trend outcomes.
  • Assists in establishing clinical documentation when needed for third party reimbursement or justification.
  • Uses knowledge in working with referral sources to educate about best practice standards.

  • Develops, implements, coordinates, and evaluates clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental illness.   • Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice. Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment. May develop, implements, and evaluates behavioral medicine and health psychology programs in a variety of settings, including primary care. Provides consultation to other care providers and health educators on matters relating to mental health, health psychology and behavioral medicine.

  • May supervise Post Masters Fellows, Associate Clinical Social Workers, Associate Marriage Family Therapists or Associate Professional Clinical Counselors as needed if supervision course is completed.

  • May provide appropriate support to members family, including explanation of treatment, instructions in how to support treatment and interventions to increase acceptance of and adherence to treatment, at members request.

  • Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment to include referral of the member and/or members family to external resources, as appropriate. Participates in departmental program development, implementation and evaluation.

  • Reports safety concerns to mandated reporting agencies.

* Hematology/Oncology clinics and Infusion Center require within six month of hire to be in possession of a valid chemotherapy provider card certification issued by the (ONS) Oncology Nursing Society, annual education related to oncology and demonstrated competency. Within 12 months of hire and every 12 months while employed perform three (3) chemotherapy starts and administer three (3) chemotherapy pushes every year. Must take and complete an ONS education class every two (2) years.

OTHER REQUIREMENTS: A background investigation/fingerprint, and/or physical exam may be required prior to the start of employment.

Summary

SUMMARY: The Minimum Data Set (MDS) Coordinator is responsible for the timely and

accurate completion of the Federal and State assessment tool. Performs related duties as

required.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees

in this classification. However, employees may perform other related duties at an equivalent

level. Not all duties listed are necessarily performed by each individual in the classification.

1. Physically assess each resident and interviews staff as necessary to ensure good standard of practice

and as instructed in the Minimum Data Set User's Manual. Coordinates scheduling and notice of resident care

planning conferences and ensures communication of outcomes/problems to the responsible staff or resident.

Designs, promotes and organizes Resident education and counseling based on health needs. Ensure all MDS

information and care delivered is as outlined in the care plan and supported by documentation.

2. Facilitates accurate determination of the Assessment Reference Date; Provides interdisciplinary schedule

for all Minimum Data Set assessments and care plans as required by OBRA and PPS.

3. Assists Case Manager with the transfer of patients to other health care providers or to home. Transfer

responsibilities include determining the appropriate level of care placement in collaboration with the physician,

patient, and family, if appropriate; discussing available insurance benefits with the health care team following

verification by the Business Office; obtaining required authorizations and ordering/or arranging for DME, SNF,

and transportation; and referring personal payment issues to the Financial Analyst as soon as possible to minimize

reimbursement. Obtains appropriate signatures as required.

4. Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS

assessments based on clinical criteria as identified in the most recent version of the RAI User's Manual.

5. Assesses for significant changes using the 24 hour report, physician orders, and verbal reports to ensure that

the Minimum Data Set and care plans are reflective of those changes. Communicate needed changes in PPS Timeliness

and Assessment Reference Dates and deficiencies in completion of Minimum Data Set, RAPS, and care plans.

6. Corrects and ensures completion of the final Minimum Data Set and submits resident assessment data to the

appropriate State and Federal government agencies. Ensures timely submission of the MDS's to the State with proper

follow up on validation errors. Maintains validation records from the submission process in a systematic and orderly

fashion.

7. Assigns, assists, and instructs all staff in the RAI Process, Case-Mix, PPS Medicare, Medicaid, and the clinical

computer system in relation to these processes.

8. Responds to Resident and family concerns; ensures that each concern is documented and a resolution is

initiated and presented weekly at Medicare or Utilization Review meetings.

MININUM QUALIFICATIONS:

Education: Graduate of accredited school of nursing.

Minimum Experience: Minimum one year clinical experience in a hospital, long term care facility, or other healthcare related facility.

Preferred Experience: Previous experience as an MDS Coordinator; including completing Minimum Data Set (MDS) and Care Plans and experience with PPS and OBRA assessments.

Required Licenses/Certifications: Valid license to practice as a Registered Nurse in the State of California. Required

The Work.

The UKG (Legacy Kronos) Advanced Scheduler Consultant for Healthcare works closely with teammates to ensure the maximum value of our clients' workforce management investment is achieved.
