- Maintains acceptable productivity standard as determined by agency leadership.
- Participates in multi-disciplinary team meetings in order to share information and assist with patient management.

Qualifications:
- Graduate of an accredited nursing program and has experience as a Registered Nurse.
- Valid RN License from Virginia or reciprocal compact state required.
- 1 year of Home Health or Medical/Surgical nursing experienced required.
- BSN required.
- AHA BLS Provider CPR required at hire
- Valid driver’s license and must meet MWHC Driver’s Suitability criteria

Job Summary:The Unit Clinical Care Coordinator (Unit C3) is responsible for overseeing the admission process, patient progression, care coordination, and discharge planning within the nursing unit to ensure operational efficiency. Reporting to the Care Coordination department, the Unit C3 acts as the primary liaison between the nursing unit and various stakeholders, including admission sources (e.g., ED, OR), the hub (bed planning) command center, diagnostic and procedural areas, as well as clinical and ancillary teams. This role ensures that patients are progressing according to the care plan, there is efficient utilization management, treatment plans are executed promptly, and discharge planning is appropriately coordinated. The Unit C3 also leads cross-functional collaboration aimed at improving patient throughput, optimizing length of stay, and driving positive patient outcomes throughout the organization.
Essential Functions and Responsibilities:
- Manages, patient progression, care coordination, and discharge planning to ensure optimal system efficiency and continuity of care.
- Assists unit leadership with unit admission and discharge processes. Conducts admission assessments for designated patient populations to identify patient discharge needs and develops a comprehensive discharge plan addressing those needs, potential barriers, and readmission risks. Collaborates with ancillary services (e.g., PT, OT, Social Work) as needed to ensure a well-rounded and effective plan.
- Leads the clinical care team, including nurses, physicians, social workers and ancillary staff in daily SNAP huddles to align discharge goals. Identifies and addresses any barriers to patient progression and discharge, ensuring timely and coordinated care. Manages documentation of designated patient progression elements in the care coordination software to include the classification, documentation, and resolution of progression and discharge barriers; escalates unresolved barriers to senior leadership via escalation huddle. Ensures timely and ongoing communication of unit capacity status with the Hub to support efficient system throughput and optimal patient flow.
- Partners with virtual Utilization Review (UR) Nurse to align payor and status requirements with patient’s clinical progression; verifies appropriate regulatory letters are delivered to patient and family when indicated.
- Coordinates patient’s plan of care in collaboration with appropriate clinicians for efficient sequencing of care/interventions.
- Communicates progression and discharge plan to the patient and/or their designated proxy in coordination with the patient’s primary care team (RN, physician).
- Oversees and ensures care team’s timely compliance with patient’s preprocedural requirements for service department diagnostics and procedures; ensures communication of service event issues with care team.
- Consults and educates medical and nursing staff on resource utilization, payor requirements, and community resources, while serving as a resource for unit associates on patient progression and care coordination.

Qualifications
- Associate’s degree in nursing, required.
- Valid RN License from Virginia or reciprocal compact state, required.
- Minimum of three (3) years of recent acute care nursing experience, required.

- Communicates and coordinates patient admission and care activities with the Hub to facilitate system-wide planning; ensures timely patient placement, patient progression, and patient flow.
- Proactively identifies patients with repeated ED visits, 30-day readmissions, or unmet social/community needs; partners with physician and ancillary services (i.e., Social Work, Rehab) to determine admission appropriateness; assists in developing an alternative level of care for patients not requiring acute hospital level of care.
- Leads clinical care team (RN, physician, ancillary staff, and social worker) in SNAP huddles (for ED holds) to identify patient progression and discharge barriers; assigns barriers to appropriate team member for resolution; identifies and escalates unresolved barriers to senior leadership; aligns care team toward discharge goals.
- Assesses patient’s discharge needs; develops and arranges for a comprehensive discharge plan that addresses patient needs, barriers, and readmission risk factors.
- Communicates plan of care and discharge plan to patient and/or their family in coordination with the patient’s care team (RN, physician).
- Manages all aspects of admission/patient progression/discharge in hospital’s bed management / discharge planning system (i.e., bed requests, patient attributes/alerts, DRG/TLOS, barriers, escalations, etc.); communicates timely updates of patient clinical status and level of care needs to the Hub to facilitate unit and bed assignment for admitted patients.

Essential Functions & Responsibilities:
- Direct and coordinate nursing care, including oversight of licensed and unlicensed personnel in any assigned or delegated tasks using the nursing process and interdisciplinary teamwork to achieve desired patient outcomes.
- Coordinate involvement of the patient, family and health team members in patient care, including patient/family teaching and discharge planning.
- Communicate and coordinate the plan of care and other pertinent information to/from other health care team members.
- Assist Leadership to define standards of excellence for patient care; participate in improvement of patient care services.
- Participate in, contribute to and integrate performance improvement into care delivery and participate in quality improvement activities.
- Participate in orientation of new permanent and temporary staff members.
- Act as point of contact on hospital and departmental policies and procedures to nurses and other health team members.
- Participate in daily operational activities necessary for safe patient/staff environment.
- Provide input to the Nurse Manager regarding unit needs.
- Participate in unit, departmental and hospital committees for formulation of nursing and hospital policies and procedures.
- Perform other duties as assigned.
Qualifications:
- Graduate of an accredited nursing program and has experience as a Registered Nurse
- Valid RN License from Virginia or reciprocal compact state required.
- BSN is preferred.
- AHA BLS Provider CPR required at hire
- CPI required within 90 days
For those hired into the External Temporary RN Program:
- If job profile requires BLS, ACLS, PALS or NRP within any period of time, it is required at hire for an External Temporary RN.
- If job profile requires CPI, ONS or NIHSS within any period of time, it is required within 30 days for an External Temporary RN
- If job profile requires TNCC or ENPC within any period of time, it is required within 30 days or by the next available course for an External Temporary RN.

Job Summary:Clinical Risk Manager assists with clinical identification, risk evaluation and coordination of corrective action implementation relates to risk issues. Conducts investigations of risk and claim matters and provides investigation information to legal counsel. Interviews staff and physicians involved in potential medical malpractice litigation. Provides intervention and education related to risk management issues to promote safe work practices and quality care and services in an environment that is beneficial to the safety, health and well being of all patients, visitors and hospital staff. Reports real and potential risk situations to the Director, Risk Management. Provides education to providers, staff and administration. Acts as resource person to administrative team, department managers and medical staff.
Essential Functions and Responsibilities:
- Conducts regular risk management surveys of hospital departments and other system owned facilities.
- Reports information gathered from risk identification assessments to the appropriate managers for analysis and corrective action to eliminate or reduce risk.
- Demonstrates knowledge of basic components of risk management, including root case analysis, potentially compensable events, loss control, risk investigation, reporting and claims management.
- Reviews, evaluates and recommends action in risk related matters pertaining to patients visitors, Associates, students, physical plant and personal property.
- Provides education to providers and staff on risk management practices including, but not limited to: risk identification, key elements of risk reduction, and education regarding proper documentation practices that present potential liability problems.
- Performs medical record review for the purpose of identification of real or potential risk and the monitoring of documentation practices.
- Assists in development and preparation of monthly, quarterly and yearly reports on risk incidents, claims and departmental activities.
- Investigates potential liability situations in all MWHC Programs and Facilities identified through various risk identification sources including conducting in-depth interviews of staff involved in general and professional liability matters and provides reports to legal counsel to assist in preparation of defense.
- Completes case summaries on all medical malpractice and general liability cases and provides to legal counsel, executive leadership and Claims Management Committee.
- Coordinates the gathering of medical and other records involved in litigation and assists legal counsel as required.
- Summarize risk management data for appropriate evaluation and risk trend analysis.
- Advises staff, providers and/or committees regarding real and potential liability situations.
- Consults other departments as appropriate to provide collaboration in patient care.
- Interacts professionally with patient/family to increase patient/family satisfaction and/or diffuse potential litigious occurrences related to patient/family perception.
- Maintains claims files and other documents/files associated with general and professional liability matters.
- Demonstrates knowledge of appropriate and current documentation practices related to the risk management.
- Assists in development of standards for the organization.
- Demonstrates knowledge of current methodology and practices. Maintains awareness of changes in the regulations and requirements by accrediting bodies.
- Serves as an on-call resource for staff and physicians on risk issues.
- Participates in risk management and safety activities.
- Ability to analyze complex clinical situations and deal with conflict and people who are upset. Provides a calming influence in difficult situations.

Qualifications:
- Valid RN License from Virginia or reciprocal compact state required.
- Bachelor's degree in Nursing (BSN) from an accredited school required
- Clinical experience within last 5 years preferred.
- CPHRM certification preferred.
- 3 or more years experience in risk management, patient safety or quality improvement in acute care setting
- Thorough understanding of acute and non-acute care delivery systems, including home health care agencies and ambulatory care and psychiatric units.
- Excellent interpersonal skills required.
- Personal computer knowledge including, but not limited to, word processing program, spread sheet programs, electronic calendars, data management programs, graphic presentation programs, Internet communication software and software applications used to support risk management (e.g., flowing charting, etc).
