Program Summary:

Our short-term services typically consist of 12-16 sessions designed to support families on our waiting list. Our treatment philosophy and clinical practice utilizes attachment theory and interventions such as Child-Parent Psychotherapy and Circle of Security Parenting Program. Clinicians also engage in regular reflective practice through supervision and group consultation.

Position Overview:

Provide telehealth psychotherapy to parents and caregivers with an emphasis on supporting caregiver-child relationships.

Participate in individual supervision, group consultation and ongoing learning and development opportunities.

Clinicians are eligible to participate in a post master's training cohort on infant and early childhood mental health

Fairview has an immediate opening for an In Clinic Visit Facilitator to join our team in the Sleep Clinic located in Brooklyn Park, MN. This is a 1.0 FTE (80 hours per two weeks pay period), benefits-eligible opening. The Sleep Clinics at Fairview are education destinations because of our M Health partnership, so we have fellows and students from the MCTC program. This is a multidisciplinary clinic covering all aspects of sleep; Obstructive Sleep Apnea, insomnia, narcolepsy, and more. Visit Facilitators are a valuable member of our care team and collaborate with providers and support staff in meeting patient care needs during and between appointments in the clinic. Visit Facilitators on our team gain experience beyond patient care by working on collaborative projects with other clinics in our service line, leading process improvement initiatives in our clinic, serving as subject matter experts in the clinic, and receive ongoing competency and skills training by our clinical leads. Responsibilities Prepare patients for their appointments including communicating with patients in advance of their scheduled appointment as needed to ensure medical records are collected, received and available for visit if applicable. Assist in the rooming process at the time of the in person visit by collecting and documenting pertinent information such as the reason for their visit, patient reported medication review, vitals collection with automated vitals machine and applicable health questionnaires. Prepare exam rooms and stock supplies Supports the patient connection to the provider for the visit and serves as a resource ensuring a positive patient/provider experience. Completes visit wrap-up ensuring the after-visit summary is provided and patients are aware of recommendations for follow up. This may include assisting with scheduling and coordinating follow-up appointments. Communicates effectively with patients, providers and other health professionals, demonstrating comfort in person and with virtual technologies. Required At least 1-year of customer service experience Proficient with technology (computer hardware and programs, smart devices such as phones or tablets) Preferred Current nursing, medical, social work, psychology, or pharmacy student Successful completion of: NA, EMT, or HUC program Experience with Medical terminology, Epic Health Records, Telephonic customer experience and has previously worked in a health-care setting Benefit Overview Fairview offers a generous benefit package including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
