This position will float between pediatric primary care clinics in the Toledo area. Hours vary by location.
Assists with the provision of quality patient care and performs required administrative tasks.

Primary Function/General Purpose of Position
The social worker assists patients and families to resolve all aspects which may prohibit or limit the patient from receiving maximum benefit from medical care. Evaluates the social-emotional functioning of patient and families, providing counseling when necessary and facilitates the discharge plan. Works cooperatively with other hospital departments, physicians, and outside agencies. Provides assistance to neonates, pediatric, adult, and geriatric patients.

In this role, under the supervision of a clinical manager, you will assess, plan, and implement patient care, educate patients and their families on disease processes, procedures, and medications, and manage various patient needs such as wound care, trachs, Gtubes, Catheters, Vents, and medication administration. We create customized care plans to accommodate our patient's needs, preferences, and concerns, as well as special request of their families.
