Evaluate and treat patients requiring speech-language therapy.
- Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
- Educate and counsel patient and family regarding treatment plans.
- Maintain clinical records and follow performance improvement recommendations.
- Plans and/or assists with patient discharge from speech therapy services. Provides information regarding appropriate support programs.
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Speech-Language Pathologist credentialed from the Wisconsin Hearing and Speech Examining Board obtained prior to hire date or job transfer date required. Common admission diagnoses include sepsis, alcohol withdrawal, COPD exacerbation, pneumonia, and acute CHF exacerbation. Many of our patients have chronic co-morbidities that requires skillful management during their acute illness, including diabetes, CHF, chronic kidney disease, and COPD.

Qualifications
Must-Haves:

Essential Functions:
- Utilize a strengths-based approach to assist clients to get, be, and stay well, in alignment with their values.
- Provide psychotherapy or counseling to individuals, groups, couples, or families, if applicable.
- Interact with clients to assist in gaining insight, defining goals, and planning action to achieve effective personal, social, educational, and vocational development and adjustment.
- Identify psychological, emotional, or behavioral issues and diagnose disorders, using information obtained from interviews, screeners, records, and reference materials.
- Utilize evidence-based intervention methods within scope of practice to meet needs of population. Common approaches at Premise sites might include Cognitive Behavioral Therapy, Acceptance & Commitment Therapy, Dialectical Behavior Therapy, EMDR, Written Exposure Therapy, and CAMS.
- Counsel individuals regarding problems such as stress, substance abuse, and family situations, to modify behavior or to improve personal, social, and vocational adjustment.
- Engage clients in collaborative discussion of treatment goals and desired outcomes.
- Participate as an active team member in the clinical care team. Duties may include consultation, case management, care coordination, and coordination of referrals.

Job Requirements:
- Master’s Degree in social work, counseling, psychology, or equivalent field of study.
- Licensed mental health clinicians including: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor of Mental Health (LPCMH), Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Professional Counselor (LCPC).
- Collaborative Assessment and Management of Suicidality (CAMS – care) certification must be obtained within 12 months of hire.
- 4 years of experience in the field or in a related area.

- Experience in crisis management, abuse reporting, and cultural competence to meet the diverse needs of the population served in a treatment setting.

Summary :
The Licensed Practical Nurse, under the supervision of a registered nurse, advanced practice clinician, or physician provides patient-centered, specialized, evidence-based nursing care across the continuum through an interprofessional approach to treatment, research, education, and advocacy. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and ThedaCare policy. Through collaborative practice with members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Furthers the professional practice of nursing at ThedaCare that involves the promotion of a culture of innovation and a commitment to growth and professional development.
Job Description:
- 5:30 PM to 6:00 AM
- every third weekend requirement (Friday, Saturday, Sunday)

KEY ACCOUNTABILITIES:
1. Performs delegated tasks using evidence-based practice and specific competencies to collect data on the physical condition and nursing needs of patients, and assists with implementing the plan of care.
2. Assists in meeting the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes.
3. Demonstrates clinical expertise in the provision of care in the clinical specialty assigned and performs all functions of the licensed practical nurse that are age appropriate, developmentally sensitive, and culturally specific.
4. Identifies ways to improve the patient's experience of care, streamline care processes, and lower costs while promoting quality to improve patient, family, and team member satisfaction.
5. Contributes to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians.
6. Maintains positive relationships with physicians and other interprofessional team members, collaborating to problem solve and to improve patient care.
7. Demonstrates, anticipates, and proactively manages risk to prevent crises.
8. Performs skillfully in life threatening emergencies, matching demands and resources during crises situations.
QUALIFICATIONS:
• Current Wisconsin LPN Licensure
• American Heart Association Healthcare Provider Basic Life Support (BLS).
PHYSICAL DEMANDS:
• Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance
• Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT:
• Frequent exposure to sharp objects and instruments.
• Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock
• Occasional high noise level in work environment
• Standing and/or walking for extended periods of time
• Transporting patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed
• Possible exposure to communicable diseases, hazardous materials, and pharmacological agents
• Occasional contact with aggressive and or combative patients.
SCHEDULE : Days, 36 hrs/week, 5:30am- 6:00pm, every 3rd weekend rotation and holiday rotation

**Location:** Menasha, WI
**Schedule:** Full-time, Monday-Friday (8 am-4:30 pm)
**Compensation:** Starting at $17.00/hour and up (pending experience)

**Responsibilities**
+ Fabricates and repairs orthotic and/or prosthetic devices.
+ Performs fabrication processes according to company quality and quantity standards.
+ Contacts vendors to order orthotic and prosthetic fabrication materials and maintains inventory of items used during the fabrication process.
+ Assists technicians and trainees as necessary and monitors quality standards of work.
+ Maintains a safe, clean and orderly work environment.
+ Adheres to all safety regulations, compliance with Medicare and other governmental regulations, compliance with Occupational Safety and Health Administration (OSHA) requirements and company guidelines.
+ Analyze reports, charts and correspondence and be able to interpret their meaning.
+ Performs other duties or special projects as assigned.
**Qualifications**
Minimum Qualifications:
+ High School Diploma
Preferred Qualifications:
+ Experience as an orthotic/prosthetic technician
+ Understanding of orthotic/prosthetic devices and O&P device fabrication and maintenance
