Duties and Responsibilities:
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Must be IV proficient, upon hire we will train new staff and expect proficiency at 90 days.

- Bachelor's degree in Social Work, Behavioral Health, Public Health, or related field
- Delaware Certified Alcohol and Drug Counselor (CADC) certification or ability to obtain within the first 9 months of hire
- Knowledge of ASAM criteria and familiarity with substance use and recovery pathways
- Experience with case management, outreach, or recovery support services
- Strong communication and documentation skills, including narrative writing
- Ability to build rapport and trust with individuals from diverse backgrounds and apply cultural humility in all interactions
- Demonstrated understanding of harm reduction strategies (as allowable under the grant)
- Leadership or supervisory experience
- Experience interpreting and applying public health policies and procedures
- Bilingual or multilingual abilities a plus
