Overview:
Provide diagnostic C.T. scans for interpretation by radiologist and or performing physician in the performance of interventional and special procedures (biopsy, ablation, myelography, arthography and any other such procedures). Assist radiologist in the performance of special C.T. procedures and provide the patient care essential all procedures. Is able to be on call if needed. Apply principles of radiation safety. Able to organize the daily schedule and be a dedicated resource to CT team on shift.

Qualifications:
- Education: Successful completion of an accredited Radiography program.
- Experience: 1+ years of relevant experience OR graduate of Bachelor's degree program.

Functions as part of the hemodialysis team in providing safe and effective dialysis therapy for assigned patients under the supervision of the Registered Nurses in accordance with policy and procedure, and training and in compliance with regulations set forth by the corporation, state, and federal agencies.

+ Observe patient, and conduct machine safety checks according to facility policy. Report any change or unusual findings to the HD RN.
+ Report any significant information and/or change in patient condition directly to the HD RN.
+ Monitor patients' response to dialysis therapy and report any unusual findings to HD RN. Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed by the HD RN. Document interventions and patients' response to interventions.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the HD RN.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment
+ Discontinue dialysis treatment according to established procedures
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Oversees disassembly and disinfection of dialysis equipment. Monitors and ensures dialysis machines are properly maintained.
+ Obtain Hemostasis and apply appropriate dressings as required.
+ Evaluate the patient for prior to completing hemodialysis session and report any unusual findings to HD RN.
+ Ensure appropriate safety alarms are reviewed and managed according to policy.
+ Performs surveillance and testing of dialysis water treatment equipment and dialysis water (such as chlorine, hardness, conductivity, water cultures) to ensure that these machines are operating within the parameters set in the policy and procedure. Complies with required logs, testing and cultures to comply with policy and procedure and regulatory standards.
+ Complies with all clinical, infection control and on-site dialysis program policies and procedures.
+ Obtain all prescribed laboratory testing, prepare, and label specimens for collection.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy. Perform all required dialysis machine testing and maintenance according to policy.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent waste and maintain a clean, safe environment of care.
+ Communicates emotional, medical, psychosocial, and nutritional concerns to the registered nurse or HD RN.
+ Displays a "customer service" attitude toward patients, visitors, and other staff members at all times.
+ Performs other duties as assigned.

**Job Summary**
We are seeking a Clinical Dietitian (Full Time) to join our Nutrition Team.
**Key Responsibilities:**
+ Provides medical nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the patient population
+ Follows all guidelines as outlined within the Company Diet Manual and the Clinical Nutrition Policies&Procedures Manual
+ Complies with all regulatory standards to include federal, state, and accrediting agencies while adhering to facility confidentiality, HIPAA regulations, and patient rights policies
+ Trains and mentors patient services staff and interns as applicable
+ Participates in patient satisfaction programs, departmental meetings, and facility wide Quality Assurance/Performance Improvement programs
**Qualifications** :
+ Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)
+ Licensed by the State Dietetics Licensing/Credentialing Board, in states where required
+ One (1) year of hospital experience, preferred
+ Possess the necessary skills to effectively utilize Microsoft office applications, electronic medical record&diet office systems, and nutrient analysis programming

Works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
**Primary Responsibilities:**
+ Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
+ Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
+ Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
+ Completes follow-up and post-discharge assessments according to documented standard operating procedure
+ Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
+ Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
+ Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
+ Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
+ Provides counsel, support and education to staff as appropriate
+ Actively participates in ongoing meetings pertaining to patient care and clinical excellence
+ Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
+ On-call for caseload during business hours. Participation in clinical after hours on-call program on rotational basis
+ Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
+ Perform related duties as necessary and other duties as assigned

**Required Qualifications:**
+ Active, unencumbered license to practice nursing in the assigned state
+ Nurse Practitioner Certification from either the ANCC, AANP or AACN as Adult, Geriatric, Family or Acute Care NP
+ 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse
+ Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
+ Ability to obtain DEA licensure/prescriptive authority post-hire in states where applicable
+ Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco users
+ Ability to lift a 30 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations
**Preferred Qualifications:**
+ Experience in Gerontology, Cardiology, Internal Medicine or Endocrinology
+ Home care or home visit experience
+ Excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
+ Computer literate and able to navigate the internet
Compensation for this specialty generally ranges from $104 500 to 156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. ***PLEASE NOTE*** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer.
