Summary of Job Duties:

We are seeking a dedicated Nurse Patient Educator to join our team at the Pine Bluff Family Medicine Clinic. This role focuses on promoting screenings, early identification, and clinical navigation for the health and development of children from prenatal through five years of age. The Nurse Patient Educator will be responsible for conducting educational sessions with families to provide information on child health and development. They will also coordinate with healthcare providers to facilitate appropriate screenings and follow-up care. Additionally, the role involves developing educational materials and resources to support families in understanding and managing their children's health needs. Minimum Qualifications:

* Associate's Degree in Nursing plus seven (7) years of experience as an educator/instructor or RN OR BSN in Nursing plus five (5) years of experience as an educator/instructor or RN with at least one (1) year of experience in a clinical environment.Valid Arkansas Registered Nursing license.
* Current BLS certification OR must successfully obtain certification within the first thirty (30) days of employment.

Nurse Case Management & Home Visiting:

* Conduct home visits and clinical assessments as needed to support high-risk clients.
* Identify medical and psychosocial needs and develop client-centered care plans.
* Provide health education and guidance tailored to each family's needs and health literacy.
* Support CHWs in care coordination and referral follow-up to ensure continuity of care.
* Assist families with navigating healthcare systems and accessing appropriate services (e.g., Medicaid, WIC, behavioral health).

Mobile Health Clinic Obligations:

* Prepares patients for clinic visits by obtaining vitals and entering data into the EHR; accompanying patient to assigned exam room; obtaining patient HPI; updating active problems and medication lists, and updating allergies and past surgical and medical history.
* Communicates brief summary of reason for patient's visit to provider.
* Gathers and places supplies and instruments appropriate for anticipated exams and/or procedure, based on stated reason for appointment or information gleaned during the intake.
* Assists the provider with patient care.
* Administers medications and vaccines per protocol or as ordered by provider, performs point of care testing.
* Reinforces patient education and instructions at conclusion of visit.
* Assists patient in scheduling follow-up appointments.
* Assists patients with scheduling appointments with outside providers when outgoing referral is ordered.
* Exhibits competency with professional nursing skills and EHR documentation.
* Exhibits professionalism by maintaining a positive working relationship with patients, providers and employees.
* Assists with cleaning, stocking, preparation, and driving of mobile health units as need

Education & Training:

* Provide ongoing training and mentorship to CHWs on maternal and child health topics, including prenatal care, postpartum recovery, safe sleep, breastfeeding, infant development, and family planning.
* Develop culturally responsive educational materials for families and CHWs.
* Facilitate group education sessions, workshops, or classes as needed.
* Stay up to date on clinical guidelines and public health trends relevant to maternal and infant health.

Quality Improvement & Compliance:

* Participate in data collection, case reviews, and program evaluation efforts.
* Collaborate with the team to identify service gaps and develop strategies to address them.
* Ensure documentation meets HIPAA and programmatic standards.
* Contribute to continuous quality improvement (CQI) projects and audits.

The DCE is responsible to the Program Director (PD) for the integration of clinical education with didactic education. The DCE is responsible for preparing students for clinical experience. This includes but is not limited to supervision of student-related clinical instructor activities; scheduling and maintenance of records pertaining to student clinical assignments; and maintain student files relative to completion of clinical requirements. This position is governed by state and federal laws and agency/institution policy. Functions: Management of the program's clinical activities include organization, development, and administration of the clinical curriculum; planning for, acquisition of, and communication with, locations needed for development of evolving practice skills; Maintains clinical affiliation agreements and/or Memorandums of Understanding (MOUs); Secures clinical sites and scheduling/coordination of student clinical practicums and lab hours; The DCE must have frequent, regular, and consistent contact with students, clinical faculty, and clinical affiliates at all program locations; The DCE must be available and accessible to students and clinical faculty when program students are engaged in clinical coursework; Ensure that appropriate supervision/assessment of students is available at all clinical sites; Ongoing assessment of the overall effectiveness of the clinical training for all students; The DCE must work with the PD to ensure that student clinical exposures are coordinated with their didactic and laboratory education. Develop and implement processes that reduce inconsistency among individuals who perform clinical evaluations; Prepares and submits reports and statistics to the program director, administration, and appropriate outside agencies; Acquisition and maintenance of the clinical laboratory equipment and supplies; Attend local, state, and national respiratory educational conferences and meetings to earn Continuing Respiratory Care Education (CRCE)) for credential and licensure maintenance; Attend respiratory program meetings, divisional meetings, professional development meetings, respiratory program pinning and graduation ceremonies; Serve on college-wide committee(s); Performs other duties as assigned. Knowledge, Abilities, and Skills: The DCE must have knowledge of the respiratory field and the clinical application of the classroom knowledge. The DCE must have problem solving skills with critical thinking a major component of his/her ability. Organization and time management skills are required. Computer applications skills. Certification and Instructor credentials in BLS, ACLS, PALS and NRP. This description incorporates full time, part time and PRN PSS roles.JOB DESCRIPTION:Gathers pre-admission information from hospital, patient or family member on scheduled patients before initial and/or follow-up visit.Obtains financial updates, documents in the billing system, completes forms and obtains signatures as "alerts" request. Ensures patient documents are completely filled out and signed.Obtains, verifies, and updates patient information and provides support services to patients and medical staff.Know how to locate path reports, outside medical records, radiology reports and retrieve if necessary for the position.Refer patients to the financial counselors when alerted by billing system or if the patient has billing questions.Contacts the social worker for assistance with patient needs such as lodging, transportation, medication request, or any type of social worker request.Forwards any medical records requests to centralized medical records department.Schedule referring and future physician appoints as requested by physician. This includes sending the required records for the referring appointments.Responsible for ensuring patient's initial appointments/schedules are up to date and in the computer.
