The **Per Diem Patient Access Representative** is responsible for greeting and welcoming patients in person. Provides a standard registration process by collecting demographic and financial data and enter information into a electronic medical record.
**Location:** 1324 5 th St N, New Ulm, MN 56073
**Primary Responsibilities:**
+ Greets and welcomes patient in person
+ Collects demographic and insurance information
+ Checks in and interviews patients to complete appropriate paperwork
+ Directs patient to their appointment or procedure
+ May assist in scheduling add-on appointments
+ Documents any issues and resolutions in electronic medical record
+ Resolve claim issues or registration errors on patient accounts
+ Uses resources, tools and procedures to complete registration for accounts and in assigned work queues
+ Verifies insurance eligibility and benefits information for payers and interprets results focusing on complex billing situations
+ Obtains cost information and explains information to patient if necessary
+ Collects co-pay or deductibles and respond to questions regarding financial assistance programs available. Refers complex questions as needed
+ Screening and approving patients for financial assistance programs
+ Assists and counsel's patient with application process for available financial assistance programs
+ Provides technical or functional direction for employees
+ Assist patient in completion of request for information forms and submits to appropriate department
+ Obtains signatures from patients as needed
+ May gather medical record information from all departments for collection
+ Provide customer service for inbound and outbound telephone calls
+ May mentor other staff as needed
+ Other duties as assigned

**Required Qualifications:**
+ 6+ months of customer service experience
+ Intermediate level of proficiency with Microsoft Office products
+ Ability to work standard daytime hours, Monday - Friday for the first 3 weeks after hire in alignment with the training schedule
+ Ability to pick up 1 st , 2 nd or 3 rd shift hours (generally 1-3 shifts a month)
+ Ability to work every third weekend overnight shift (Friday and Saturday 11:00pm to 7:00am)
+ Must be 18 years of age OR older
**Preferred Qualifications:**
+ Revenue Cycle experience
+ General office experience
+ Epic experience
**Physical Demands:**
+ Consistent walking, standing, bending, turning, etc.
+ Lifting weight up to 10 lbs. occasionally, up to 2-5 lbs. frequently

**CARE Counseling** , part of the Optum family of businesses is seeking a **Board Certified Social Worker Supervisor** to join our team in Mankato. As a member of the Optum Behavioral Care Team, you'll be an integral part of our vision to make healthcare better for everyone.
This position provides an array of services using evidence-based practices that support client treatment, and facilitate safe, secure, and therapeutic interventions. Responsibilities include developing therapeutic alliance, clinical skills application, conducting diagnostic and comprehensive assessments, individualized treatment planning and daily interventions.
This position offers the flexibility to work onsite or a hybrid arrangement that requires at least two on-site days per week once a consistent caseload has been established.
**Primary Responsibilities:**
+ Manage and support the supervisee re: diagnosing, treatment planning, conducting therapy, care coordination, client termination, and all other aspects of the Mental Health Practitioner position
+ Maintain caseload of 16 clients and between 6 - 8 supervisees
+ Consistently review and provide timely feedback (e.g., within three business days) on all supervisee documentation, including diagnostic assessments, treatment plans, progress notes, and discharge summaries. Provide constructive feedback and guidance to supervisee regarding documentation skills
+ Attend required meetings for supervisors (Supervision of Supervision, case consult, weekly supervision sessions with supervisees). Facilitate weekly staff case consult in a leadership role, as needed
+ Maintain a record of all supervision sessions and submit to admin staff monthly
+ Effectively manage schedule to allow for availability to support supervisee learning and development
+ Represent CARE positively and model professional behavior as a representative of CARE with internal and external professionals
+ Develop a Supervision Plan and Learning Contract with each supervisee
+ Support supervisee clinical and professional growth through strengths -based, reflective supervision and case consultation
+ May be required to lead, plan, and/or participate in Clinical Trainee Training Day and associated events
+ Arrange for supervisor coverage in the event of a planned or unexpected absence. Additionally, be available to provide coverage for other supervisors as needed

**Required Qualifications:**

+ Licensed Supervisor - Board approval to supervise Mental Health Practitioners and Clinical Trainees
+ 2+ years of experience in a Licensed Mental Health Professional role
+ Basic understanding of Microsoft products (Word, Excel, PowerPoint, etc.)
+ Ability to adhere to one of the following work models:
+ Onsite (must complete all work at one consistent site)
+ Hybrid (at least 2 days on-site once a consistent caseload has been established). Must have a distraction-free space in home for home office with access to high-speed internet.
