**Responsibilities:**

Negotiates surgical/procedure schedule by communicating complex details directly to the Operating Room scheduling office and Ambulatory Surgical Center Staff. Authorizes overbooking and rearranging of surgeon's Operating Room schedules based on case complexity and medical urgency. Authorizes rescheduling when cancellations occur by applying protocols for surgical scheduling. Orders all necessary surgical/procedure equipment, devices, and medication based on the application of protocols needed to conduct the surgery/procedure. Follows-up to confirm the order is fulfilled and ready for surgery/procedure. Employs multiple contingency strategies to ensure all details, materials, and locations are ready for surgery/procedures as scheduled. Ensures patient education and follow-up details vary according to pre-operative readiness and post-operative rehabilitation expectations and activities. Accountable for completeness of all relevant information from pre-op testing and evaluations for review by the anesthesiologist and the Operating Room.
+ Assembles the necessary details from multiple tabs within the electronic medical record for review and inclusion when completing disability paperwork. Assesses calls to understand patient needs, prioritizes medical concerns, and obtains responses from providers and follows up with patients. Serves as main point of contact for patients pre and post-operative.

**MINIMUM EDUCATION & EXPERIENCE**
+ High School diploma and 2 years of relevant experience required

**Responsibilities:**
GENERAL PURPOSE:
The Physician Support Specialist provides support to the Heart Failure Department. Responsible for using independent judgement and initiative, performs routine and non-routine clerical duties and support of onsite and regional activities of a team of physicians, APP's and RN's. Performs functions associated with patient information processing for ambulatory care visits. Completes the task of appointment scheduling, eRecord task management, In Basket management and Telephone Encounter management using the electronic medical record and patient access. Responsible for functions being completed in an accurate, efficient, and customer friendly manner. Requires excellent organizational skills, working knowledge of privacy and other policies and procedures in daily contact with confidential information and material in accordance of HIPPA regulations. Candidate must be able to work independently as well as collaboratively within immediate team to foster inclusive environment focused on continuous improvement.
**ESSENTIAL FUNCTIONS**
**Telephone Management:** Assess and triage incoming calls in a timely and courteous manner. Gather pertinent medical information at the time of call from patient and/or other health care professionals. Evaluate urgency of response and communicate messages using established protocols to allow collection of accurate and complete medical information, while maintaining patient confidentiality issues.
**Scheduling Management:** Identify appropriate scheduling protocol for Advance Heart Failure patients, establishing a sequence of appointments, ancillary tests such as lab work, cardiology procedures, and coordination of multidisciplinary resources (cardiology, nursing, dietitians, finance resources, echocardiology, social workers and /or surgeons.) Facilitate hospital admissions, scheduling of procedures, coordinate overnight accommodation and transportation arrangements.
**Data Management:** Maintain responsibility for own Electronic Medical Record (EMR) in basket as well as the in baskets for any pools assigned. Possesses the knowledge, skills, and abilities to support the Medical Records department as needed. Entry and management of data in specialized database. Documentation of patient care to ensure full compliance with regulatory agencies. Ensure databases reflect current status, demographics and on-going evaluation actions. Document each patient encounter (phone call, ancillary test/lab results, inpatient or outpatient clinical evaluation, medications, infection management) to maximize patient care and ensure full compliance with regulatory agencies.
Contributes to operations including supply maintenance. Solves operational problems and provides direction to coworkers. Cross-coverage in shared clerical office; on call schedule, & special projects.

**Responsibilities:**
Responsible for the administrative, clinical and professional direction of a clinical program in Is responsible for the administrative, clinical and professional direction of a clinical team in Psychiatry offering Forensic Assertive Community Treatment for a cohort of patients with serious mental illness, often comorbid substance use, and involvement with the legal system, who are not engaging in traditional outpatient clinical care. Oversees all administrative and clinical matters on the ACT Team. Manages services and program development; ensures that applicable regulations are operationalized within the program. Responsible for the ongoing performance improvement initiatives on the team. Represents the program to the community, fostering referrals, and assumes teaching and direct clinical care responsibilities. The team provides most of its interventions in the community, wherever the patient is located and provides 24/7 coverage for acute patient needs. ACT services are provided to patients who cannot engage with traditional mental health clinics; thus, the ACT Team Clinical Coordinator engages with multiple government agencies such as the jail, mental health court, homeless shelter providers, etc. to facilitate meeting regulatory requirements and high-quality clinical care. The Clinical Coordinator oversees the daily clinical operations of the interdisciplinary team including making assignments, triaging cases, and prioritizing staff activities to meet acute patient needs. The Clinical Coordinator strives to maintain regulatory required staffing levels through recruitment, onboarding and retention strategies for this team who interact with the most challenging and vulnerable patient population in Behavioral Health clinical services. The Clinical Coordinator is also an integral member of the ACT team, providing comprehensive integrated mental health and substance abuse treatment to enrolled clients and provides administrative and clinical supervision of an interdisciplinary group of clinical staff including multidisciplinary mental health therapists, CASAC's, Rehabilitation Therapists, Case Managers, and other team members.
RESPONSIBILITIES:
Administrative:
Ensures that the ACT team adheres to all aspects of the OMH regulatory requirements for the ACT team in terms of service delivery, documentation that the team completes OMH reporting in a timely and comprehensive manner (i.e., CAIRS, AOT, etc.), billing and data reporting.
Work with Strong Ties leadership and Dept of Psychiatry Utilization Review team to address documentations concerns in a timely way and problem-solving reduction in risk of noncompliance going forward.
Prepares for and participates in regulatory surveys for ACT with the Strong Ties Leadership team and engages in the development, monitoring and evaluation of Performance Improvement Plans required as part of the survey process.
Approval of purchases for patients directly related to patient care as WRAP funding. Ensure appropriate process for procurement card acquisition, purchases, completion of SOE┐s, reconciliation, and doumentation of WRAP utilized per patient.
Facilitates daily team meetings to assign cases to team members and reviews days┐ work responsibilities
Accepts referrals for ACT and completes necessary collaboration with community providers in the assessment and determination for enrollment in ACT. Works with the team to complete the enrollment process as indicated
Supervisory:
Is responsible for the hiring, onboarding, and supporting new clinicians and ACT team staff.
Provides weekly clinical supervision to all direct care staff on the team per OMH guidelines.
Works with Strong Ties leadership and HRBP regarding any staff disciplinary issues.
Provides feedback to the treatment team regarding patient progress in recovery and facilitates and supervises the placement of patient in other community treatment programs as necessary.
Clinical:
Serves as liaison between the ACT program and other community agencies that provide chemical dependence or MICA treatment programs for ACT clients who choose additional or inpatient chemical dependence treatment services.
Serves as liaison between the ACT program and the courts, probation, and parole.
Transports clients as needed to appointments and assists clients in moving or shopping.
Participates in a client on-call phone rotation with other ACT team members once every 5-8 weeks.
The Team Leader also provides back-up (when not directly on call) to any clinical staff on the team that is on call as needed in the event of patient acuity.
Works with clients individually and in group format to help initiate and maintain recovery from substances of abuse and other addictive behaviors.
Engages clients in both individual and group psychoeducational to minimize the incidence of relapse using recovery-based principles, collaboration and shared decision making.
Provides cognitive behavioral therapy for psychosis including education about stress-vulnerability and symptom management.
Monroe County designee to complete 9.45 Mental Hygiene Transport of patients to psychiatric facilities.
Staff Development:
Subscribes to creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
Participates in available cultural competency events and training appropriate to job duties.
Participates in OMH led Quality Improvement initiatives specific to ACT.
QUALIFICATIONS:
Masters prepared clinician of any qualified health professional discipline with NYS Licensure or Certification required.
Five years of related clinical, supervisory, and administrative experience, previous experience working in the mental health field with patient experiencing severe mental illness required.
Or equivalent combination of education and experience required.
Previous supervisory experience required.
Must possess a valid NYS driver┐s license in good standing required.

Statement of Purpose:
The Director of Acute Care Services manages and coordinates the planning, development, implementation, evaluation, and supervision of the nursing regimen and other activities involved in the provision of quality patient care on the Medical/Surgical and Intensive Care Units, as well as Respiratory Therapy. The Director of Acute Care Services provides leadership for these areas by directing and working collaboratively with the Clinical Leads of Medical/Surgical and ICU units, nursing staff, ancillary staff, physicians, and other personnel. The Director maintains standards for professional nursing practice in the clinical setting at all times. Provides both clinical and administrative leadership to staff at all times.
MAJOR TASKS, DUTIES AND RESPONSIBILITIES
Assessment:
+ Assesses the quality of the process and delivery of care, on an ongoing basis through observation and communication with staff, patients, families and physicians. Assures patient-centered care is delivered in a safe, cost-effective, evidence-based manner. Assures excellence in patient care delivery.
+ Assures nursing care is assigned to staff members based on qualifications, experience and the needs of the patient.
+ Communicates and interprets changes in policy, procedure and methods to staff and provides feedback from staff to Nursing Administration through staff meetings, group conferences, or individual conferences.
+ Acts as a liaison with other departments to promote cooperative relationships in promoting a seamless continuum of care. Confers with all members of the health team to plan for the patient's total care; acts as a resource person for patient care related issues.
+ Makes patient rounds independently or with other health care providers.
+ Interviews, recommends hire, and assists in the orientation of new staff members. Oversees the orientation process and assures required documentation is complete.

Planning:
+ Maintains and promotes practice within the established mission and philosophy of JMH and the policies and standards of the Department of Nursing.

**Responsibilities:**
GENERAL PURPOSE
Responds to potential organ donor referrals at area hospitals, evaluates potential organ donors, assists with the recovery of organs for transplant in the Operating Room and is responsible for labeling and packaging organs according to OPTN policy. Takes initial referral information and taking organ offers from other OPO's for patients listed on the local transplant lists. May travel on small aircraft with surgical teams to assist with the coordination and recovery of organs for local patients.
**ESSENTIAL FUNCTIONS**
+ Prepares the Operating Room staff for the recovery procedure, facilitates the arrival of the visiting surgeons and obtains temporary privileges, supplies and equipment needed for surgery.
+ Monitors surgical recovery in the operating room.
+ Coordinates and communicates with transplant surgeons on all imported organs and arranges transportation for incoming organs.
+ Participates in the recovery of local imported accepted organs as needed, including flying out with the transplant team and coordinating and working with surgeons and other OPO's when preservation machines are utilized.
+ Answers calls to screen and respond to organ donor referrals.
+ Travels to donor hospitals to assess and evaluate potential organ donors.
+ Determines medical suitability for organ and/or tissue donation through laboratory studies, medical record review and physical assessment.
+ Attends clinical meetings and joint staff meetings as scheduled.
+ Completes special projects and general administration.
+ Completes authorization forms, obtains a Donor Risk Assessment Interview (DRAI) once willingness to donate is identified and sends off blood for testing.
+ Coordinates and communicates with all ancillary services related to recovery activity, such as medical examiner/coroners and/or pathologists as appropriate.
+ Completes all FLDRN paperwork and all information in iTransplant associated with local donor referrals, organ donors, organ import offers and imported organs accurately and in a timely manner.
+ Resolves documentation issues as identified by quality assurance deportment for post donor follow up.
