**Hospice Aide**
**What You'll Do** :
The Hospice Aide provides compassionate personal care and homemaker services for patients when appropriate and maintains a safe and healthy environment as directed by the IDT plan of care.
**More Specifically** :
+ Assist patients with appropriate techniques to maintain personal hygiene and grooming, including shower, tub or bed baths and mouth, skin, nail and hair care.
+ Assist patients in the use of toilet facilities, including bed pans.
+ Provide skin care with observation for pressure areas and sores and knowledge of preventive measures.
+ Assist patients in and out of bed using safe transfer techniques, including the use of gait belts and mechanical lifting equipment, when applicable (with appropriate completed/demonstrated skills competency).
+ Assist patients with walking, including the use of walkers, canes and wheelchairs, when applicable.
+ Assist with cuing of self-administered medications.
+ Assist with meal preparation and feeding, when required.
+ Observe, report, and document patient status and care provided.
+ Take and record of vital signs (temperature, pulse, and respirations) as directed (with appropriate completed/demonstrated skills competency).
+ Practice basic infection control procedures.
+ Document status of patient functioning and changes in patient status to be reported to the hospice nurse case manager.
+ Maintain a clean, safe and healthy environment in the patient's home.
+ Recognize emergencies and contacting emergency personnel and supervisor when necessary.
+ Understand the physical, developmental and emotional needs of the populations served.
+ Treat the patient and his/her property with respect.

Responsible for evaluation and treatment of communication and swallowing disorders affecting speech, voice, language and swallowing abilities in adults and pediatric patients. In the acute care setting, provide evaluations and recommendations for patient's candidacy for PO diet or need for non-oral nutritional support as well as assisting in speech and language screenings and recommendations for discharge needs. In the out patient setting, carry a varied caseload including: pediatric speech and language disorders, voice disorders, professional voice care, augmentative communication evaluations, alaryngeal speech training, dysphagia, aphasia and dysarthria.

**Essential Duties & Responsibilities including but not limited to:**
+ Maintains comprehensive records regarding patient care in reports and correspondence both within the clinic and with outside healthcare providers. Maintains daily statistics and submits billing in timely order in compliance with department protocol.
+ Provides comprehensive diagnostic and treatment for speech, voice and swallowing disorders.
+ Participates in research projects coordinated by the Speech Pathologists in conjunction with the Medical Director of Otolaryngology.
+ Attends and participates in continuing education within Lahey Clinic as well as outside conferences.
+ Complies with Speech, Voice and Swallowing infection control measures and safety program; and maintains a safe and neat working environment.
+ Maintains courteous and effective interactions with colleagues and patients.
+ Demonstrates an understanding of the job description, performance expectations, and competency assessment.
+ Participates in departmental and/or interdepartmental quality improvement activities.
+ Participates in and successfully completes Mandatory Education.
+ Performs all other duties as needed or directed to meet the needs of the department.
**Minimum Qualifications:**
+ Education: Knowledge of Speech Language Pathology typically acquired by attaining a Master's degree in this field.
+ Licensure, Certification, Registration: Licensure by state of Massachusetts; certification by American Speech-Language Hearing Association.

APG patient personal health information is only shared with individuals who are actively managing the patient's care. NO other parties are entitled to receive, discuss a patient's personal health information without the patient's permission. The Audiology Assistant will ensure patient safety practices to prevent/eliminate risk of patient harm. Adverse patient events and near misses will be promptly reported to the APG Risk Management Dept.

We are currently seeking a dynamic and compassionate team member to join our growing Urgent Care practice located in the vibrant Chestnut Hill Square Plaza. Our clinic is in a prime street-level storefront location at 200 Chestnut Hill Square, conveniently situated near SoulCycle.
The complex offers a range of great amenities, including Wegmans, popular eateries, and free parking-both in a covered garage and open lot.
Our modern, well-equipped clinic features:
- Modest-sized exam rooms for personalized care
- On-site full imaging and laboratory services
- Comprehensive care for patients ages 2 through geriatrics
We are open 7 days a week from 9:00 AM to 7:00 PM, and closed on major holidays.

**Job Description:**
**Essential Responsibilities:**
+ Communicates with and assists patients, other employees and the general public in a courteous, helpful manner by phone and in person. Provides accurate information, directions and/or guidance and follows up promptly to ensure that needs have been met.
+ Takes complete and accurate information from patients/callers, prioritizes information and facilitates communication of information to providers.
+ Prepares schedules, patient charts and other documents as needed in accordance with quality standards. Maintains patient confidentiality at all times, whether by phone, discussion with providers or distribution of patient information by e-mail, fax, etc.
+ Performs check-in, registration, and verification of demographic and fiscal information utilizing a computer system.
+ Enters data from visit tickets and ensures that information is complete, accurate and timely to facilitate the billing process.
+ Collects copayments following standards for managing copayment processes. Directs patients at time of check in.
+ Schedules related diagnostic and lab tests making every reasonable effort to accommodate patient and provider needs. Inputs patient insurance and managed care plans, including prior authorizations, referrals, and pre-certifications. Explains processes as needed to help patients meet their needs and insurance requirements.
+ Verifies patient schedule to assure patient was seen and enters billing tickets into system.
**Required Qualifications:**
+ High School diploma or GED required.
+ Basic familiarity with computers. Ability to navigate at a basic level within web-based applications.
+ MA, EMT, CNA, LNA certificate or diploma; or at least 5 months of direct patient care experience in a clinical or research setting, such as: experience working as a medical assistant (MA), a patient care technician (PCT) or a clinical nursing assistant (CNA); nursing or medical student with completion of one clinical rotation.
**Preferred Qualifications:**
+ Phlebotomy certificate

**Written Communications:** Ability to communicate clearly and effectively in written English with internal and external customers.
**Oral Communications:** Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.
**Knowledge:** Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
**Team Work:** Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.
**Customer Service:** Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
**Age based Competencies:**
Employees in this job must be competent to provide patient care to the following age groups: Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.
**Physical Nature of the Job:**
Heavy work: Exerting up to 100 pounds of force occasionally and/or 50 pounds of force frequently. Work is physically demanding and requires extended periods of standing and/or moving.
