Schedule - Monday to Friday Flexible schedule  Floating between Various Imaging and  Speciality Practice in Nassau and some parts of Suffolk County 



Primary Responsibilities:



  • Prepares and positions patients for diagnostic imaging procedures

  • Explains procedures and process to patients

  • Moves imaging equipment into the specified position; determines exposure factors on the basis of height, weight, the portion of the body involved, and the degree of penetration required; and adjusts equipment controls to set exposure factors and produce images of proper detail, density, and accuracy

  • Practices radiation protection techniques to minimize radiation to patients and staff

  • Performs specialized and routine diagnostic radiology procedures

  • Processes images and reviews them for proper identification and quality control

  • Prepares images for reading by the radiologist or requesting physician

  • Performs routine maintenance, diagnoses malfunctions, and makes minor repairs on radiologic equipment, arranging for repairs as needed

Required Qualifications:



  • High school diploma / GED

  • License or temporary permit to practice Diagnostic Radiology in the state of New York

  • American Registry of Radiologic Technologists (ARRT) certification or willing to obtain in 180 days



Preferred Qualifications:



  • 1+ years of experience working as a Radiologic Technologist

  • Experience with Epic EMR software

  • Orthopedic Exp.  



New York Residents Only: The hourly range for this role is $23.22 to $45.43 per hour. What you will do.

The Care Specialist works in patient’s homes and community 90% of the time and virtually the other 10% to deliver chronic care management to high complexity patients. During initial outreach the Care Specialist informs patients about our services and helps them get enrolled.  Reaching out via phone is our top strategy for outreach and it’s important that the Care Specialist is comfortable and confident communicating by phone.  Field-based approaches are utilized as well, and the Care Specialist should be prepared to use whatever strategy is most effective.  Once the patient is enrolled, the Care Specialist will facilitate virtual visits from the patient’s home to our providers remotely to support our integrated care delivery model, focused on the following goals: Promote timely access to appropriate care; increase utilization of preventative care; reduce emergency room utilization and hospital readmissions; create and promote adherence to a care plan developed in coordination with the patient, primary care provider, and family/caregiver(s); increase patient’s ability for self-management and shared decision-making; provide medication reconciliation; connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs

KEY RESPONSIBILITIES:

  • Conduct direct outreach to patients via phone, in-person, mailings, and other strategies including cold calling and unscheduled door knocks. 
  • Enroll patients into Upward Health’s program and collect key data about patients during the enrollment process through continuous and on-going phone and in-person interactions. 
  • Meets patients in their home or in the community to conduct a needs assessment, including helping patients to set health goals.
  • Facilitate in-home provider appointments and coordinate care between patients and care team as needed. Ensuring that all of your assigned patients have an initial provider visit and follow up visits scheduled.
  • Support your patients in meeting their healthcare goals as it relates to Quality measures.
  • Work within an interdisciplinary team to support the team’s effort in meeting market and/or organizational goals. 
  • Obtain and record vital signs and other health information in electronic medical record (EMR)
  • Analyze patient data to determine patient needs or treatment goals.
  • Assess physical conditions of patients to aid in diagnosis, treatment; and/or need for additional referrals in support of health and social needs.
  • Explain technical medical information to educate the patients.
  • Cultivate and support the primary care providers with timely communication, inquiry follow-up, and integration of information into the care plan regarding transitions-in-care and referral.
  • Builds rapport with Upward Health patients utilizing motivational interviewing techniques.
  • Conduct one-on-one extended in-person patient appointments.
  • Makes follow-up calls and home visits to patients per Upward Health policy.
  • Documents each patient encounter with accuracy and precision.
  • Prepares reports and documents as needed or requested.
  • Attends regular daily huddle, team meetings and participates in clinical rounds.
  • Other duties as needed. 

KNOWLEDGE, SKILLS & ABILITIES:

  • Strong critical thinking skills for assessing patient needs and treatment goals.
  • Self-starter with the ability to work independently in an unstructured environment.
  • Interpersonal savvy, demonstrated by the ability to interact with and influence people to establish trust and build strong relationships.
  • Familiar with concepts like Motivational interviewing, trauma informed care and care coordination.
  • Ability to complete unscheduled home visits, completed cold-calls and outreach.
  • Strong organization skills and ability to manage and maintain a personal schedule.
  • Proficient in time management and the ability to prioritize tasks effectively.

QUALIFICATIONS:

  • At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role
  • High school graduate or GED required.
  • A valid driver’s license and auto liability insurance.
  • Reliable transportation and the ability to travel within your assigned territory or other as needed to support the patient needs plan and training requirements.
  • Experience in Chronic Care Management model OR experience with chronically ill/elderly patients.
  • Long-time resident of the community with good knowledge of the resources of this community.
  • Ability to complete Upward Health’s initial training program and ongoing educational requirements as assigned, both virtually and in-person. 
  • Technologically savvy with basic computer skills, including ability to type.
  • Multi-lingual capabilities preferred, but not required.
  • Prior Home Care experience a plus

  • Performs medical and nursing treatments in accordance with plan of care, provider orders, and NYSBON scope of practice.
  • Perform procedures, e.g.: hot & cold applications, placement/ removal of orthopedic appliances, casts, splints, suture and staple removal, urinary bladder irrigation w/ and w/o chemotherapy, insertion/removal/catheter care of urinary catheter, ear irrigation.
  • Applies and removes dressings, and performs wound care Assists with Minor and Major Procedures
  • Performs patient monitoring including pre, intra, post procedure, testing, medication administration, medical/surgical treatment and/or sedation
  • Provides medication administration: (IM, PO, SQ, ID, IV topical, inhalation, ocular, vaginal, rectal) as per provider orders or protocols to patients in practice, following nursing rights of medication administration and organizational policies. This includes oxygen therapy.
  • Facilitates medication refills by reviewing clinical information and preparing order for provider (prospective) authorization in EMR.
  • Administers IV therapy: including blood, blood products, IV fluids, chemotherapy (with proper certification), IV medications via peripheral or central venous devices.
  • Starts and discontinues peripheral IV lines.
  • Maintains peripheral and central IV lines and manages complications with appropriate certification

Communication and Message Management
  • Ensures clinical in-baskets are managed and encounters closed in a timely manner.
  • Receives, documents, and reports critical lab values as indicated
  • Relays normal and abnormal clinical results, follow up, and explanation to patients, as directed by provider.
  • Performs preliminary review of results for prioritization of urgent findings, then notifies provider for final verification.
  • Performs in person and phone triage for patient clinical concerns with appropriate escalation; provides clinical advice as within RN scope and/or direction from provider.
  • Performs Care/Case management of a defined population via multi-disciplinary plan to coordinate/facilitate patient care as needed.

Health Promotion & Education
  • Identifies, through case-finding and critical analysis, patient, family and/or community deviations from health and wellbeing; advocates for, initiates and/or recommends appropriate actions including the establishment of safe environments.
  • Collects patient healthcare data systematically and continuously, using appropriate assessment techniques and instruments.
  • Analyzes assessment data to determine health status and nursing diagnoses
  • Reports deviations requiring medical diagnosis and/or treatment to provider.
  • Escalates urgent/emergent deviations to medical provider and/or emergency care.
  • Identifies expected outcomes and anticipates unexpected or adverse outcomes as individualized to the patient.
  • Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.

• Performs onsite testing, e.g.: POC lab testing, EKG, specimen collection (including phlebotomy), auditory & visual test, psychiatric & physical risk screening, fetal monitoring (ante partum) and non-stress test, spirometry, allergy skin testing, bladder, tb skin testing and result evaluation.
• Performs medical and nursing treatments in accordance with plan of care, provider orders, and NYSBON scope of practice.
• Perform procedures, e.g.: hot & cold applications, placement/ removal of orthopedic appliances, casts, splints, suture and staple removal, urinary bladder irrigation w/ and w/o chemotherapy, insertion/removal/catheter care of urinary catheter, ear irrigation.
• Applies and removes dressings, and performs wound care
• Assists with Minor and Major Procedures
• Performs patient monitoring including pre, intra, post procedure, testing, medication administration, medical/surgical treatment and/or sedation
• Provides medication administration: (IM, PO, SQ, ID, IV topical, inhalation, ocular, vaginal, rectal) as per provider orders or protocols to patients in practice, following nursing rights of medication administration and organizational policies. This includes oxygen therapy.
• Facilitates medication refills by reviewing clinical information and preparing order for provider (prospective) authorization in EMR.
• Administers IV therapy: including blood, blood products, IV fluids, chemotherapy (with proper certification), IV medications via peripheral or central venous devices.
• Starts and discontinues peripheral IV lines.
• Maintains peripheral and central IV lines and manages complications with appropriate certification

Minimum Qualifications:
To qualify you must have a A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associates Degree in Nursing requires a minimum of one year experience in conjunction with plans to earn a baccalaureate degree required. Current Registered Professional Nurse in New York licensure is required. BCLS is also required. Emergency room or critical care nursing experience and membership in pertinent professional organizations preferred. Membership in pertinent professional organization preferred. Fluent written and spoken English language skills necessary. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required.

Required Licenses: Registered Nurse License-NYS, Basic Life Support Cert

Preferred Qualifications:
ACLS certification. Spoken mastery of a second language strongly preferred.

In this role, the successful candidate The Ambulatory Care Nurse will support the coordination, integration, communication, implementation and evaluation of nursing practice and clinical policies and procedures. • Performs medical and nursing treatments in accordance with plan of care, provider orders, and NYSBON scope of practice.
• Perform procedures, e.g.: hot & cold applications, placement/ removal of orthopedic appliances, casts, splints, suture and staple removal, urinary bladder irrigation w/ and w/o chemotherapy, insertion/removal/catheter care of urinary catheter, ear irrigation.
• Applies and removes dressings, and performs wound care
• Assists with Minor and Major Procedures
• Performs patient monitoring including pre, intra, post procedure, testing, medication administration, medical/surgical treatment and/or sedation
• Provides medication administration: (IM, PO, SQ, ID, IV topical, inhalation, ocular, vaginal, rectal) as per provider orders or protocols to patients in practice, following nursing rights of medication administration and organizational policies. This includes oxygen therapy.
• Facilitates medication refills by reviewing clinical information and preparing order for provider (prospective) authorization in EMR.
• Administers IV therapy: including blood, blood products, IV fluids, chemotherapy (with proper certification), IV medications via peripheral or central venous devices.
• Starts and discontinues peripheral IV lines.
• Maintains peripheral and central IV lines and manages complications with appropriate certification

Communication and Message Management
• Ensures clinical in-baskets are managed and encounters closed in a timely manner.
• Receives, documents, and reports critical lab values as indicated
• Relays normal and abnormal clinical results, follow up, and explanation to patients, as directed by provider.
• Performs preliminary review of results for prioritization of urgent findings, then notifies provider for final verification.

Farmingdale State College invites applicants for the Pre-Health Professions Advisor position. This professional will advise matriculated students interested in pursuing admission to various healthcare programs, and provide support and assistance for the application process to professional schools such as Medical, Dental, Optometry, Veterinary, Physical Therapy, Occupational Therapy, etc. The advisor must be well versed in admissions requirements of these and other programs. This professional position requires knowledge of current developments in MCAT, DAT, GRE, and other admissions examinations and is responsible for recommending resources for test preparation. This position will also support the Office of Pre-Health Professions Advisement with assistance with campus and local events and mock interviews. Excellent record keeping is a must for this position.

This position may require the availability to work evening hours as needed to meet operational requirements.

Job Requirements:

MINIMUM QUALIFICATIONS:

  • Master's degree in a science area (biology, chemistry, physics, etc.), counseling, or related field.

  • A minimum of two years experience as an academic or pre-health advisor.

  • Prior work experience with statistics and record keeping.

PREFERRED QUALIFICATIONS:

  • Prior committee experience with faculty in academic departments.

  • Excellent organizational and collaborative skills, with a team-oriented work ethic.

Additional Information:

This is a full-time UUP position.

  • For the first 10 working days, beginning Tuesday, September 9, 2025, and ending Thursday, September 18, 2025, applications will be limited to UUP Farmingdale.

  • CLOSING DATE FOR RECEIPT OF APPLICATIONS: Wednesday, October 8, 2025

  • SALARY: $57,151 + $,000 in downstate location pay = 61,151 Total Compensation.

Optum Urgent Care of Huntington has an immediate opening for a friendly, patient focused and detailed oriented Patient Care Coordinator/Medical Assistant to join our team. The Patient Care Coordinator/Medical Assistant is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.

Schedule: Office is open seven days a week. This role would consist of a 37.5-hour work week, between the hours of 7:00 am to 10:00 pm including holidays. The schedule will be determined by the supervisor upon hire. (tentative schedule is Monday 7:00 am to 10:00 pm, Tuesday 7:00am to 3:00pm, Friday 2:00pm to 10:00 pm and Saturday 7:00am to 3:00 pm)

  Primary Responsibilities:

  • Greets patients, family members and guests with a friendly smile
  • Assists patients with signing in and completing registration
  • Schedule appointments for new or established patients
  • Ability to be cross trained as a medical assistant
  • Sends patient records to primary care physician
  • Takes vital signs, performs EKGs and assists providers with patient care
  • Prepares exam rooms for patients and documents patient encounters in Epic
  • Performs lab testing and quality assurance
  • Prepares and tracks specimens sent to lab
  • Collect copays
  • Obtain referrals
  • Answers incoming phone calls in a courteous and prompt manner; directs calls and takes messages
  • Enters and scans patient's demographic and insurance information into Epic
  • Verifies insurance eligibility and benefit information; informs patients of insurance benefits
  • Assists physician and clinical staff by coordinating follow up care for patients; schedules referral appointments for patients and obtains authorization for diagnostic testing
  • Sends patient records to primary care physician
  • Ability to be cross trained as a medical assistant
  • Greets patients, family members and guests with a friendly smile
  • Assists patients with signing in and completing registration
  •   Demonstrate respect for patients and Optum staff by presenting a friendly, courteous manner at all times and maintaining patient confidentiality
  • Enters and scans patient's demographic and insurance information into Epic
  • Performs other tasks as assigned

Required Qualifications:

  • High School Diploma/GED (or higher)
  • 1+ years of experience working in a medical front office position performing duties such as scheduling appointments, checking patients in/out, insurance verification, collecting co-pays, and maintaining medical records
  • Intermediate level of computer proficiency (be able to work with multiple monitors at one time)
  • Ability to travel to other offices if needed for coverage
  • Ability to be cross trained as a medical assistant OR have your certification as a medical assistant presently

Preferred Qualifications:

  • Experience working with an electronic health record (EPIC)
  • Knowledge of Medical terms
  • Knowledge of insurance carriers and managed care plans  
  • Knowledge of Phlebotomy
  • Bi-Lingual English and Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
