The Community Health Outreach & Enrollment Specialist enhances health literacy, expands healthcare access, and connects the underserved communities to vital resources. This role conducts outreach, provides health education, and screens patients for eligibility in programs like Medi-Cal, Covered California, FPACT, CHDP, Ryan White, and other financial assistance potions. The specialist also assists with enrollment, explains program benefits, and ensures patients receive the coverage needed for continued care. Working collaboratively with TrueCare departments and community partners, this role strengthens community connections and promotes long-term health access.

Required Qualifications:

  • Bilingual in English and Spanish is required.
  • Associate degree in healthcare or related field or equivalent work experience.
  • At least two (2) years’ experience in health care or related field performing outreach, education, case work, and/or customer service to diverse populations.
  • Knowledge of local community-based agencies.
  • Availability to work regularly outside of normal shifts (i.e., 8am to 5pm).
  • Public speaking, computer literacy, including internet navigation and research skills, and proficiency with the Microsoft Office suite, including Outlook, Word, Excel, and PowerPoint.

The primary duties and responsibilities of the role are:

* Under technical direction and within standard limits and authority provide clerical support to claims adjusters to facilitate timely and accurate management of commercial claims.
* Update new and existing claims in claims database and contact brokers as needed.
* Screen all incoming phone calls, assess and assign out to proper party.
* Prepare written correspondence
* Print attached backup documentation/invoice and mail checks.
* Electronic and paper filing as needed.
* Determining coverage and adjuster assignment.
* Investigating the claim - this requires calling the claimant, insured

* Technical information gathering through ordering reports, contacting police departments for vehicle/ equipment recovery.

+ Conduct PACE required nursing assessments (minimally every 6 months), educate participants and their support network on acute, chronic, and end-stage conditions, and perform urgent and routine virtual or in-person follow-up visits as determined by participants' risk stratification and needs, and as a direct extension to the PCP

+ Develop and implement a discipline specific plan of care (POC) in collaboration with the IDT, participants, families, and/or points of action, to ensure the delivery of high quality, value-based, holistic care, capturing their assessments, findings, relevant information, and respective actions through timely and compliant documents in electronic health records

+ Coordinate the care for select participants with highest level of complexity, including those in the hospital, ER, and skilled facility, with the appropriate clinical team members to ensure seamless transitions of care, participant satisfaction, and to prevent unfavorable outcomes, inappropriate utilization of resources, and unnecessary hospitalizations

+ Act as the point person for triage, focusing on maximizing WelbeHealth's internal ability for ER diversion though walk-in clinic management and clinical escalations from the Welbe Advocate Hub

+ Dispense ordered medications to participants while in the center or clinic, and reinforce education for participants regarding their medications

+ Act as a coordinator of care, on behalf of participants and with other health care personnel, with evaluation and follow up on patient care measures (e.g., dentures, glasses, personal care, procedures, labs, referrals, or transportation)

**Job Requirements:**

+ Graduate of an accredited school of nursing

+ Unencumbered RN license

+ Minimum of two (2) years of nursing experience in a clinical setting with a frail or elderly population

+ Highly motivated, self-directed, able to execute tasks in a quickly changing environment and can make sound decisions in emergency situations

+ Excellent clinical, organizational, and communication skills in settings with seniors, their families, and IDT members

+ Ability to travel up to 100% of the time, multi-state support depending on the needs of the business as directed by Director of Operations; travel expenses covered as according to company travel and expense policies

The Work.

The UKG (Legacy Kronos) Advanced Scheduler Consultant for Healthcare works closely with teammates to ensure the maximum value of our clients' workforce management investment is achieved. The UKG (legacy Kronos) Advanced Scheduler Consultant for Healthcare, leads large, complex workforce management strategic and implementation engagements and manages these projects to successful completion.

Job Responsibilities

* Provides advanced-level knowledge of the UKG Dimensions or Workforce Central (Legacy Kronos Workforce Dimensions or Kronos Workforce Central) Advanced Scheduler module, specifically as it relates to clients in the healthcare industry


* Leads all aspects of workforce management configurations, implementations, modifications and upgrades of time and attendance initiatives including gathering and defining requirements, gap analysis, design, development and support


* Participates in and contributes to pre-sales and sales strategies


* Facilitates complex working sessions for both internal and client teams, including defining strategic objectives and tailors these to meet client-specific needs as necessary


* Creates detailed functional and technical design documents, including test plans, test cases, user training documents and implementation documentation


* Analyzes complex data or facts and summarizes and presents findings in a compelling way


* Provides thought leadership to develop new or improved processes, methodologies, systems, tools and/or services to enhance clients' operating environment based on practical experience, optimal outcomes and best practices


* Provides mentorship and knowledge transfer to our consulting base


* Attends to administrative expense tracking and time keeping duties required for billing

Qualification

Lung transplant patients at Cedars-Sinai have access to all of the services and programs for which the Medical Center is renowned. These include the S. **Essential Job Duties** **:**

+ The Lung Transplant Coordinator is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services-from initial referral to follow-up care.

+ Serves as the primary coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff.

+ Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community.

+ Collects/documents data to meet clinical, financial, and regulatory requirements.

+ Participates in the development of guidelines, protocols, pathways, and other performance improvement and research activities.

+ Serves as a patient advocate throughout the patient care process.
