As a Medical Records Specialist, you will play a key role in managing patient health information across our clinics. You'll ensure that all requests for records, documentation, and data entry into the EMR system are processed efficiently and accuratelyall while upholding the highest standards of patient confidentiality and care coordination.

Distribute and manage incoming requests for Protected Health Information (PHI) via fax, phone, email, and in-person

Scan, fax, mail, or copy patient records as requested

Research and process missing charges, incoming mail, and chart reviews

Handle incoming calls professionally and direct communications to the appropriate team

Log and track requests for medical records using internal systems (including Excel spreadsheets)

Collaborate with providers, community partners, and internal teams to ensure accurate and timely recordkeeping

Support office operations by managing supplies and other administrative duties as needed

Ensure compliance with HIPAA and internal privacy policies

High School Diploma or GED

13 months of healthcare, clerical, or administrative experience

Strong attention to detail and organizational skills

Ability to work both independently and as part of a collaborative team

Proficiency with computers, document handling, and basic office equipment

Excellent communication and customer service skills

Familiarity with EMR systems

Understanding of medical terminology and document types

PACT (Program for Assertive Community Treatment) is an intensive and highly integrated approach for community mental health service delivery. The multidisciplinary team shares a caseload of individuals who experience symptoms of the most serious forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.

Essential Duties & Responsibilities:

• Collaborate with a multi-disciplinary team including mental health professionals, nurses, prescribers, peer counselors, SUD specialists, and vocational staff.
• Deliver recovery-oriented services such as case management, treatment planning, resource assistance, crisis intervention, and group/individual services.
• Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
• Assist clients in the development of healthy coping mechanisms in individual or group settings.
• Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
• Provide ongoing support to clients in their homes and community environments.
• Participate in the 24/7 on-call rotation and deliver after-hours support as needed.

This position requires regular driving and mobility in community settings.

Position Summary

Responsibilities

• You will conduct screenings including psycho-social assessments, assess suicidal risk and provide appropriate interventions
• You will assist patients with orientation to the department and provide patients and families with psycho-education
• You will provide person-centered counseling, advocacy, and case management for patients
• You will develop treatment plans which clearly identify the patient's voice, treatment strategies, and specific outcome measures
• You will meet agency documentation standards and authorization requirements of contracts

Requirements

• A bachelor's degree in mental health related field of study required
• Five (5) years of experience working under a licensed Mental Health Professional required
• Counselor Agency Affiliated Registered
• Driver License/Abstract
• Those who meet the definition of a Mental Health Professional under RCW may be considered
