The ANCC is a Registered Professional Nurse with broad knowledge and skills who participates in performance improvement strategies to improve the patient experience, and serves as a resource for nursing staff in the delivery of quality care based upon nursing theory, research and approved Department of Nursing and institutional standards.

Responsibilities

PATIENT CARE
1. Acts as a clinical leader to support the daily functioning of the unit on a shift basis.
2. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
3. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
4. Integrates relevant assessment and intervention skills for the delivery of nursing care.
5. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
6. Coordinates and prioritizes all aspects of patient care including teaching, rounding, coaching, and planning "after hospital care" and delegating to others as appropriate.
7. Educates patients and caregivers, while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease.
8. Communicates effectively and professionally with patient, family and all members of the Interdisciplinary Patient Care Team.
9. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively.

PATIENT EXPERIENCE
1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration.
2. Considers the patient's values, preferences, expressed needs and knowledge in all aspects of care.
3. Establishes and maintains a therapeutic relationship with the patient and family.
4. Uses evidence-based practices to increase understanding of patients' perceptions of care.
5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed.
6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions.
7. Supports staff use of evidence-based practices to increase understanding of patients' perceptions of care.
8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps.

QUALITY AND SAFETY
1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served.
3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures.
4. Engages in formal and informal peer and institutional review processes.
5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies.
6. Contributes to quality and safety practices and required compliance measures.
7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients.
8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol.
9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives.

OPERATIONS
1. Coordinates nursing care on a shift basis within a specific unit using the Care Delivery Model of Modified Primary Nursing.
2. Assigns staff for the oncoming shift and recommends need for additional staff as necessary.
3. Coordinates with Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure that supplies are appropriately utilized and inventory is sufficient for patient care.
4. Recommends the progression of new employees within their unit based orientation.
5. Provides feedback to members of the nursing staff regarding adherence to job obligations, ensure and regulatory requirements in consultation with the Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or designee.
6. Assists in preparing time and staffing schedules in accordance with unit needs, patient care and contractual obligations.

Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.

The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. *To help support continuity of operations and care, staff selected to work in the OTxHU may also be required to work in CHS locations within the jails. Duties & Responsibilities

Position Overview

Under the supervision of the Assistant Director of Nursing, the Staff Nurse will collaborate with physicians to provide patients with care, education, and support. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. Educate patients on health maintenance and disease prevention or provide case management. Coordinating and assigning duties to other health care personnel, such as licensed practical nurses, nursing aides, behavioral health aides and patient care associates.

Responsibilities

1) Comply with procedures, regulations, and rules in maintaining medical records

2) Monitor, record and report symptoms and changes in patients' conditions.

3) Record patients' medical information and vital signs.

4) Modify patient treatment plans as indicated by patients' responses and conditions.

5)Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.

6) Monitor all aspects of patient care, including diet and physical activity.

7) Direct and supervise less skilled nursing/health care personnel, or supervise a particular unit on one shift.

8) Prepare patients for, and assist with, examinations and treatments.

9) Observe nurses and patients to ensure that proper nursing care is provided.

10) Instruct individuals, and other groups on topics such as health education, disease prevention and childbirth, and develop health improvement programs.

11) Prepare rooms, sterile instruments, equipment and supplies, and ensure that stock of supplies is maintained.

12) Administer local, inhalation, intravenous, and other anesthetics.

13) Provide health care, first aid, immunizations and assistance in convalescence and rehabilitation.

14) Conduct specified laboratory tests.

15) Direct and coordinate infection control services, advising and consulting with specified personnel about necessary precautions.

16) Perform administrative and managerial functions, such as taking responsibility for a unit's staff, and planning.

17) Provide or arrange for training/instruction of auxiliary personnel.

18) Refer patients to specialized health resources or community agencies furnishing assistance.

19) Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing.

20) Work with individuals, groups, and families to plan and implement programs designed to improve the overall health of communities.

21) Triage nurse works to assess patient needs and provide clear patient information and education providing the patient with clinical advice on the management of their care

22) Engage in research activities related to nursing.

23) Interacting with Computers Using computers and computer systems, enter data, or process information.

24) Documenting/Recording Information Entering, transcribing, recording, storing, or maintaining information in written or electronic form

25) Inspecting Equipment, Structures, or Material Inspecting equipment, structures, or materials to identify the cause of errors or other problems or defects

EMERGENCY RESPONSE
  • Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, and burns as well as recovering post-operative patients
  • Evaluate a patient's condition in order to determine the right course of treatment
  • When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc.
  • Monitor patient's condition and keep stable
  • Communicate with interdisciplinary team for the care of patients.
  • Aid in the preparation of the transfer of patients to the emergency units and/or hospital floors
  • Document medical treatment given to the patient
  • Keep the equipment clean after use; check and replace damaged and used supplies.
  • Collaborate with pharmacy to replenish supplies.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. WORKING CONDITIONS

Because patents require round the clock care, working hours include days, nights weekends and holidays. RNS are required to stand for long periods of time. RNs may face hazards from exposure to chemical and infectious diseases. In addition, they have to treat patients that are confused, irrational, agitated and/or uncooperative

Correctional Health Services

The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD).

Responsibilities:

Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.

Responsibilities Include:
  • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs
  • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
  • Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
  • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
  • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
  • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.
  • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
  • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist.
  • Provide clinical oversight for psychiatric nurse practitioner trainees, including appropriate direct supervision and documentation of evaluation and feedback.
  • Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director, or Chief of Service for Psychiatry, or their respective designees.
  • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certification to practice as a Psychiatric Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

  1. A valid New York State license and current registration to practice as a Registered Professional Nurse and,
  2. Current certification as a Nurse Practitioner issued by the New York State Department of Education and,
  3. A current certificate issued by the State Education Department to prescribe medication, and,
  4. Two years satisfactory experience as a Nurse Practitioner, or two years satisfactory experience as a Registered Professional Nurse, eighteen months of which must be as a Nurse Practitioner
  5. At least 3 years of hospital experience both as RN & NP in Behavioral Health
  6. An equivalent and satisfactory combination of education and experience may be considered

Additional Preferences
  • Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis.
  • Demonstrate advanced knowledge of psychopharmacology as it pertains to the patient population being treated and writing prescriptions.

Additional Salary Compensation

Departmental Preference : Strongly prefer National Board Certification in specialty in which you are assigned.
