The role you'll contribute:

The Transition Care Coordinator is responsible to identify high risk patients on admission, target risk specific interventions, assess patient's needs including post hospital needs and services, implement interventions in order to support quality care and meet patient's needs across the continuum. The individual will work closely with interdisciplinary team members within the organization and professional staff outside of the organization to ensure delivery of care coordination and transition across the continuum.

POSITION SUMMARY
The Behavioral Health Technician is responsible for performing a wide variety of patient care and coordination activities as directed by the Clinical Director. The primary purpose of this role is to provide supervision for our clients and to ensure their safety while building a rapport with them. This includes providing assistance to each client in working with a group and in handling individual problems; responsibility for setting limits for behavior; creating documentation of the client's progress; identifying the possible need for professional services and communicating such findings to professional staff.

The Healthcare Manager is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. + Conducts N-95 respirator fit testing in accordance with OSHA standadrds 29 CFR 1910.134 and AdventHealth policy and procedures.

+ Appropriately relays discharge and follow up instructions/care to the employee utilizing approved after-care instruction tools.

+ Participates in AdventHealth compliance initiatives and training as needed.

+ Supports all Employee Health services such as return to work, Covid-19 employee line.

+ Follows CDC isolation guidelines, infection control practices, demonstrates appropriate use of PPE, performs handwashing according to policy and enforces safety regulations.

+ Supports quality standards and initiatives set by the department.

Qualifications Education: - Diploma/Associates Degree Licensure/Certification/Registration: - Florida Registered Nurse License - BLS - ACLS - PALS Special Skills/Qualifications/Additional Training/Experience Required: - Must be able to read, write, speak and understand English - 2 years of recent ED experience highly preferred
