The role of the ER Care Manager is to increase patient through put to the most appropriate level of care while facilitating interdisciplinary care across the continuum. The ER Care Manager will assist in identifying patients appropriate for Admission, Observation or other patient care status. The ER Care manager will function as a liaison between attending physicians, Physician Advisors, local CCRCs, SNFs, PCFs, patients, families and community. The role includes the management of social issues that impact patient classification, discharge from the emergency room and ongoing medical management of patient care needs. The ER Care Manager will assist in identifying physician and staff documentation opportunities to support Quality and Pay for Performance indicators. Goal is to collaborate with patients, families and health care providers to promote health and manage illness in a cost effective and quality driven manner.

Three years of acute care experience in Care Management, Discharge Planning or Utilization management preferred. Basic knowledge of Word, Excel, and Power Point desired.
Demonstrates ability to coordinate care and utilize level of care criteria (InterQual, , etc.). Strong knowledge ofManaged Care, Medical Assistance and Medicare reimbursement required. Ability to perform duties and documentation per departmental, DNV, PA Department of Health and other healthcare requirements. Working knowledge of PA. Laws impacting health care delivery, i.e. Advance Directives, Guardianship, Abuse and Neglect and of community resources.
Demonstrates knowledge and understanding of physiological and psychosocial implications to enhance the care of individual patients. Demonstrates ability to correlate medical necessity in achieving financial and quality of care outcomes. Possesses strong knowledge of DRG's, LOS, variance analysis, and discharge planning related to the utilization of health care resources. Acute care and long term care knowledge base required.
Works independently. Utilizes time management techniques in accomplishing assigned tasks. Recognizes patient and/or family level of understanding relative to medical information and communicates appropriately. Possesses ability to negotiate and communicate with other departments, physician practice groups, 3rd party reviewers and outside facilities. Exhibits professional written and verbal communications that are accurate and concise. Possesses strong leadership skills and interpersonal skills.

The Sex Offender Clinician is involved in working with patients, both male and female population, classified to Sex Offender treatment at the Delaware Department of Corrections facilities.
SEX OFFENDER TREATMENT PROGRAM CLINICIAN ESSENTIAL FUNCTIONS:
- Performs group therapy interventions and individual treatment planning for clients.
- Performs Sexual Risk Assessment Interviews and writes Sexual Risk and Needs Assessments.
- Completes Discharge Summaries for client’s post-treatment participation.
- Assists in planning and implementing the goals and objectives of programs and projects.
- Provides the necessary preparation of documentation, necessary records, treatment notes, and reports.
- Write documentation and assessments using clinical language.
- Attends trainings and meetings as required and works towards completing Sex Offender Specific trainings to be credentialed on the Delaware Sex Offender Management Board.
SEX OFFENDER TREATMENT PROGRAM CLINICIAN MINIMUM REQUIREMENTS:
- Must have a master’s degree in psychology, social work, or counseling.
- Licensed (i.e., LCSW, LPCMH), or actively working toward a license. Note - a LMSW is not considered a clinical license.
- Must obtain licensure testing within 6 months of completing the required number of supervised hours and actively work towards obtaining credentialing with the Delaware Sex Offender Management Board (DE-SOMB).
- Travel to specific locations to meet schedule needs.
SEX OFFENDER TREATMENT PROGRAM CLINICIAN PREFERRED EXPERIENCE:
- Experience providing clinical therapy to individuals who have engaged in sexual offending behavior or individuals who have experienced sexual harm.
- Experience with Sexual Risk Tools such as Static99r, STABLE-2007, and ISORA.
- Experience with implementing Cognitive Behavioral Therapy in a therapeutic setting.

The RN develops and supervises the implementation of a resident's individualized service plan, observes the resident's condition, and communicates to the Physician and family/responsible party as needed. The RN oversees the resident's care delivered by the Home Support team of LPN's, Aides, and Home Care workers and companions.

Compensation: $41/hr. Weekday, day-time hours with occasional (compensated) on-call. Average of 20/hrs week. Homecare - Oasis- Experience Required.

How you will make an impact
* Partnering and collaborating in the initial evaluation to assess the residents' needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitioner
* Communicating any changes in the resident's condition
* Ensuring that the resident's individualized plan is implemented and followed by the Home Support agency team of providers
* Delivering nursing care as outlined in the plan of care and according to acceptable nursing standards
* Participating in care conferences and other resident-related meetings
* Participating in the orientation and training of other agency nurses as well as training any Aide staff development activities
What you will need
* Must have a minimum of 6 months' experience as an RN
* Experience in caring for seniors and/or senior with cognitive impairment preferred
* Basic computer skills required including experience with Microsoft Office, internet and web applications
* Experience with an electronic medical record is preferred
