Additional Information:
* The externship will begin on December 15, 2025.
* This is a paid, year-round position working 12-hour variable shifts (7a to 7p or 7p to 7a).
* A minimum of four shifts per month is required (two of the four shifts will be weekend shifts) typically equating to one 12-hour shift per week.
* The position is PRN and not eligible for benefits.
* Nurse Externs selected for a position will undergo a two-week, full-time (day shift) orientation program to ensure preparedness for working on the unit.
Under general supervision and in accordance with the policies, procedures, and guidelines established within the organization, unit, and the Nurse Practice Act, the Nurse Extern works with an RN to implement patient care based on the patient's age, and maintain and upgrade his/her skills based on the patient population served, the type and nature of care provided, and advances in health care technology. The Nurse Extern is responsible for communicating learning needs to the nurse preceptor. The Nurse Extern will seek out a variety of learning experiences and is accountable, under the direction of an RN, for setting priorities, meeting deadlines, and promoting the achievement of patient outcomes as defined in the nursing care plan, as follows: utilizing the nursing process of assessment, planning, implementation and evaluation; coordinating the care provided by other professionals and unlicensed staff, including delegation and coaching in selected situations; providing selected aspects of physical care within the scope of professional nursing practice as appropriate given the patient's level of acuity and complexity of care required; and documenting and communicating during each tour of duty the patient's response to care provided, consumption of resources and anticipated needs. Employees providing direct patient care must demonstrate competencies specific to the population served.
High school diploma or general education degree (GED) required.
Applicants must be enrolled in an accredited school of nursing and be within one year of graduation, with an expected graduation date in Spring 2026 or Winter 2026.
The Nurse Extern must complete the individual unit requirements for orientation which includes Hospital, Core, and Department specific orientation based upon the unit and experience level (the Nurse Extern is considered to be in orientation during the full time of employment as a Nurse Extern).
BLS Certification
* Ability to read, interpret, and analyze data from various computer systems and equipment in order to set an appropriate plan of action.
* Satisfactory completion of a pharmacology course or satisfactory performance on a pharmacology medication test.
* Must be able to constantly communicate in a clear and caring manner, both verbally and in writing.
Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time.
Involves considerable standing or walking, regular lifting of light-weight objects (i.e., 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients. Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.
Collaboration with an RN preceptor the Nurse Extern performs the following:
* Documents nursing history and physical/emotional needs/strengths for assigned patients.
* Develops a plan of care for assigned patients, based upon assessment.
* Manages care plan for assigned patients by organizing and delegating therapeutic patient care interventions to be implemented and evaluating effectiveness of care given by self and others.
* Takes temperature, pulse, blood pressure, and other vital signs to assess condition of patient and detect deviations from normal.
* Observes patient, records significant conditions and reactions, implements appropriate interventions and/or notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
* Administers prescribed medications and treatments in accordance with approved nursing techniques and policies.
* Explains procedures and treatments to patient to gain cooperation, understanding, and allay apprehension.
* Prepares equipment and aids physician during treatment and examination of patient.
* Maintains awareness of comfort and safety needs of patient.
* Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Physician, and/or hospital policy and nursing care plan, including patient and family instruction.
* Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
* Manages discharge planning and preparation for assigned patients.
* Manages personal, unit, and hospital resources effectively.
* Rotates among clinical services of institution within cluster (i.e., obstetrics, surgery, orthopedics, outpatient and admitting, pediatrics, etc.).
* Prepares rooms, appropriate instruments, equipment, and supplies for procedures of surgery (as assignment indicates).
* Participates in department or unit performance improvement activities.
* Assumes responsibility for professional development.
* Develops and provides a positive work climate and the overall team effort of the department.
* May act as a clinical resource and role model to Unit Clerk, USAs, PCAs, and LPNs.

Western Michigan University Homer Stryker M.D. School of Medicine (WMed) is seeking an On-Call Medical Assistant in our Primary Care Clinic. This career allows you to actively participate in coordination of patient flow and services with a focus on patient centered care. As a medical assistant for WMed, you will collect patient information related to the patient visit or procedure. Assisting with patient care and administering approved medication is a major part of the day. Assisting physician and team members with clerical, environmental, and organizational responsibilities as well as optimizing education for residents and students in department rotations are some of the unique and satisfying aspects of this position. The ideal candidate will have a positive outlook and will be eager to be part of a fast-paced work environment. WMed requires all employees to have proof of initial Covid-19 vaccination series and annual influenza vaccine.
We offer a Monday Friday schedule between the hours of 7 a.m. and 6 p.m. (nights and weekends off!). Our full time employees receive 8 full-day and 2 half-day paid holidays, an 8 hour preference holiday, 8 hours of paid volunteer time per year, and an extremely generous Paid Time Off (PTO) accrual as well as an annual wellness reimbursement of $300. Tuition reimbursement is available within the first year of full time employment.

Job Description
The **Field Care Coordinator- HIDE SNP** is an essential element of an Integrated Care Model and is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that the person receiving LTSS receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into the community.
This position is Field Based with a Home-Based office. The expected travel time for member home visits is typically 75% within a 50-mile radius and/or 50-minute drive from your home pending business needs.
If you reside in or near **Kalamazoo County, MI or surrounding area** , you will have the flexibility to telecommute* as you take on some tough challenges.
**Primary Responsibilities:**
+ Develop and implement care plan interventions throughout the continuum of care as a single point of contact
+ Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
+ Advocate for persons and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
+ Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care
+ Identifies problems/barriers to care and provide appropriate care management interventions
+ Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
+ Provides resource support to members for local resources for services (e.g., Children with Special Health Care Services (CSHCS), employment, housing, independent living, foster care) based on service assessment and plans, as appropriate
+ Manage the person-centered service/support plan throughout the continuum of care
+ Conduct home visits in coordination with the person and care team
+ Conduct in-person visits, which may include nursing homes, assisted living, hospital or home
+ Gathers, documents, and maintains all member information and care management activities to ensure compliance with current state and federal guidelines

**Required Qualifications:**
+ Must possess one of the following
+ Current, unrestricted independent licensure as a Registered Nurse (RN) in state of Michigan
+ Master's degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
+ Bachelor's degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
+ 2+ years of experience working within the community health setting in a health care role
+ 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH (e.g., food banks, non-emergent transportation, utility assistance, housing/rapid re-housing assistance, etc.)
+ 1+ years of experience working with persons with long-term care needs and/or home and community-based services
+ 1+ years of experience working in electronic documentation systems and with MS Office (Outlook, Excel, Word)
+ Access to reliable transportation and the ability to travel within assigned territory to meet with members and providers up to 75% of the time depending on member and business needs
+ Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
+ Ability to travel to Southfield, MI office for quarterly team meetings
+ Must reside within the state of Michigan
**Preferred Qualifications:**
+ RN or LMSW, LLMSW, LCSW
+ 1+ years of medical case management experience
+ Demonstrated experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
+ Experience in serving individuals with co-occurring disorders (both mental health and substance use disorders)
+ Experience with MI Health Link (MMP)
+ Experience working in Managed Care
+ Working knowledge of NCQA documentation standards

The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.

As a Sales Representative at Stryker, you will be at the forefront of promoting and selling our Emergency Care products, with a focus on Fire Emergency Medical Services solutions, to pre-hospital providers. You'll work with a diverse range of products, including Emergency Patient Transport, Defibrillators, Automated External Defibrillators (AED), and Chest Compression systems. By staying informed on industry trends, competitor activity, and regulatory shifts, you'll position Stryker's products effectively in the marketplace. **What you will do**
+ Continue experience in sales or clinical setting.
+ Promote and sell Stryker Emergency Care products to meet our customers' needs.
+ Achieve your assigned quota by taking a consultative approach to Pre-Hospital sales, marketing, clinical and demonstrations of all related Fire/Emergency Medical Services products and programs.

**What you need**
**Required:**
+ High school diploma and 5+ years of professional experience.
+ Valid driver license in the state of residence and a good driving record as you will be operating a company vehicle capable of transporting equipment.
**Preferred:**
+ Bachelor's Degree.
