The role of the ER Care Manager is to increase patient through put to the most appropriate level of care while facilitating interdisciplinary care across the continuum.  The ER Care Manager will assist in identifying patients appropriate for Admission, Observation or other patient care status. The ER Care manager will function as a liaison between attending physicians, Physician Advisors, local CCRCs, SNFs, PCFs, patients, families and community.  The role includes the management of social issues that impact patient classification, discharge from the emergency room and ongoing medical management of patient care needs. The ER Care Manager will assist in identifying physician and staff documentation opportunities to support Quality and Pay for Performance indicators. Goal is to collaborate with patients, families and health care providers to promote health and manage illness in a cost effective and quality driven manner.

RECRUITMENT REQUIREMENTS:

All candidates must excel in delivering a positive patient outcome, exhibit a commitment to quality, and have knowledge of population health and associated metrics. Candidates must also demonstrate critical thinking and have excellent communication (both written and oral) skills.  This role requires impeccable organizational skills and the ability to work and adjust to different environments.  Knowledge of Interqual and utilization review standards. Experience in clinical documentation within electronic medical records required. Experience in Care Coordination, Discharge Planning, Clinical Documentation and Utilization Review preferred. Strong verbal, written and leadership skills.

Motivated, self-starter, enthusiastic personality.

Three years of acute care experience in Care Management, Discharge Planning or Utilization management preferred.  Basic knowledge of Word, Excel, and Power Point desired.

Demonstrates ability to coordinate care and utilize level of care criteria (InterQual, , etc.). Strong knowledge ofManaged Care, Medical Assistance and Medicare reimbursement required. Ability to perform duties and documentation per departmental, DNV, PA Department of Health and other healthcare requirements. Working knowledge of PA. Laws impacting health care delivery, i.e. Advance Directives, Guardianship, Abuse and Neglect and of community resources.

Demonstrates knowledge and understanding of physiological and psychosocial implications to enhance the care of individual patients. Demonstrates ability to correlate medical necessity in achieving financial and quality of care outcomes.  Possesses strong knowledge of DRG's, LOS, variance analysis, and discharge planning related to the utilization of health care resources. Acute care and long term care knowledge base required.

Works independently.  Utilizes time management techniques in accomplishing assigned tasks. Recognizes patient and/or family level of understanding relative to medical information and communicates appropriately.  Possesses ability to negotiate and communicate with other departments, physician practice groups, 3rd party reviewers and outside facilities.  Exhibits professional written and verbal communications that are accurate and concise.  Possesses strong leadership skills and interpersonal skills.

LICENSE AND REGULATORY REQUIREMENTS:

Must have current RN licensure (BSN preferred)).

Adheres to the Case Management Society of America (CMSA) Standards, Nurse Practice Act as applicable.

Short Description

Assists interventionalists in all procedures within the scope of interventional Radiology and Vascular surgery to include but not limited to oncology, vascular and trauma.

Skill set must be maintained to handle supplies/instrumentation associated with interventional procedures. Must be able to assist physicians in the capacity of both a scrub and circulating technologist.
Maintains work area: Reports maintenance issues for equipment, monitors supplies and deficiencies.
Adheres to all Infection Prevention policies, utilizing the appropriate PPE needed.

Maintains licensure and BLS. Stays current and participates departmental training offered.

Demonstrates courteous behavior, positive attitude, etc. through polite communication with all co- workers/customers.
Understands and appreciates the need to provide good customer service, both internal and external.
Participates in clinical activities (i.e. assists with patient flow/care, helps to optimize throughput of patients via Radiology Transport).
Remains compliant of timekeeping rules: Avoids early, late and unauthorized entries. A Adheres to Departmental procedures, fulfills compliance with life safety, fire and hazardous waste, Radiation Safety, and Infection Prevention requirements.

Attends staff meetings and reviews daily huddles to keep current on department communications.

Experience Required

1-2 year of Interventional Radiology or Cardiovascular Interventional Technology preferred


Education Requirements

Graduate of AMA approved program for Diagnostic Radiology and therefore registry eligible. 

NJ DEP license required upon hire.


License/Certification Requirements
