Summary:
Coordinates the assessment, planning, and implementation of rapid intake of eligible patients referred for acute inpatient medical rehabilitation with the purpose of creating a "user friendly", internally cohesive and seamless system for admission.
Responsibilities:
1. Accesses cases to determine most effective and cost-efficient model for care. Performs concurrent and retrospective admission and continued stay reviews for private insurance companies, Medicare and Medicaid, in accordance with utilization review guidelines.
2. Reviews assigned patients' cases prior to, or upon admission, and determines most effective and cost-efficient model for care. Appropriately recommends eligibility for pathways, protocols, or guidelines that could improve patient care and utilization of resources. Effectively collaborates with and mobilizes resources and team efforts toward utilizing the optimal model, where possible.
3. Ensures that requirements with respect to admissions and length of stay are continually monitored, and that any deviations from established guidelines are identified and addressed with the appropriate physicians. Tactfully and effectively informs Manager of physicians' impact days and constructively offers more cost-effective alternative approaches and/or recommendations to minimize reoccurrences.
4. Effectively evaluates, documents, and coordinates patient's physical, psychological, spiritual, and educational needs while an inpatient, in preparation for discharge, and post discharge, in collaboration with other members of the health care team. Reviews and assesses patient's condition and maintains an up-to-date status of patient's medical condition to facilitate and expedite discharge. Identifies and documents high risk areas and patients and consults with appropriate personnel regarding indicated special care needs, infection control and safety precautions.
5. Effectively serves as a patient/family resource and advocate in ensuring that their questions, concerns, and needs are addressed appropriately and in a timely manner. Effectively coordinates and documents the placement of patients into residential or rehabilitative settings, as needed, ensuring that administrative requirements are accurately and completely accomplished in accordance with procedure and time frames.

Summary:
Organizes and directs the delivery of patient care, and associated operations and personnel for a nursing unit. Summary:
Performs venous and capillary collection of blood specimens for analysis. May perform EKGs, waived and moderately complex testing with supervision. May perform pre-employment drugs of abuse screens. Performs various other support services for the laboratory as assigned. Monitors incomplete worklists on LIS throughout shift. May perform assay for ABG's and COOX samples with appropriate supervision. The phlebotomist II will have a certification in a phlebotomy registry.
Responsibilities:
1. Performs various clinical and technical functions in support of the laboratory to include, but not limited to:
· Collects venous and capillary blood samples from patients
· May performs EKGs if appropriate for job assignment
· Admits, orders and processes laboratory specimens for Nash Day Hospital, if needed.
· Performs specimen collections off-site, if needed.
· Performs POCT if trained
· Administers the correct procedure to the correct patient.
· Follows established procedures for the blood collection procedure, so that collection is successful on the first attempt, and without undue trauma to the site, when possible.
· Solicits more experienced/skilled person to collect blood from difficult patients after two failed attempts, watching other person's technique and asking questions, so that own skills are improved.
· Accurately and properly labels blood specimen tubes immediately after drawing them with the patient's full name, room number, date, time, medical records number, and initials.
· When performing EKGs, ensures that an appropriate history and/or examination data is taken, and that contraindicating conditions are identified and communicated to the physician prior to the initiation of the procedure.
· When performing EKGs, effectively adjusts and controls equipment to produce quality tracings or images. When performing EKGs, contacts interpreting physician as soon as possible upon detection of severely abnormal studies
· Is trained for POCT, performs correctly and accurately and is adequately trained.
· Observes patient condition and promptly reports unusual or adverse signs or responses to the appropriate person.
· Promptly and appropriately responds in accordance with procedure or established protocol in emergency or STAT situations.
· Conforms to established laboratory Safety and Infection Control Guidelines.
· Ensures that patient questions and concerns are satisfactorily addressed, notifying appropriate patient care personnel when this cannot be accomplished, prior to the initiation of the procedure.
· Required documentation and computer entry regarding the patient or the procedure is accurate, complete, legible, properly edited and includes appropriate terminology.
· Makes efficient use of supplies, so that usage correlates to amount needed, without undue waste.
· Gets a minimum of 4 hours of CME or hospital sponsored Inservice annually.
· Demonstrates knowledge and skills necessary to provide appropriate care based on the age of the patients served.
· Demonstrates ability to assess, provide and interpret age specific data.
· Selects and appropriately applies procedures, techniques and communications appropriate to the age and developmental status of patients.
· As assigned, provides instruction in the basic technical skills, and application of phlebotomy ¬ procedures, EKGs to Phlebotomist I's, students and new employees.
· Contributes to the technical development of specified personnel, by observing work, answering questions, explaining proper procedures, demonstrating techniques and intervening as warranted to correct deficiencies or help resolve problems.
· Effectively explains procedures and protocols, demonstrates procedures and techniques, and answers questions when assisting with the orientation of new personnel and students, as assessed through observation and feedback.
· Provides objective, substantiated feedback regarding educational program effectiveness, and new employee or student progress, when asked.
· Effectively accomplishes assigned audits on work results or systems, as assigned.
· Performs miscellaneous support services of a technical nature to include, but not limited to:
· Monitors uncollected list.
· Responds to Code Blue (cardiac arrest) calls or Neonatal Emergency calls.
· Assists in maintaining "nurse station collected specimens" computer system.
· Monitors the specimen uncollected list and ensures that test collections are completed in accordance with specified time frame.
· Carries out designated role during Code Blue or Neonatal Emergencies; responds immediately, picks up and ABGs, calls back results, returns to site of Code Blue or Neonatal Emergency until no longer needed.
· Follows established procedure for the computer entry, collection, identification and distribution of "nurse station collected specimens," when assigned.
2. Performs other duties as needed to include, but not limited to:
· Assists with routine work in various divisions of the laboratory under direct supervision.
· Cleans, organizes and restocks work area.
· Monitors, inventories, requisitions, receives, verifies and stores supplies.
· Purges expired tubes from inventory.
· Delivers lab reports to various locations in the facility.
· Relays lab results by telephone.
· Looks up reports for physicians.
· Retrieves and transports specimens;
· Put up supplies on units for Nursing patients;
· Keeps glove boxes stocked in all patient rooms and/or collection areas;
· Disposes needle disposal containers as they become ¾ full appropriately;
· Cleans stretchers and gets blankets for patients in ECC when working there.
· Follows instructions of Medical Technologist or Medical Laboratory Technician when assisting with routine work, asking questions as necessary to ensure understanding, before proceeding.
· Maintains work area in a clean and organized manner throughout the day, and upon conclusion of the shift, restocking supplies as needed so that depletions do not occur.
· Stays abreast of departmental activities and issues by attending departmental meetings and/or reading meeting minutes.
· Accomplishes routine and non-routine miscellaneous assignments in accordance with procedure or instructions, and time frames.
· Monitors uncollected lists, and takes appropriate action in response to findings.
· Follows established procedure for computer data entry, accessioning, collection, etc.
3. Additional Tasks for the Phlebotomist Lead Position:
These responsibilities will be performed and directed by the laboratory manager or laboratory director
+ Performing various phlebotomy functions including specimen tracking and processing.
+ Overseeing scheduling tasks.
+ Training phlebotomists.
+ Performing clerical and secretarial functions.
+ Ensuring that clients understand the procedure, potential risks, and instructions for care following the test.
+ Performing venipuncture on patients using sterile techniques and equipment to collect blood samples.
+ Providing coverage and travel to various sites to perform phlebotomy job duties.
+ Assisting in the supervision of a team of phlebotomists covering multiple Patient Service Centers - client sites.
+ Observing new employee performance and reporting observations to the supervisor.
+ Performing site inspections on a regular basis and accurately reporting all findings.
+ Providing continuous training to phlebotomy staff as directed.
+ Completing new hire and annual competency assessments when necessary.

Summary:
Provides routine direct and indirect patient care in a hospital, clinic or home health care setting. All work performed is under the direct supervision of a registered nurse or physician.
Responsibilities:
1. Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies.
