Primary Responsibilities:

* Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
* Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter, including visits to address changes in condition
* Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization
* Consults with Physician Leaders and coordinate with PCP in these decisions as available
* Comfortable with basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc. New hire and annual skills training provided
* Performs post discharge visits to ensure patient stability is consistent with their goals of care
* Participates in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
* Participates in clinical after hours on-call program on rotational basis

Required Qualifications:

* Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC)

• Keep up-to-date course syllabi and assist with new course development as needed.

ESSENTIAL JOB FUNCTIONS:

  • Partner with clinical, medical, nursing, and intake departments to support patients

  • Assists with admissions, room searches, performs rounds every 2 hours to ensure patient safety

  • Properly document inappropriate behavior or problem behaviors.

  • Monitor grounds for violations and unauthorized visitors.

  • Assists the Department supervisor in performance management.

  • In the absence of the department supervisor, facilitates team meetings as well as shift lead meetings.

  • Completes new patient admissions

  • Assists in developing work schedules and in obtaining coverage for shifts as needed.

QUALIFICATIONS:

  • Must be at Least 21 years old.

  • High school diploma/GED required

  • Current NC Driver License and acceptable driving history required

  • Knowledge or experience working with patients who are chemically dependent preferred.

  • Knowledge of 12 step recovery program preferred

  • Crisis Prevention and CPR Certification (offered on site)

  • Ability to work with male and female adult (18+) patients with chemical dependency and mental health disorders

  • Teamwork and excellent customer service

  • Associates or Bachelors degree in Human Services field

  • Substance use Addictions Certification

  • Supervisory experience, preferably in healthcare.

Job Description:

  • Perform health care institution and physician office coordination activities.
  • Establish relationships with health care institutions to facilitate and manage the transfer of patients to the home.
  • Coordinate patient services activities.
  • Coordinate health care institution and physician education activities.
  • Coordinate community education activities and attend community events.
  • Interact with Branch Management and other appropriate intra company contacts.
  • Assist with continuous recruitment activities to attract qualified staff.
Job Requirements:
  • Requires a bachelor's degree or LPN, RN, SP, LPT, MSW.
  • Prefer two years' experience working with home health, hospice, hospitals, physicians, HMO's, outpatient services or social services concerning public relations or office procedures.
  • Knowledge of Medicare/Medicaid home health and hospice regulations preferred.
  • If licensed, successful completion from an approved school of nursing, physical therapy, or speech therapy is required.
  • Ability to coordinate the transfer of patients to home health and hospice services, resulting in uninterrupted continued medical care.
  • Strong knowledge of home health and hospice services, comprehension of federal, state, and ACHC guidelines.
  • Effective communication skills (oral and written).
  • Strong interpersonal and telephone skills.
  • Ability to travel as necessary.
